التشخيص ليس دائماً بديهياً، فحرقة التبوّل قد تنجم عن أسباب غير معدية مثل التهاب مهبلي أو تهيّج أو حصوة أو حالة جلدية. ولهذا يُطلب تحليل البول وزراعته قبل بدء العلاج في كثير من الحالات، وخاصة عند الرجال والحوامل والأطفال ومن تكرّرت عندهم العدوى أو فشل علاج سابق. \*\*ووجود جراثيم في البول بلا أعراض لا يُعالج عادة\*\* إلا في الحمل أو قبل بعض الإجراءات، لأن علاجه بلا داعٍ يزيد المقاومة بلا فائدة.

هناك سياقات تُغيّر القرار كلياً: العدوى عند الرجل ليست شائعة كما عند المرأة ولا تُعتبر أمراً عابراً، بل تستدعي البحث عن سبب مثل انسداد أو حصوة أو تضخّم بروستاتا أو تصريف ناقص. والعدوى في الحمل تُعالج دائماً لأنها قد تؤثّر على الحمل. وعند الطفل الصغير قد تكون الحمّى بلا سبب واضح هي العَرَض الوحيد، وتكرار العدوى عند الأطفال يستدعي تقييماً لاحتمال ارتجاع بولي حالبي أو مشكلة تشريحية.

حدود العلاج يجب أن تُقال: \*\*العلاج يقضي على العدوى الحالية ولا يمنع تكرارها\*\*، ولا يصحّح حصوة أو انسداداً أو خللاً تشريحياً. وشرب الماء مفيد ومساعد لكنه \*\*لا يعالج عدوى مستقرّة\*\*، والكرانبيري والأعشاب ليست بديلاً عن العلاج عند تأكّد العدوى. ومن أخطر العادات المنتشرة أن يصرف المريض مضاداً حيوياً بمبادرته أو من بقايا علبة سابقة أو بوصفة قريب: هذا يطمس التشخيص، ويُفسد نتيجة الزراعة، ويصنع جراثيم مقاومة يصعب علاجها لاحقاً. والترشيح لأي دواء ومدّته يُحدَّدان بالفحص السريري وبنتيجة الزراعة لا بالقراءة.

## خطوات الإجراء

### 1. التقييم وتحديد مستوى العدوى

يسأل الطبيب عن نمط الأعراض ومدّتها وعن حمّى وألم خصر وتقيّؤ، ويفحص البطن والخصر، ويقيس الحرارة والضغط والنبض، ليحدّد إن كانت العدوى سفلية بسيطة أو التهاب كلية يحتاج تعاملاً أسرع.

### 2. تحليل البول وزراعته

تُؤخذ عيّنة من منتصف التبوّل بعد تنظيف المنطقة، ويُجرى تحليل سريع ثم تُرسل للزراعة وتحديد حساسية الجراثيم. الزراعة هي التي تضبط العلاج، ولهذا تُؤخذ قبل بدء الدواء كلما أمكن.

### 3. بدء العلاج المناسب

يبدأ الطبيب علاجاً مبدئياً بحسب صورة الحالة وعمرك وحملك وأمراضك المزمنة وتاريخ العدوى السابقة وأنماط المقاومة، مع مسكّن وسوائل. ويُعدَّل العلاج لاحقاً بحسب نتيجة الزراعة.

### 4. البحث عن سبب مهيّئ

عند الرجال، أو تكرار العدوى، أو فشل العلاج، أو وجود دم في البول، يُقيَّم الجهاز البولي بتصوير مناسب للبحث عن حصوة أو انسداد أو تصريف ناقص أو تضخّم بروستاتا أو ارتجاع عند الأطفال.

### 5. المتابعة والوقاية

يُتابع التحسّن خلال يومين إلى ثلاثة، وتُكمل مدة العلاج كما وُصفت حتى لو زالت الأعراض. وتُشرح تدابير الوقاية العملية: سوائل كافية، عدم حصر البول، نظافة صحيحة، والتبوّل بعد الجماع.

## Procedure steps

### 1. Assessment and localising the infection

The doctor asks about the pattern and duration of symptoms and about fever, flank pain and vomiting, examines the abdomen and flanks, and measures temperature, blood pressure and pulse to decide whether this is a simple lower infection or a kidney infection needing faster action.

### 2. Urine analysis and culture

A midstream sample is collected after cleaning the area, a rapid analysis is performed and the sample is sent for culture and sensitivity testing. The culture is what guides treatment, which is why it is taken before starting medication whenever possible.

### 3. Starting appropriate treatment

The doctor starts initial treatment based on the clinical picture, your age, pregnancy status, chronic illnesses, previous infection history and local resistance patterns, together with pain relief and fluids. Treatment is then adjusted according to the culture result.

### 4. Looking for a predisposing cause

In men, in recurrent infection, after treatment failure, or when blood is present in the urine, the urinary tract is assessed with appropriate imaging to look for a stone, obstruction, incomplete emptying, prostatic enlargement, or reflux in children.

### 5. Follow up and prevention

Improvement is reviewed within two to three days, and the prescribed course is completed even if symptoms disappear. Practical prevention is explained: adequate fluids, not holding urine, correct hygiene, and passing urine after intercourse.

