## Procedure steps

### 1. Clinical diagnosis and localising the pain

You are asked about first-step morning pain and how the pain relates to standing and walking distance. The origin of the fascia on the heel is then palpated, and Achilles flexibility, ankle movement and heel alignment are measured. The clinician looks for features suggesting another cause, such as numbness, night pain or multiple swollen joints, and imaging is requested only when needed, since the diagnosis is usually clinical.

### 2. Load and footwear modification

Activities that provoke the pain are reduced without stopping movement altogether, and flat, worn-out shoes are replaced with footwear that has a slight heel, arch support and a sole that does not bend in the middle. Walking barefoot on tiled floors is discouraged, and excess weight is addressed where present, since it influences the speed of recovery more than any single device.

### 3. The daily exercise programme

This is the core of treatment: stretching the plantar fascia by pulling the toes upwards before getting out of bed, stretching the Achilles tendon and calf, and progressive strengthening such as graded heel raises and work for the intrinsic muscles of the sole. The exercises are done daily for weeks, and you are told that discomfort may increase slightly at first before it settles, and that consistency matters more than intensity.

### 4. Local support and physiotherapy

A supportive insole, heel cushion or custom orthosis is added where appropriate, along with local cooling after activity, and taping or a night splint that holds the foot stretched may be used to reduce morning pain. Physiotherapy is directed at stretching, strengthening, deep soft tissue work and gait correction, and you are taught the programme so that you can continue it at home.

### 5. Escalating care in resistant cases

If pain persists after adequate adherence over several months, the diagnosis is reconsidered first, and options such as shockwave therapy or a selected injection are then discussed with their limits and risks stated frankly. Surgery is reserved for the rare, highly resistant case, and is not performed simply to remove a bone spur.

## About this service

The plantar fascia is a strong fibrous band running from the heel bone to the front of the foot, acting as the tie-beam of the arch that holds the curve of the sole. Plantar fasciitis is irritation and degeneration of this band at its origin on the heel, caused by repeated load beyond what the tissue can tolerate. It therefore affects people who stand or walk for long hours on hard floors, those who have gained weight, those who suddenly increase activity or change footwear, and those with tightness in the calf and Achilles tendon.

Its hallmark is distinctive: sharp pain under the heel with the first steps in the morning or after sitting for a while, easing after a few minutes of walking, then returning after prolonged standing or walking, with tenderness concentrated at one specific point inside the heel that can be found with a fingertip. The diagnosis is clinical in most cases, and imaging is requested only when another cause is suspected, such as a stress fracture, nerve entrapment or inflammatory disease.

One widespread myth deserves correction. The so-called heel spur seen on X-ray is a consequence of chronic traction, not the cause of the pain. Many people have a spur on imaging with no pain at all, and removing it is not the solution. The practical truth is that plantar heel pain improves in the great majority with regular exercise, graded loading and footwear change without any surgery, but it needs time measured in weeks and months rather than days.

The limits of treatment must be stated plainly. A supportive insole alone does not cure the condition, a single physiotherapy session changes nothing, and no injection ends the problem in one dose. Repeated steroid injections can weaken the fascia itself or the fat pad under the heel, and their use has clear limits set by your doctor. Surgery is a rare exception reserved for long-standing resistant cases after proper conservative care has been exhausted. The decisive factor in the outcome is your daily consistency with stretching the fascia and Achilles tendon and strengthening the muscles of the sole and calf, together with graded increases in distance and load, and avoiding walking barefoot on hard tiled floors. Suitability for any advanced step is determined by clinical examination, not by reading.

## Preparation

Before your visit, note how your pain behaves: whether it is worst with the first step in the morning or after activity, how far you can walk before it appears, and whether there is any numbness or tingling spreading into the toes. Bring your everyday and sports shoes, including old pairs, because their wear pattern reveals how you load the foot, and bring any insoles you have already tried. Tell the doctor your weight and any recent gain, the nature of your work and how many hours you stand, any sudden change in sporting activity, and any previous heel or ankle injury. Mention whether you have diabetes, rheumatoid arthritis, spondyloarthritis or gout, list all your medicines, and if you have had heel injections before, state how many and when, because that changes the treatment decision.

## Aftercare

Do your stretching and strengthening exercises every single day and do not stop at the first improvement, since stopping early is the commonest reason the pain returns. Avoid walking barefoot on hard tiled floors, wear supportive footwear even indoors, and increase your walking distance gradually rather than returning to your previous activity all at once. Apply ice to the heel after activity, and reduce excess weight, as this lowers the load on the fascia with every step. Expect gradual improvement over weeks and months, not days. \*\*See your doctor if you have heel pain at night that wakes you or persistent pain at rest, numbness or tingling spreading into the sole and toes, a sudden sharp pain with a popping or tearing sensation, morning stiffness and swelling in other joints together with low back pain, or if there is no improvement at all after months of consistent treatment.\*\*
