في التهاب الأذن الخارجية يكون الأساس تنظيف القناة تحت الرؤية وقطرات موضعية مناسبة، وقد يُوضَع فتيل ماصّ إن كانت القناة متورّمة لتصل القطرة إلى العمق، مع إبقاء الأذن جافّة تماماً. أمّا في التهاب الأذن الوسطى الحادّ فأولوية العلاج هي ضبط الألم والحرارة، ثم قرار المضاد الحيوي الذي يتّخذه الطبيب بحسب العمر وشدّة الحالة ومدّتها وإصابة أذن واحدة أو الأذنين، إذ إن جزءاً من الحالات فيروسي ويتحسّن دون مضاد حيوي تحت مراقبة قريبة.

أخطاء شائعة يجب تجنّبها: تقطير زيت أو ثوم أو أعشاب في الأذن، فهذا غير مثبت وخطير إن كانت الطبلة مثقوبة؛ واستخدام بقايا مضاد حيوي من مرّة سابقة أو من فرد آخر؛ ومحاولة تنظيف القناة المتورّمة بعود قطن. أمّا خروج إفراز من الأذن أثناء الالتهاب فقد يعني انفتاح ثقب في الطبلة، وهو يستوجب فحصاً لا تجاهلاً، ويمنع أي غسل مائي.

حدود العلاج: المضاد الحيوي لا يُفيد الالتهاب الفيروسي، ولا يُذيب السائل الذي يبقى خلف الطبلة بعد انطفاء الالتهاب، وهذا السائل قد يستمرّ أسابيع مع نقص سمع خفيف ويُتابَع بقياس سمع لا بكورسات متكرّرة. كذلك لا يمنع أي علاج تكرار الالتهاب عند من لديه استعداد، ولا يعني اختفاء الألم بعد يومين أن الشفاء تمّ؛ فالمتابعة هي ما يؤكّد عودة الطبلة إلى طبيعتها.

الوقاية عملية أكثر من الدواء: علاج انسداد الأنف والحساسية، الابتعاد عن دخان التبغ، عدم إرضاع الطفل مستلقياً على ظهره، وتجفيف الأذن جيداً بعد السباحة. والترشيح لأي خطوة، بما فيها أنبوب التهوية عند تكرار الالتهاب، يُحدَّد بالفحص السريري.

## خطوات الإجراء

### 1. التاريخ المرضي وتنظير الأذن

يسأل الطبيب عن مدّة الألم والحرارة والإفراز ونقص السمع والنزلات السابقة، ثم يفحص الأذنين بالمنظار ليحدّد نوع الالتهاب وحالة الطبلة، ويفحص الأنف والبلعوم لأنهما مصدر المشكلة غالباً.

### 2. تنظيف القناة عند اللزوم

في الالتهاب الخارجي يُنظَّف الحَرَش والإفرازات بالشفط تحت الرؤية لأن القطرة لا تعمل فوق طبقة إفرازات. وإن كانت القناة شديدة التورّم يُوضع فتيل يحمل الدواء إلى الداخل ويُستبدل بعد أيام.

### 3. ضبط الألم والحرارة

تسكين الألم أولوية مستقلّة عن المضاد الحيوي، ويُوصف بما يناسب العمر والحالة الصحية بقرار الطبيب. الكمّادات الدافئة قد تريح، ولا يُقطَّر أي شيء في الأذن قبل معرفة حالة الطبلة.

### 4. قرار المضاد الحيوي أو المراقبة

يوازن الطبيب بين البدء بمضاد حيوي والمراقبة القريبة مع مراجعة محدّدة، بحسب العمر والشدّة والمدّة وحالة الطبلة ووجود إفراز. وإن بُدئ العلاج فيُكمَل كما وُصف ولا يُوقَف عند اختفاء الألم.

### 5. المتابعة وتقييم السمع

تُعاد الفحص بعد انطفاء الالتهاب للتأكّد من عودة الطبلة وعدم بقاء سائل. إن بقي السائل مع نقص سمع يؤثّر على الكلام أو الدراسة، يُقيَّم السمع وتُدرَس خطوات أخرى مثل أنبوب التهوية.

## Procedure steps

### 1. History and otoscopy

The clinician asks about the duration of pain, fever, discharge and hearing loss and about recent colds, then examines both ears with an otoscope to establish the type of infection and the state of the eardrum, and checks the nose and throat, which are usually the source.

### 2. Clearing the canal when needed

In otitis externa debris and discharge are cleared by suction under vision, since drops cannot work through a layer of secretions. If the canal is very swollen, a medicated wick is inserted to carry treatment inwards and is changed after a few days.

### 3. Pain and fever control

Pain relief is a priority in its own right, independent of antibiotics, and is prescribed to suit the patient's age and health by the treating clinician. Warm compresses may soothe, and nothing should be instilled into the ear before the eardrum has been assessed.

### 4. Antibiotic or watchful waiting

The clinician weighs starting an antibiotic against close observation with a defined review, based on age, severity, duration, the appearance of the eardrum and the presence of discharge. If treatment is started it is completed as prescribed and not stopped as soon as pain settles.

### 5. Follow-up and hearing assessment

The ear is re-examined after the acute episode to confirm the eardrum has recovered and no fluid remains. If fluid persists with hearing loss that affects speech or schooling, hearing is formally tested and further steps such as a ventilation tube are considered.

