العلاج ثلاثة أشياء متلازمة لا يُجزئ أحدها عن الآخر: إزالة الجير واللويحة في العيادة وتلميع الأسنان، وتدريب عملي على تقنية التفريش والخيط لأن معظم النتيجة يحدث في المنزل، ومعالجة العوامل التي تجعل التراكم أسهل — حشوة أو حافة تاج مخالفة، تسوّس بين الأسنان، أجهزة تقويم أو جسر قديم، تنفّس فموي، جفاف الفم، تغيّرات الحمل، وسوء ضبط سكّر الدم. وقد يُوصف غسول مطهّر لفترة محدودة كمساعد مؤقّت لا كبديل عن الفرشاة.

خرافات شائعة يجب تصحيحها: النزيف عند التفريش ليس سبباً للتوقّف عن تفريش المكان — بل هو علامة الالتهاب، والتوقّف يزيده؛ نظّف بلطف بفرشاة ناعمة واستمرّ. ولا تكوِ اللثة ولا تفرك عليها ملحاً خشناً أو ليموناً أو مواد كاوية، فذلك يزيد الضرر ويؤذي الطبقة السطحية. ولا يُشخَّص التهاب اللثة من صورة على الهاتف. واستخدام الغسول المطهّر شهوراً بلا إشراف قد يُصبغ الأسنان ويغيّر حسّ الطعم.

حدود العلاج: تنظيف الجير لا يبيّض الأسنان ولا يُصلح تصبّغاً داخلياً، ولا يُغني عن العناية اليومية، ولا يُعيد عظماً إن كان قد فُقد فعلاً، وليست كل رائحة فم سببها اللثة — فقد تكون من اللسان أو الجيوب الأنفية أو المعدة أو التنفّس الفموي أو جفاف الفم.

وبعض ما يبدو «التهاب لثة» ليس كذلك: تضخّم اللثة قد يكون أثراً جانبياً لأدوية معيّنة تُوصف للضغط أو الصرع أو بعد زرع الأعضاء، والنزيف العفوي الغزير أو التضخّم السريع قد يكون أوّل علامة لمشكلة في الدم — خصوصاً عند الأطفال والشباب — وتقرّحات مؤلمة سريعة الظهور مع حرارة وتعب عامّ تستوجب فحصاً عاجلاً لا مجرّد غسول. لهذا يبقى الفحص السريري هو ما يحدّد التشخيص والخطة.

## خطوات الإجراء

### 1. الفحص والتفريق بين التهاب اللثة والتهاب دواعم السن

يُقاس عمق الجيوب بمسبار مُدرَّج ويُلاحَظ النزيف عند السَّبر، وتُطلب صور شعاعية عند الشكّ في فقد عظم. هذه الخطوة هي التي تُحدّد إن كانت حالتك قابلة للشفاء الكامل أم تحتاج خطّة أطول.

### 2. إزالة الجير واللويحة والتلميع

يُزال الجير من أسطح الأسنان وحدّ اللثة بجهاز الموجات فوق الصوتية وأدوات يدوية، ثم تُلمَّع الأسنان لتصبح أسطحها أقلّ التقاطاً للويحة. لا يحتاج ذلك تخديراً في معظم حالات التهاب اللثة.

### 3. تدريب عملي على التفريش والخيط

تتعلّم أمام المرآة زاوية الفرشاة الصحيحة على حدّ اللثة، وطريقة تمرير الخيط أو فرشاة بين الأسنان، والمناطق التي تُنسى عادة كالأسطح الداخلية والأضراس الخلفية. هذه أهمّ خطوة في النتيجة.

### 4. معالجة العوامل المساعدة

تُصحَّح حشوة أو حافة تاج مخالفة، ويُعالَج التسوّس بين الأسنان، وتُراجَع أجهزة التقويم أو الجسور القديمة، ويُقيَّم جفاف الفم والتنفّس الفموي وضبط سكّر الدم وتغيّرات الحمل ودواء قد يُضخّم اللثة.

### 5. إعادة التقييم وخطة المتابعة

تُعاد مراجعة اللثة بعد أسبوعين إلى أربعة أسابيع: إن توقّف النزيف وعادت اللثة إلى لونها فالخطة نجحت وتُحدَّد فترة المتابعة الدورية؛ وإن بقي النزيف تُبحث الأسباب أو يُعاد التشخيص نحو التهاب دواعم السن.

## Procedure steps

### 1. Examination and distinguishing gingivitis from periodontitis

Pocket depths are measured with a graduated probe and bleeding on probing is recorded, with radiographs requested whenever bone loss is suspected. This step determines whether your condition is fully reversible or requires a longer treatment plan.

### 2. Scaling, plaque removal and polishing

Calculus is removed from the tooth surfaces and the gum margin with an ultrasonic scaler and hand instruments, and the teeth are then polished so their surfaces retain less plaque. In most cases of gingivitis this requires no anaesthesia.

### 3. Hands-on brushing and flossing coaching

In front of a mirror you learn the correct brush angle at the gum margin, how to pass floss or an interdental brush, and the areas most often missed, such as inner surfaces and back molars. This is the single most decisive step in the outcome.

### 4. Addressing contributing factors

An overhanging filling or crown margin is corrected, decay between the teeth is treated, orthodontic appliances and old bridgework are reviewed, and dry mouth, mouth breathing, blood sugar control, pregnancy-related changes and any medicine that can cause gum overgrowth are assessed.

