ومن حدود العلاج التي يجب أن تُقال بوضوح: لا يوجد اليوم علاج يُنهي المرض، والعلاجات المعدّلة لسيره تعمل على المستقبل لا على ما فُقد سابقاً؛ والوظائف التي تأذّت بشكل دائم يتعامل معها التأهيل بالتكيّف وتقوية ما تبقّى لا بإعادتها كما كانت. كذلك خلوّ صورة الرنين من بؤر جديدة لا يعني أن المرض توقّف نهائياً، والعكس صحيح أيضاً: قد تظهر بؤرة بلا أعراض. ونقطة يُخطئ فيها كثيرون: ارتفاع حرارة الجسم بسبب حمّى أو حمّام ساخن أو جوّ حارّ قد يُظهر أعراضاً قديمة بشكل مؤقت، وهذا ليس نكسة جديدة ويزول مع عودة الحرارة إلى طبيعتها.

التشخيص والترشيح للعلاج يُحدَّدان بالفحص العصبي والتصوير والتحاليل لا بقراءة الأعراض على الإنترنت، لأن أمراضاً كثيرة تُشبه التصلب اللويحي في صورتها الأولى ويجب استبعادها قبل بدء علاج طويل الأمد. والمتابعة المنتظمة هي جوهر الخطة: مقارنة صور الرنين على الزمن، تحاليل السلامة الدورية، ومراجعة الأهداف الوظيفية مع المريض وأسرته. أمّا التوقّف عن التدخين وضبط الوزن والنشاط البدني المنتظم فهي عناصر مساندة مثبتة الفائدة على القدرة الوظيفية والصحة العامة، ولا تُغني عن الخطة الدوائية ولا تُستبدل بها.

## خطوات الإجراء

### 1. تثبيت التشخيص واستبعاد ما يشبهه

يجمع طبيب الأعصاب تاريخ الأعراض وتواريخها، ويجري فحصاً عصبياً مفصّلاً، ويطلب رنيناً مغناطيسياً للدماغ والحبل الشوكي مع صبغة، وتحاليل دم لاستبعاد نقص الفيتامينات وأمراض المناعة والعدوى، وقد يحتاج بزلاً قطنياً أو فحص الجهود المستحرَقة البصرية عند الحاجة.

### 2. تحديد النمط ومستوى النشاط

يُصنَّف المرض إلى انتكاسي هاجع أو تقدّمي، ويُقاس عبء البؤر على الرنين ومدى تأثّر الوظائف. هذا التصنيف هو ما يحدّد شدّة الخطة العلاجية وسرعتها، ويُوثَّق خطّ أساس يُقاس عليه التقدّم في المراجعات القادمة.

## Procedure steps

### 1. Confirming the diagnosis and excluding mimics

The neurologist takes a detailed symptom history with dates, performs a full neurological examination, and orders MRI of the brain and spinal cord with contrast plus blood tests to exclude vitamin deficiency, autoimmune disease and infection. A lumbar puncture or visual evoked potentials are added when needed.

### 2. Defining the disease pattern and activity

The disease is classified as relapsing-remitting or progressive, and the lesion burden on MRI and the degree of functional impairment are quantified. This classification determines how intensive and how urgent the treatment plan should be, and it sets a baseline against which future reviews are measured.

### 3. Choosing disease-modifying therapy together

The neurologist discusses the available long-term options with the patient: route of administration, required monitoring and possible side effects, taking into account other illnesses and plans for pregnancy, breastfeeding and work. A monitoring schedule is agreed before starting; specific drug names are not listed here because the choice is strictly individual.

### 4. Treating acute relapses and managing symptoms

For a confirmed relapse, a short course of treatment is given to speed recovery after infection and fever have been excluded as a temporary cause of symptoms. In parallel, chronic symptoms are managed — spasticity, neuropathic pain, fatigue and bladder problems — combining medication, rehabilitation and adjustments to daily routine.

## About this service

Multiple sclerosis treatment is a long-term medical plan for an immune-mediated disease in which the immune system attacks the myelin sheath around nerves in the brain, spinal cord and optic nerve. Nerve signalling is disrupted, producing symptoms such as weakness or numbness of a limb, imbalance, double vision or painful loss of vision in one eye, profound fatigue, muscle spasticity and bladder problems. \*\*This is a disease that is managed, not cured.\*\* The goals are to reduce the number and severity of relapses, slow the accumulation of disability, control day-to-day symptoms and preserve function — not to promise recovery.

The plan rests on four pillars that work together. The first is disease-modifying therapy, given continuously to reduce the immune activity directed against nerve tissue; the choice is made by the neurologist according to disease activity, other medical conditions and pregnancy plans. The second is treatment of an acute relapse when one occurs. The third is symptom management: spasticity, fatigue, neuropathic pain, bladder and bowel problems, walking difficulty, and changes in concentration and memory. The fourth is neurological rehabilitation — physiotherapy, occupational therapy and speech therapy when needed — alongside psychological and vocational support, because this disease affects people in their working and family years.

The limits must be stated plainly. No treatment today ends the disease, and disease-modifying therapy works on the future rather than on damage already done; function that has been permanently lost is addressed by rehabilitation through adaptation and strengthening what remains, not by restoring it as it was. An MRI with no new lesions does not mean the disease has stopped for good, and the reverse also happens: a new lesion may appear without symptoms. One point is widely misunderstood — a rise in body temperature from fever, a hot bath or hot weather can temporarily bring back old symptoms; this is not a new relapse and it settles as the temperature returns to normal.

Diagnosis and treatment eligibility are established by neurological examination, imaging and laboratory tests, not by matching symptoms found online, because many other conditions mimic multiple sclerosis at first presentation and must be excluded before starting long-term therapy. Regular follow-up is the core of the plan: comparing MRI studies over time, periodic safety blood tests, and reviewing functional goals with the patient and family. Stopping smoking, weight control and regular physical activity are supportive measures with proven value for function and general health, but they do not replace the medical plan and are not an alternative to it.

## Preparation

Bring every previous report and any discs or links for older MRI studies: comparison over time tells the neurologist far more than a single recent scan. Before the visit, write a symptom log — what the symptoms were, when they started, how long they lasted, whether they recurred and what makes them worse — because this timeline is central to both diagnosis and follow-up. Tell the doctor about all medications and every dietary supplement or herbal mixture, since some interact with immune therapies or affect the liver and blood tests. Report any current infection, fever or urinary tract infection, because treatment may be postponed until it is treated. Bring your vaccination record, and mention early if you are planning pregnancy or breastfeeding, as timing and treatment choice change completely. Be candid about smoking and how much, and about your activity level, occupation and functional needs. Bring a companion if prolonged sitting tires you, and ask which periodic blood tests are required and what the plan is if you suspect a relapse.

## Aftercare

Adherence is what changes the long-term picture: never stop your treatment on your own because you feel better or stable, and discuss any side effect with your neurologist rather than stopping abruptly. Do not miss periodic safety blood tests or comparative MRI appointments. Regular physical activity is safe and beneficial and does not worsen the disease; start gradually and ask a physiotherapist for a programme matched to your balance and strength. Protect your sleep, spread effort across the day to limit fatigue, and avoid excessive heat and very hot baths, which can bring on temporary symptoms. Keep vaccinations up to date as your doctor advises, particularly on immune-suppressing therapy.
