حدود العلاج يجب أن تكون واضحة: الأكسجين يُسرّع طرح الغاز لكنه لا يُصلح كل أذى وقع في الدماغ والقلب. كما أن جهاز قياس الأكسجين على الإصبع قد يعطي قراءة طبيعية مطمئنة كذباً في هذا التسمم، وقيمة تحليل الكربوكسي هيموغلوبين لا تتناسب دائماً مع شدة الحالة، فالقرار سريري بالأساس. وقد تظهر أعراض متأخرة بعد أيام إلى أسابيع مثل النسيان وتبدّل المزاج وبطء التفكير واضطراب الحركة، ولذلك تُطلب متابعة عصبية بعد التعرّض الشديد. أما الأكسجين المضغوط في الغرفة الضاغطة فيُدرَس في حالات مختارة كفقدان الوعي أو الحمل أو الأعراض العصبية أو القلبية، ويُحدَّد بقرار طبي عاجل لا برغبة المريض.

والوقاية هي العلاج الحقيقي: لا تُشغّل صوبة فحم أو غاز في غرفة نوم مغلقة، وأطفئ الصوبة قبل النوم، واترك تهوية دائمة ولو صغيرة، وافحص المدخنة وخرطوم الغاز ومسخّن الماء سنوياً عند فنّي مؤهل، ولا تُدفّئ السيارة داخل مرآب مغلق. وكاشف أول أكسيد الكربون المنزلي وسيلة غير مكلفة تُنبّه قبل ظهور الأعراض.

## خطوات الإجراء

### 1. الإخراج إلى هواء نقي وطلب الإسعاف

أول علاج يبدأ قبل المستشفى: إخراج المصاب وكل من في المكان إلى الهواء الطلق، وطلب الإسعاف على 911 من الخارج. إن كان المصاب لا يتنفّس فيُبدأ الإنعاش القلبي الرئوي بتوجيه من مركز الإسعاف. لا يدخل أحد إلى مكان مشتبه دون حماية، لأن المنقذ يُصاب كما المصاب.

### 2. الأكسجين بتركيز عالٍ مبكراً

يُعطى الأكسجين بأعلى تركيز ممكن عبر قناع محكم في سيارة الإسعاف وفي الطوارئ، ويُكمَل بحسب الأعراض والفحوص. الأكسجين هو ما يُزيح الغاز عن الهيموغلوبين، ويستمر عادة ساعات، ولا يُوقف بمجرّد أن يشعر المريض بتحسّن.

### 3. التقييم والفحوص

يُقاس مستوى الكربوكسي هيموغلوبين في الدم مع غازات الدم والسكر، ويُجرى تخطيط للقلب وفحص إنزيماته لأن الغاز يُجهد عضلة القلب، ويُطلب فحص حمل عند النساء في سن الإنجاب، وقد تُطلب صورة للدماغ إن كان هناك تشوّش أو نقص عصبي. يُسأل عن نوع المصدر ومدة التعرّض وعدد المصابين في المكان.

### 4. علاج المضاعفات والمراقبة

تُعالج المضاعفات بحسب ظهورها: سوائل وريدية عند الحاجة، علاج التشنّج، دعم التنفّس، مراقبة القلب لمن لديه ألم صدر أو تغيّرات في التخطيط، وتصحيح الحرارة إن كان التعرّض في حريق. تستمر المراقبة عادة عدة ساعات، وقد يُقرَّر إدخال المستشفى في الحالات الشديدة أو عند الحوامل والأطفال وأصحاب أمراض القلب.

### 5. قرار الأكسجين المضغوط وفحص المصدر وبقية أفراد المنزل

يُقيَّم الطبيب حاجة الحالة إلى الأكسجين المضغوط في مركز مؤهل، وهو خيار لحالات مختارة لا لكل مصاب. وبالتوازي يُقيَّم كل من كان في المكان — حتى من يبدو بخير — ويُمنع الرجوع إلى المسكن حتى يفحص فنّي مؤهل المصدر ويُصلحه، ويُنصح بتركيب كاشف لأول أكسيد الكربون وتُرتّب متابعة عصبية عند اللزوم.

## Procedure steps

### 1. Move to fresh air and call for an ambulance

Treatment starts before hospital: move the patient and everyone else out into open air and call 911 from outside. If the person is not breathing, start cardiopulmonary resuscitation with guidance from the dispatcher. Nobody should enter a suspected space unprotected, because rescuers are poisoned just as easily as victims.

### 2. Early high-concentration oxygen

Oxygen is delivered at the highest achievable concentration through a tight-fitting mask in the ambulance and in the emergency department, and is continued according to symptoms and test results. Oxygen is what displaces the gas from haemoglobin; it usually runs for several hours and is not stopped simply because the patient feels better.

### 3. Assessment and investigations

Blood carboxyhaemoglobin is measured together with blood gases and glucose, an electrocardiogram and cardiac enzymes are obtained because the gas strains the heart muscle, a pregnancy test is requested in women of childbearing age, and brain imaging may be needed if there is confusion or a neurological deficit. The type of source, duration of exposure and number of people affected are all documented.

### 4. Treating complications and observation

Complications are treated as they appear: intravenous fluids when needed, seizure control, respiratory support, cardiac monitoring for anyone with chest pain or electrocardiogram changes, and temperature correction if the exposure occurred in a fire. Observation usually lasts several hours, and admission may be decided in severe cases and for pregnant women, children and people with heart disease.

