الالتهاب المزمن قصّة أخرى: هو غالباً جزء من طيف الانسداد الرئوي المزمن ويحتاج تأكيداً بفحص وظائف الرئة لا بالتخمين. وهنا الحقيقة التي لا تُجامل: الإقلاع عن التدخين هو التدخّل الوحيد الذي يبطّئ تدهور وظيفة الرئة فعلاً. كل ما عداه من بخّاخات وتأهيل رئوي وتطعيمات وأكسجين يخفّف الأعراض ويقلّل النكسات ويحسّن جودة الحياة، لكنه لا يوقف انحدار الوظيفة. من استمرّ في التدخين يستمرّ تدهوره مهما تعدّدت الأدوية.

وتصحيحاً لخرافة تؤذي المرضى كثيراً: البخّاخ لا يسبّب إدماناً ولا يفقد أثره بالاستخدام. الحاجة المتكرّرة إليه دليل على مرض غير مضبوط يستوجب مراجعة الخطة، لا دليل تعوّد، وتأخير استخدامه خوفاً من ذلك يزيد الخطر. كما أن الأدوية المثبّطة للكحة ليست علاجاً للسعال المنتج، فهي تحبس البلغم داخل الشعب بدل إخراجه.

وأخيراً، السعال ليس دائماً التهاب قصبات. سعال يتجاوز أسبوعين مع حرارة وتعرّق ليلي ونزول وزن يستوجب فحص البلغم وصورة الصدر لاستبعاد السلّ الذي ما زال موجوداً في الأردن، وعلاجه طويل بالأشهر ويجب إكماله كاملاً حتى بعد اختفاء الأعراض.

## خطوات الإجراء

### 1. تمييز الحادّ من المزمن

يسأل الطبيب عن مدّة السعال وتكراره في السنوات السابقة وعن البلغم والأزيز وتاريخ التدخين والتعرّض المهني للغبار والأبخرة. هذا التمييز يحدّد المسار كلّه: علاج داعم قصير في الحادّ، أم تقييم كامل لمرض مزمن يحتاج فحص وظائف رئة وخطة طويلة.

### 2. الفحص واستبعاد الأخطر

يُقاس الأكسجين والنبض ومعدل التنفس ويُسمع الصدر، وتُطلب صورة صدر عند الشكّ بالتهاب رئوي أو عند طول المدّة أو نزول الوزن أو وجود دم في البلغم. عند الاشتباه بالسلّ يُطلب فحص بلغم، وعند الشكّ بمرض مزمن يُطلب فحص وظائف الرئة.

### 3. العلاج الداعم وضبط الأعراض

في الالتهاب الحادّ يُعتمَد على السوائل والراحة وخفض الحرارة وترطيب الجوّ وتجنّب الدخان، مع موسّع للشعب عند الأزيز بقرار الطبيب. لا تُستخدم مثبّطات الكحة مع سعال منتج، والمضاد الحيوي يُوصف بقرار طبي لا بطلب المريض ولا لأن لون البلغم تغيّر وحده.

### 4. خطة المرض المزمن

إن ثبت مرض مزمن تُبنى خطة تبدأ بالإقلاع عن التدخين بوصفه التدخّل الوحيد الذي يبطّئ تدهور الوظيفة، مع بخّاخات موصوفة وتصحيح لطريقة استخدامها، وتأهيل رئوي وتمارين تنفّس، وتطعيمات، وخطة مكتوبة للتصرّف عند النكسة، ومتابعة دورية بفحص وظائف الرئة.

### 5. المتابعة وعلاج المحرّضات

يُتابع السعال المتبقّي وتُراجع أسباب تُطيله: تنقيط أنفي خلفي وحساسية، حمض المعدة، ربو غير مشخّص، تعرّض للدخان أو غبار العمل. إن استمرّ السعال بعد ثمانية أسابيع فهو سعال مزمن يحتاج تقييماً أوسع لا تكرار المضادات الحيوية.

## Procedure steps

### 1. Distinguishing acute from chronic

The physician asks how long the cough has lasted, how often it has recurred in previous years, and about sputum, wheeze, smoking history, and occupational exposure to dust and fumes. This distinction sets the whole path: brief supportive care for the acute form, or a full assessment for chronic disease requiring lung function testing and a long-term plan.

### 2. Examination and excluding the dangerous

Oxygen saturation, pulse, and respiratory rate are recorded and the chest examined, with a chest X-ray requested when pneumonia is suspected or when the cough is prolonged, weight is falling, or blood appears in sputum. Sputum testing is ordered when tuberculosis is a possibility, and lung function testing when chronic disease is suspected.

### 3. Supportive care and symptom control

In acute bronchitis, management rests on fluids, rest, fever control, humidified air, and avoiding smoke, with a bronchodilator when wheeze is present and the doctor agrees. Cough suppressants are avoided with a productive cough, and antibiotics are prescribed on medical judgement, not on patient request and not merely because sputum colour changed.

### 4. The chronic disease plan

If chronic disease is confirmed, the plan starts with smoking cessation as the only intervention that slows loss of lung function, alongside prescribed inhalers with technique correction, pulmonary rehabilitation and breathing exercises, vaccinations, a written action plan for flare-ups, and periodic follow-up with lung function testing.

### 5. Follow-up and treating triggers

Any residual cough is followed up and the factors that prolong it are reviewed: postnasal drip and allergy, acid reflux, undiagnosed asthma, and exposure to smoke or workplace dust. A cough persisting beyond eight weeks is a chronic cough needing broader assessment rather than repeated courses of antibiotics.