## التحضير قبل الإجراء

اذهب إلى العيادة ومثانتك ليست فارغة تماماً إن أمكن، لأن العيّنة هي أهمّ خطوة تشخيصية. ونظّف المنطقة بالماء قبل جمع العيّنة، واجمع من منتصف التبوّل في وعاء معقّم تعطيه العيادة، ولا تلمس داخله. أخبر الطبيب بكل مضاد حيوي تناولته في الأسابيع الأخيرة ولو جرعة واحدة، لأنه يؤثّر على نتيجة الزراعة. اذكر إن كنت حاملاً أو تحاولين الحمل أو مرضعة، وإن كان لديك سكري أو حصوات أو تضخّم بروستاتا أو قسطرة أو عملية سابقة على المسالك أو كلية واحدة أو زرع كلية، وأي حساسية دوائية معروفة. أحضر تقارير التحاليل والزراعات السابقة إن توفّرت. ولا تبدأ علاجاً بنفسك قبل الفحص، ولا تشرب كميات ضخمة من الماء قبل العيّنة لأن تخفيفها الشديد قد يُضعف دلالتها.

## العناية بعد الإجراء

أكمل مدة العلاج كاملة ولو تحسّنت في يومين، ولا توفّر جرعات لاستخدام لاحق. اشرب سوائل بانتظام، ولا تحصر البول، وراقب أعراضك يومياً؛ فالتحسّن المتوقّع يبدأ عادة خلال 48 إلى 72 ساعة. أجرِ زراعة المتابعة إذا طلبها الطبيب، وخاصة في الحمل. راجع فوراً عند \*\*حرارة ورجفة مع ألم في الخصر — احتمال التهاب كلية\*\*، أو \*\*تقيّؤ يمنعك من تحمّل السوائل أو الدواء\*\*، أو \*\*دم في البول ولو مرة واحدة وبلا ألم، فهذا يستوجب تقييماً دائماً ولو زالت الأعراض\*\*، أو \*\*احتباس بولي حاد وعجز عن التبوّل مع انتفاخ أسفل البطن\*\*، أو \*\*ألم أو تورّم في الخصية\*\*، أو \*\*تلخبط أو خمول عند المسنّ\*\*، أو \*\*عدم أي تحسّن بعد ثلاثة أيام من بدء العلاج أو عودة الأعراض بعد انتهائه\*\*. وراجع فوراً في الحمل، أو مع السكري أو نقص المناعة، أو عند \*\*حمّى لدى طفل أقلّ من ثلاثة أشهر\*\*.

## About this service

A urinary tract infection is an infection of one part of the urinary system. In practice it is divided into a lower infection involving the bladder and urethra, which causes burning on passing urine, frequency, urgency, lower abdominal pain and cloudy or strong smelling urine, and an upper infection involving the kidney, which adds fever, chills, flank pain and vomiting. Treatment rests on confirming that infection is genuinely present, then an antibacterial medicine chosen by the doctor and adjusted according to the urine culture result, together with fluids, pain relief and treatment of any predisposing cause.

The diagnosis is not always straightforward, since burning on urination can arise from non infectious causes such as vaginal inflammation, irritation, a stone or a skin condition. Urine analysis and culture are therefore requested before starting treatment in many cases, particularly in men, pregnant women, children, and anyone with recurrent infection or a previous treatment failure. \*\*Bacteria in the urine without symptoms are usually not treated\*\*, except in pregnancy or before certain procedures, because treating them needlessly increases resistance with no benefit.

Some contexts change the decision entirely. Infection in a man is not as common as in a woman and is not regarded as a passing event; it prompts a search for a cause such as obstruction, a stone, prostatic enlargement or incomplete drainage. Infection in pregnancy is always treated because it can affect the pregnancy. In a young child, fever without an obvious source may be the only sign, and recurrent infection in children warrants assessment for vesicoureteral reflux or an anatomical problem.

The limits should be stated: \*\*treatment clears the current infection, it does not prevent recurrence\*\*, and it does not correct a stone, an obstruction or an anatomical abnormality. Drinking water helps and is supportive but \*\*it does not cure an established infection\*\*, and cranberry products or herbal remedies are not a substitute for treatment once infection is confirmed. One of the most damaging habits is obtaining an antibiotic on personal initiative, from the remains of a previous box, or on a relative prescription: this blurs the diagnosis, invalidates the culture result, and breeds resistant organisms that are harder to treat later. Which medicine suits you and for how long is decided by clinical assessment and by the culture result, not by reading.

## Preparation

Come to the clinic without having completely emptied your bladder if possible, because the sample is the single most important diagnostic step. Clean the area with water before collecting, take a midstream sample in the sterile container the clinic provides, and do not touch the inside of it. Tell the doctor about every antibiotic you have taken in recent weeks, even a single dose, because it affects the culture result. Mention if you are pregnant, trying to conceive or breastfeeding, and whether you have diabetes, stones, prostatic enlargement, a catheter, previous urinary surgery, a single kidney or a kidney transplant, along with any known drug allergy. Bring previous test and culture reports if you have them. Do not start treatment yourself before assessment, and do not drink very large volumes of water before the sample, since heavy dilution can weaken its value.

## Aftercare

Complete the full course even if you feel better within two days, and do not save doses for later use. Drink fluids regularly, do not hold urine, and track your symptoms daily, since expected improvement usually begins within 48 to 72 hours. Have a follow up culture if the doctor requests one, particularly in pregnancy. Seek care immediately for \*\*fever and chills with flank pain, which may indicate a kidney infection\*\*, \*\*vomiting that prevents you keeping fluids or medication down\*\*, \*\*blood in the urine even once and even if painless, which always requires assessment regardless of symptoms settling\*\*, \*\*acute retention with inability to pass urine and lower abdominal distension\*\*, \*\*pain or swelling in a testicle\*\*, \*\*confusion or lethargy in an older person\*\*, or \*\*no improvement at all after three days of treatment or a return of symptoms once it finishes\*\*. Seek care promptly in pregnancy, with diabetes or reduced immunity, and for \*\*any fever in a child under three months\*\*.