### 5. Hyperbaric decision, source inspection and checking the household

The treating physician assesses whether hyperbaric oxygen at a qualified centre is indicated, which applies to selected cases rather than every patient. In parallel, everyone who was in the building is assessed even if they feel well, return home is deferred until a qualified technician inspects and repairs the source, a carbon monoxide alarm is recommended, and neurological follow-up is arranged when required.

## العناية بعد الإجراء

التزم بمدة الأكسجين والمراقبة التي يحدّدها الطبيب، ولا تطلب الخروج مبكراً لأن الأعراض قد تخفّ قبل أن يُطرح الغاز من الدم. لا تعد إلى المكان قبل أن يفحص فنّي مؤهل المصدر ويُصلحه، وركّب كاشفاً لأول أكسيد الكربون في المنزل. توقّع تعباً وصداعاً بضعة أيام، وتجنّب قيادة السيارة والأعمال الخطرة حتى يسمح الطبيب. \*\*راجع فوراً عند: عودة الصداع أو الدوخة أو الغثيان، ألم في الصدر أو ضيق نفس أو خفقان، تشوّش أو نعاس شديد أو تشنّج، ضعف أو تنميل أو صعوبة في الكلام أو المشي، أو ظهور نسيان وتبدّل سلوك وبطء تفكير خلال الأسابيع التالية.\*\* واعرض الحامل على طبيبها لتقييم الجنين، واعرض الأطفال حتى لو كانت أعراضهم خفيفة، وتابع تقييماً عصبياً إذا فقد المصاب وعيه أثناء التعرّض.

## About this service

Carbon monoxide poisoning is an emergency caused by inhaling a gas that has no colour, smell or taste, produced by incomplete combustion of fuel: gas, kerosene, wood and charcoal heaters, water heaters, stoves and ovens, and car engines running in a garage or enclosed space. The gas binds to haemoglobin far more strongly than oxygen does, so it blocks oxygen delivery to the brain and heart even while breathing looks normal.

The symptoms are deceptive and mimic influenza or exhaustion: headache, dizziness, nausea, general weakness and poor concentration, progressing in more severe exposure to vomiting, loss of consciousness and seizures. Two clues separate poisoning from flu: several people in the same home feeling unwell at the same time, pets included, and symptoms that ease in fresh air and return on going back inside. Pregnant women, children and people with heart or lung disease are more vulnerable, and an unborn baby is affected more than the mother.

\*\*If you suspect it, do not wait and do not look for another explanation: get everyone out into fresh air, call emergency services on 911 from outside the building, and do not go back in to switch off the source or collect belongings.\*\* The mainstay of treatment is high-concentration oxygen, and the earlier it starts the sooner the gas clears from the blood. No home remedy neutralises the gas: opening a window and drinking milk, coffee or yoghurt is not treatment, and assuming that someone is safe because they are still awake and talking is a common and costly mistake.

The limits of treatment must be stated plainly. Oxygen speeds elimination of the gas but does not undo injury already sustained by the brain and heart. A fingertip pulse oximeter can give a falsely reassuring normal reading in this poisoning, and the measured carboxyhaemoglobin level does not always match the severity of the case, so decisions are primarily clinical. Delayed symptoms may appear days to weeks later, such as forgetfulness, mood change, slowed thinking and movement problems, which is why neurological follow-up is arranged after significant exposure. Hyperbaric oxygen is considered in selected cases such as loss of consciousness, pregnancy, or neurological or cardiac features, and that decision is an urgent medical one.

Prevention is the real treatment: never run a charcoal or gas heater in a closed bedroom, switch heaters off before sleeping, keep permanent ventilation even if small, have the chimney, gas hose and water heater checked yearly by a qualified technician, and never warm a car inside a closed garage. A household carbon monoxide alarm is an inexpensive device that warns before symptoms begin.

## Preparation

There is no preparation for this emergency, but what you do in the first minutes changes the outcome. Get the patient and everyone else out into open air and \*\*call emergency services on 911 before anything else\*\*; do not drive a confused or unconscious person yourself if an ambulance is available. Switch off the source only if it can be reached safely from outside, and never enter a suspected space unprotected. Open doors and windows and then leave; do not stay inside to ventilate. Take everyone who was in the building to the emergency department, even those who seem well, especially children, pregnant women and older adults. Tell the treating team what the source was and where it is, roughly how long the exposure lasted, whether anyone lost consciousness, vomited or had a seizure, whether pregnancy is possible, and about any heart or lung disease, epilepsy and current medicines.

## Aftercare

Complete the duration of oxygen and observation set by the treating team, and do not ask to leave early, because symptoms can settle before the gas has cleared from the blood. Do not return to the building until a qualified technician has inspected and repaired the source, and install a carbon monoxide alarm at home. Expect fatigue and headache for a few days, and avoid driving and hazardous work until your doctor agrees. \*\*Seek care immediately if you develop: recurrent headache, dizziness or nausea; chest pain, breathlessness or palpitations; confusion, marked drowsiness or a seizure; weakness, numbness or difficulty speaking or walking; or forgetfulness, personality change and slowed thinking over the following weeks.\*\* A pregnant woman should be reviewed for fetal assessment, children should be seen even with mild symptoms, and neurological follow-up is needed for anyone who lost consciousness during the exposure.