### 5. Reassessment and follow-up plan

The gums are reviewed after two to four weeks. If bleeding has stopped and colour has returned, the plan has worked and a recall interval is set. If bleeding persists, the contributing causes are re-examined or the diagnosis is revised towards periodontitis.

## التحضير قبل الإجراء

## العناية بعد الإجراء

توقّع حساسية بسيطة ونزيفاً خفيفاً في اليوم أو اليومين الأولين بعد إزالة الجير، ثم تحسّناً واضحاً خلال أسبوع إلى أسبوعين إن التزمت بالعناية. فرّش مرّتين يومياً بفرشاة ناعمة وبحركة لطيفة على حدّ اللثة، ونظّف بين الأسنان يومياً بالخيط أو الفرشاة البينية، ولا تُهمل السطح الداخلي والأضراس الخلفية. استخدم الغسول الموصوف للمدّة المحدَّدة فقط، ولا تجعله عادة دائمة. اشرب ماءً كافياً، وقلّل السكّريات والوجبات المتقطّعة بين الوجبات، وأقلع عن التدخين، واضبط سكّر الدم إن كنت مصاباً بالسكري. راجع في الموعد المحدَّد لإعادة التقييم حتى إن شعرت أن اللثة تحسّنت. \*\*راجع فوراً عند نزيف لثة عفوي لا يرتبط بالتفريش أو لا يتوقّف، أو تضخّم سريع في اللثة أو كتلة لا تزول، أو تقرّحات مؤلمة تظهر بسرعة مع حرارة وتعب عامّ ورائحة نفّاذة، أو تورّم في الوجه أو تحت الفكّ، أو صعوبة في البلع أو فتح الفم، أو ألم نابض مع طعم قيح، أو نزيف عند مريض يتناول مضاداً للتخثّر.\*\*

## About this service

Gingivitis is superficial inflammation confined to the gum tissue, caused by bacterial plaque accumulating on and around the teeth. It shows as red, swollen gums that bleed on brushing or flossing, an unpleasant breath odour, and an itchy or uncomfortable sensation at the gum margin. Its defining feature is that it is fully reversible: the bone supporting the tooth has not yet been affected, so removing the cause returns the gum to its normal state. Once the inflammation extends into bone the condition becomes periodontitis, and what has been lost is not recovered by cleaning alone.

Treatment rests on three inseparable elements. Calculus and plaque are removed in the clinic and the teeth are polished. You receive hands-on coaching in brushing and flossing technique, because most of the result is produced at home. And the factors that make plaque accumulate are addressed, such as an overhanging filling or crown margin, decay between the teeth, orthodontic appliances or an old bridge, mouth breathing, a dry mouth, pregnancy-related changes and poorly controlled blood sugar. An antiseptic rinse may be prescribed for a limited period as a temporary aid, never as a substitute for the brush.

Several common myths need correcting. Bleeding while brushing is not a reason to stop brushing that area; the bleeding is a sign of inflammation and stopping makes it worse, so clean gently with a soft brush and continue. Do not cauterise the gums or rub coarse salt, lemon or caustic substances on them, as this only adds injury to the surface tissue. Gingivitis cannot be diagnosed from a phone photograph. And using an antiseptic mouthwash for months without supervision can stain teeth and alter taste.

There are clear limits. Scaling does not whiten teeth and will not correct internal discolouration, it does not replace daily care, and it does not restore bone that has genuinely been lost. Not all bad breath comes from the gums either: it may originate from the tongue, the sinuses, the stomach, mouth breathing or a dry mouth.

Some conditions that look like gingivitis are something else. Gum overgrowth can be a side effect of certain medicines prescribed for blood pressure, epilepsy or after organ transplantation. Heavy spontaneous bleeding or rapid swelling may be the first sign of a blood disorder, particularly in children and young adults. And painful ulceration appearing quickly with fever and general fatigue needs urgent assessment rather than a mouthwash. This is why clinical examination, not self-diagnosis, sets the diagnosis and the plan.

## Preparation

Tell your dentist about all your medicines, especially anticoagulants, antiplatelet drugs, blood pressure and epilepsy medication and post-transplant drugs, since some of these cause gum overgrowth, and do not stop any of them on your own. Mention diabetes and how well controlled it is, pregnancy or breastfeeding, and any immune deficiency, blood disorder or recurrent bleeding from the nose and gums. Do not start an antiseptic rinse in the days before the visit if it would mask the true severity of the inflammation; come as you actually are. Do not stop brushing because the gums bleed, but continue gently with a soft brush. Bring a written medication list and any earlier radiographs, and note down your questions about bleeding or breath odour.

## Aftercare

Expect mild sensitivity and slight bleeding for a day or two after scaling, then clear improvement over one to two weeks if you keep to the plan. Brush twice daily with a soft brush using gentle strokes along the gum margin, clean between the teeth every day with floss or interdental brushes, and do not neglect the inner surfaces and back molars. Use the prescribed rinse only for the stated period and do not make it a permanent habit. Drink enough water, reduce sugars and frequent snacking, stop smoking, and keep blood sugar controlled if you are diabetic. Attend the scheduled reassessment even if the gums already feel better. \*\*Seek care immediately if you have spontaneous gum bleeding unrelated to brushing or that will not stop, rapid gum swelling or a lump that does not resolve, painful ulcers appearing quickly with fever, fatigue and a strong odour, swelling of the face or under the jaw, difficulty swallowing or opening the mouth, throbbing pain with a taste of pus, or any bleeding while taking an anticoagulant.\*\*
