## Procedure steps

### 1. Confirming the diagnosis with tests

Urine protein is quantified alongside albumin, kidney function, electrolytes, blood count and lipids, with an examination of how the swelling is distributed plus blood pressure and weight. Other causes of oedema such as heart or liver disease and malnutrition are excluded.

### 2. Deciding whether a kidney biopsy is needed

A biopsy is not done routinely. The paediatric nephrologist reserves it for defined situations such as an atypical age at onset, no response to first-line treatment, or blood in the urine with hypertension. When indicated it is the basis of the treatment decision, since it identifies the tissue pattern.

### 3. Starting drug treatment under specialist care

Treatment follows a staged schedule set by the paediatric nephrologist and is continued even after the swelling clears. Never stop the medicine or reduce it yourself: early cessation is a leading cause of relapse and abrupt withdrawal can be dangerous.

### 4. Managing oedema and preventing complications

Salt is reduced and weight tracked daily, with a diuretic used only on medical instruction. Hand hygiene is emphasised and vaccinations reviewed with the doctor because protein loss weakens immunity, and gentle mobility is encouraged during an episode since clotting risk rises.

### 5. Follow-up and monitoring for relapse at home

The family learns to use a urine protein dipstick and log the result with the child's weight, so relapse is caught before swelling returns. Height, growth, blood pressure and bone health are monitored during long treatment, and the child is supported at school and emotionally.

## About this service

Nephrotic syndrome in children is a condition in which the kidney filters lose their ability to hold protein back, so large amounts of protein leak into the urine, blood albumin falls and swelling appears. What families usually notice first is puffiness of the eyelids in the morning, often mistaken for allergy, followed by swelling of the feet and abdomen and a rapid weight gain that comes from fluid held in the tissues rather than from food. The urine may look frothy.

Diagnosis rests on measuring urine protein along with albumin, kidney function, blood count and lipids, combined with a careful examination and exclusion of other causes of swelling. In most pre-school children the underlying pattern is the type that responds to medical treatment, which is why \*\*a kidney biopsy is not performed on every child\*\*. Biopsy is reserved for defined situations: an unusual age at onset, failure to respond to first-line treatment, blood in the urine with raised blood pressure and impaired kidney function, or suspicion of a systemic disease. When a biopsy is decided upon, it becomes the basis of the next treatment decision because it identifies the tissue pattern.

One widespread misunderstanding needs correcting. Childhood nephrotic syndrome \*\*is not kidney failure\*\* and does not mean dialysis, and the swelling is not a toxin to be flushed out with herbal diuretics. Equally important, \*\*relapse is expected\*\*: many children go through repeated episodes, each managed in turn, and a relapse does not mean the treatment failed or that the family did something wrong. The long-term aim is to control episodes, limit the side effects of treatment and protect the child's growth, not to promise a cure by a fixed date.

Drug treatment is started, adjusted and stopped only under a paediatric nephrologist. It follows a staged schedule that must not be shortened or halted abruptly once the swelling disappears; stopping early is among the commonest triggers of relapse. Oedema is managed by reducing salt and tracking daily weight, and some children need a diuretic strictly on medical instruction. Because protein loss weakens immunity and makes the blood more likely to clot, infection prevention is emphasised, the vaccination schedule is reviewed with the treating doctor, and prolonged immobility during an episode is discouraged.

Follow-up includes teaching the family to use a urine protein dipstick at home and to record the result with the child's weight, monitoring height, growth, blood pressure and bone health during long treatment, and supporting the child at school and emotionally. Suitability for any step, from biopsy to a change of medicine, is decided by clinical assessment and laboratory results, not by reading a page or by another child's experience.

## Preparation

Bring one record to every visit containing the child's daily weight, the urine protein dipstick result and the date, because this is the single most useful piece of information the doctor works from. Also bring previous laboratory reports, the names, doses and duration of every medicine given, the vaccination booklet, and the growth chart. Tell the doctor if the child has had fever, diarrhoea, a sore throat or chickenpox, or contact with an infectious illness recently, and whether abdominal pain or breathlessness has appeared. Mention any herbal product, diuretic or homemade mixture given to the child, as some harm the kidneys. If the appointment is for fasting blood tests or a planned biopsy, follow the fasting and medication instructions exactly as written, and tell the team about any blood thinner or painkiller the child takes. Do not adjust diet or medication before the visit in order to produce a better looking result.

## Aftercare

Continue the medicine at the written dose and for the full written duration even if the swelling has completely gone, and never stop or reduce it except on the paediatric nephrologist's decision. Test the urine with a dipstick at home as often as you were asked and log the result with the daily weight in the same notebook; rising protein with rapid weight gain is the earliest sign of relapse. Reduce salt and processed food without starving the child or restricting protein, and encourage activity and school attendance as far as the condition allows. Keep up hand hygiene, tell any treating doctor that the child is on treatment that lowers immunity, and review vaccination timing with the specialist rather than with a health centre alone. \*\*Seek urgent care immediately for\*\* fever with \*\*abdominal pain or a rigid abdomen\*\*, which may mean infection of the abdominal lining, \*\*breathlessness or chest pain\*\*, swelling or pain in one leg, \*\*a clear fall in urine output or tea coloured urine\*\*, vomiting that prevents drinking or taking medication, severe headache or confusion, or a rash with fever during treatment.