قبل الزيارة سجّل مدّة السعال بالأيام أو الأسابيع، وشكل البلغم ولونه، وهل ظهر فيه دم، ومتى يزيد السعال: ليلاً أم بعد المجهود أم مع الغبار أم بعد الأكل. اكتب عدد نوبات السعال الطويل التي مررت بها في السنوات الماضية وعدد مرات استخدام المضاد الحيوي، وأحضر علبة كل بخّاخ تستخدمه لعرض طريقة استخدامك أمام الطبيب لأن الخطأ في التقنية سبب شائع لفشل العلاج. أخبره بتاريخ التدخين بالسنوات وبالأرجيلة والتدخين الإلكتروني، وبطبيعة عملك وأي تعرّض للغبار أو الأبخرة أو الدهانات، وبأمراضك المزمنة وأدويتك وأي حساسية دوائية. أخبره أيضاً بأي مخالطة لمريض سعال طويل أو مريض سلّ.

## العناية بعد الإجراء

اشرب سوائل كافية، وارتح، وابتعد تماماً عن الدخان والأرجيلة والتدخين الإلكتروني، فالتعرّض للدخان يطيل السعال أسابيع. استخدم بخّاخك بالطريقة التي دُرّبت عليها ومع المِبعِدة إن وُصفت لك، ولا توقف العلاج الوقائي لأنك تحسّنت. توقّع أن يستمرّ السعال أسبوعين إلى ثلاثة بعد الالتهاب الحادّ، وسجّل عدد المرات التي احتجت فيها للبخّاخ المسعف وأخبر بها طبيبك. \*\*راجع الطوارئ فوراً عند ضيق نفس يمنعك من إكمال جملة كاملة، أو زُرقة الشفاه، أو تشوّش الوعي.\*\* \*\*وراجع فوراً عند ألم صدر حاد مع ضيق نفس مفاجئ، فقد يكون جلطة رئوية أو انسدال رئة.\*\* \*\*وراجع فوراً عند سعال دموي.\*\* وراجع طبيبك خلال أيام إن تجاوز السعال أسبوعين مع حرارة وتعرّق ليلي ونزول وزن، لأن هذا يستوجب فحصاً لاستبعاد السلّ، أو إن عادت الحرارة بعد تحسّن، أو زادت حاجتك للبخّاخ.

## About this service

Bronchitis is inflammation of the tubes that carry air into the lungs. Their walls swell and produce more mucus, giving a cough with sputum, wheeze, breathlessness, and chest tightness. It has two forms whose management differs completely: an acute form lasting days to weeks, and a chronic form defined as a productive cough on most days for three months a year over two consecutive years.

Acute bronchitis is viral in most cases and settles by itself, yet the cough afterwards can linger for two to three weeks. That does not mean treatment failed and does not mean bacteria have taken over. Antibiotics do not shorten viral bronchitis or prevent its complications; they are prescribed only when the physician judges that a bacterial infection or a flare of chronic lung disease is present. Core care is supportive: fluids, rest, fever control, avoiding smoke and cold dry air, with a bronchodilator when wheeze is present and the doctor advises it.

Chronic bronchitis is a different story. It usually sits within the spectrum of chronic obstructive pulmonary disease and must be confirmed with lung function testing rather than assumed. Here is the fact that should not be softened: stopping smoking is the only intervention that genuinely slows the decline in lung function. Everything else — inhalers, pulmonary rehabilitation, vaccination, oxygen — relieves symptoms, reduces flare-ups, and improves quality of life, but does not halt that decline. Anyone who keeps smoking keeps deteriorating no matter how many medications are added.

A damaging myth needs correcting: inhalers are not addictive and do not lose their effect with use. Needing one often indicates uncontrolled disease that warrants a review of the plan, not habituation, and delaying it out of that fear increases risk. Cough suppressants are also not a treatment for a productive cough, since they trap sputum in the airways instead of clearing it.

Finally, not every cough is bronchitis. A cough lasting more than two weeks with fever, night sweats, and weight loss requires sputum testing and a chest X-ray to exclude tuberculosis, which still occurs in Jordan; its treatment runs for months and must be completed in full even after symptoms disappear.

## Preparation

Before the visit, record how long the cough has lasted in days or weeks, what the sputum looks like, whether blood has appeared, and when the cough worsens: at night, after exertion, around dust, or after eating. Note how many prolonged coughing illnesses you have had in past years and how many antibiotic courses you took. Bring every inhaler device you use so you can demonstrate your technique, because poor technique is a common reason treatment appears to fail. Report your smoking history in years, including waterpipe and vaping, your occupation and any exposure to dust, fumes, or paints, plus your chronic conditions, medications, and drug allergies. Also mention contact with anyone who has a prolonged cough or tuberculosis.

## Aftercare

Drink enough fluids, rest, and stay completely away from smoke, waterpipe, and vaping, since smoke exposure prolongs a cough by weeks. Use your inhaler exactly as you were taught, with the spacer if one was prescribed, and do not stop preventive treatment because you feel better. Expect the cough to persist for two to three weeks after acute bronchitis, and record how often you needed your reliever inhaler so you can report it. \*\*Go to the emergency department immediately if breathlessness stops you completing a full sentence, if your lips turn blue, or if you become confused.\*\* \*\*Seek emergency care for sharp chest pain with sudden breathlessness, which may indicate a pulmonary embolism or a collapsed lung.\*\* \*\*Seek urgent care for coughing up blood.\*\* See your doctor within days if the cough passes two weeks alongside fever, night sweats, and weight loss, since tuberculosis must be excluded, if fever returns after improvement, or if your reliever use increases.