قبل الموعد سجّل متى بدأ الألم، وهل هناك حرارة أو إفراز أو نقص سمع أو دوار، وهل الالتهاب في أذن واحدة أم الأذنين، وكم مرّة تكرّر خلال السنة. أخبر الطبيب بأي حساسية دوائية، وبكل ما استخدمته فعلاً من مسكّنات أو قطرات أو مضادات حيوية بما فيها بقايا كورسات سابقة، وبحالات مثل السكري أو نقص المناعة أو جراحة أذن سابقة أو أنبوب تهوية أو ثقب في الطبلة. لا تقطّر زيتاً أو ثوماً أو أي وصفة منزلية في الأذن، ولا تحاول تنظيف الأذن بعود قطن قبل الفحص لأن ذلك يزيد التورّم ويُصعّب الفحص. أبقِ الأذن جافّة وامتنع عن السباحة. إن كان المريض طفلاً فاذكر تاريخ نزلاته وتأخّر الكلام إن وُجد وتعرّضه لدخان التبغ.

## العناية بعد الإجراء

أكمل العلاج بالمدّة التي حُدّدت لك ولا تتوقّف عند تحسّن الألم، واستخدم القطرات بالطريقة التي شُرحت لك مع تدفئة العلبة بيدك قليلاً قبل التقطير لتقليل الدوار. حافظ على جفاف الأذن حتى يسمح الطبيب بغير ذلك: لا سباحة ولا غوص، وحماية الأذن أثناء الاستحمام. لا تُدخل أعواد القطن. توقّع تحسّناً تدريجياً في اليومين إلى الثلاثة الأولى، وامتلاءً أو نقص سمع خفيفاً قد يستمرّ أسبوعين أو أكثر بعد زوال الألم. راجع فوراً عند \*\*حرارة عالية مع نعاس شديد أو تصلّب في الرقبة\*\*، أو \*\*تورّم واحمرار وألم خلف الأذن أو بروز الأذن للأمام\*\*، أو \*\*ضعف في عضلات الوجه\*\*، أو \*\*دوار شديد أو تقيّؤ أو عدم توازن\*\*، أو \*\*صداع شديد\*\*، أو \*\*إفراز دموي\*\*، أو \*\*عدم تحسّن بعد ثلاثة أيام من العلاج\*\*، أو \*\*نقص سمع مستمرّ بعد شهر من الشفاء\*\*.

## About this service

Ear infection treatment covers two conditions that are frequently confused. Otitis externa affects the skin of the ear canal and typically hurts when the ear is touched or pulled. Acute otitis media affects the space behind the eardrum, usually follows a respiratory infection, and presents with deep pain, fever and reduced hearing. Distinguishing the two requires otoscopy, and the entire plan follows from that distinction.

In otitis externa the mainstays are clearing the canal under vision and using appropriate topical drops, with an absorbent wick placed if the canal is too swollen for drops to reach the depth, while keeping the ear completely dry. In acute otitis media the first priority is controlling pain and fever; the decision about antibiotics is made by the clinician according to age, severity, duration and whether one or both ears are involved, since a proportion of cases are viral and settle without antibiotics under close review.

Common mistakes to avoid: instilling oil, garlic or herbal preparations into the ear, which is unproven and hazardous if the eardrum is perforated; using leftover antibiotics from a previous episode or from someone else; and trying to clean a swollen canal with a cotton bud. Discharge appearing during an infection may mean the eardrum has perforated; this needs examination rather than being ignored, and it rules out any irrigation.

What treatment does not do: antibiotics do not help viral infection, and they do not clear the fluid that often remains behind the eardrum after the acute episode settles. That fluid can persist for weeks with mild hearing loss and is followed with hearing testing rather than repeated antibiotic courses. No treatment prevents recurrence in a predisposed child, and pain settling after two days does not confirm resolution; follow-up examination is what confirms the eardrum has returned to normal.

Prevention is more practical than medication: treating nasal obstruction and allergy, avoiding tobacco smoke, not feeding an infant lying flat on the back, and drying the ears thoroughly after swimming. Suitability for any further step, including a ventilation tube for recurrent infection, is determined by clinical examination.

## Preparation

Before the appointment note when the pain began, whether there is fever, discharge, hearing loss or dizziness, whether one or both ears are affected, and how many episodes have occurred in the past year. Tell the doctor about drug allergies and about everything already used, including painkillers, drops and any leftover antibiotics, as well as conditions such as diabetes, reduced immunity, previous ear surgery, a ventilation tube or a known perforation. Do not instil oil, garlic or any home remedy into the ear, and do not clean the ear with a cotton bud before examination, as this worsens swelling and obscures the view. Keep the ear dry and avoid swimming. For a child, mention the pattern of recent colds, any speech delay and exposure to tobacco smoke.

## Aftercare

Complete the prescribed course for its full duration rather than stopping when the pain eases, and use drops exactly as demonstrated, warming the bottle briefly in your hand to reduce dizziness. Keep the ear dry until your doctor says otherwise: no swimming or diving, and protect the ear while showering. Do not use cotton buds. Expect gradual improvement over the first two to three days, and be aware that mild fullness or muffled hearing can persist for two weeks or more after the pain has gone. Seek care immediately for high fever with marked drowsiness or neck stiffness, swelling, redness and tenderness behind the ear or an ear that protrudes forwards, facial muscle weakness, severe dizziness, vomiting or unsteadiness, severe headache, bloody discharge, no improvement after three days of treatment, or hearing loss still present a month after recovery.
