## Procedure steps

### 1. Clinical assessment of throat and neck

The tonsils, pharynx, tongue and cervical lymph nodes are examined and temperature measured, with questions about duration of pain, swallowing, cough and the number of previous episodes, to judge whether the picture is viral or bacterial.

### 2. Investigations where needed

A throat swab or rapid streptococcal test may be taken where there is clinical suspicion, and blood tests may be requested in severe or prolonged cases. Not every sore throat needs a swab or a blood test.

### 3. Supportive care and pain control

Analgesia suited to your circumstances is prescribed, with adequate fluids, soft cool or lukewarm foods and salt-water gargles. Irritants are avoided and rest is advised, since pain here genuinely interferes with drinking and nutrition.

### 4. The antibiotic decision

An antibiotic is prescribed where streptococcal infection is suspected or the clinical picture is severe, chosen with your drug allergies in mind and completed in full. Leftover antibiotics from a previous episode are not used, and treatment is not stopped when pain resolves.

### 5. Assessing the need for surgery

If episodes recur in a documented pattern, or an abscess or sleep-disordered breathing occurs, tonsillectomy is discussed with an explanation of recovery and risks. Documented dates of episodes across the year are what support this decision.

## About this service

Treating tonsillitis in adults begins with identifying the cause, not with prescribing an antibiotic. Tonsillitis may be viral, which is the commonest scenario, or bacterial, most importantly group A streptococcus. The clinician distinguishes them on clinical grounds: the appearance of the tonsils and any exudate, body temperature, tender enlarged neck lymph nodes, and the absence of cough and coryzal symptoms, supported where needed by a throat swab or a rapid test.

For viral episodes the mainstay is supportive care: rest, adequate fluids, analgesia chosen by the clinician according to your health status, gargling with warm salt water, and avoiding irritants such as cigarettes and waterpipe smoke. Where streptococcal infection is suspected, an appropriate antibiotic is prescribed and completed for its full course even if symptoms settle quickly, because the aim is not only pain relief but reducing the risk of complications such as rheumatic fever and peritonsillar abscess.

Tonsillectomy is not a treatment for any single episode. It is considered in defined situations: documented recurrent episodes recorded by number and date over a year or more, recurrent peritonsillar abscess, obstructive sleep-disordered breathing linked to tonsillar enlargement, or clinical suspicion of a tumour. In adults recovery is longer and more painful than in children, usually around ten to fourteen days, with a risk of secondary bleeding that requires immediate review.

The limits stated frankly: tonsillectomy does not prevent colds, pharyngitis or sinus infections, it does not weaken immunity as is commonly believed, and it does not resolve bad breath arising from the teeth, the sinuses or the stomach. Antibiotics do not help viral tonsillitis or shorten it, and expose you to side effects and bacterial resistance. Small white plugs in the tonsillar crypts are also a common finding and do not necessarily indicate chronic infection.

An important caution in adults: enlargement of one tonsil only, ulceration or bleeding on it, or persistent throat or ear pain with hoarseness, weight loss or a neck lump, must not be managed as recurrent tonsillitis but assessed without delay, particularly in smokers. Suitability for any step, medical or surgical, is determined by clinical examination rather than by reading a symptom list.

## Preparation

Keep a written record of the dates of your sore-throat episodes over the past year, what was prescribed each time and how many days of work you missed, since this record underpins any later surgical decision. Tell the doctor about drug allergies, especially to penicillin, about all medication you take including blood thinners and steroids, about heart, kidney or liver disease, and about any previous peritonsillar abscess or bleeding. Mention whether you smoke cigarettes or use a waterpipe. Do not start an antibiotic on your own before assessment, as it confounds the findings, and do not gargle with caustic substances. If you cannot swallow or open your mouth, or you feel short of breath, do not wait for an appointment; seek urgent care.

## Aftercare

Drink plenty of fluids even if swallowing hurts, because dehydration increases pain and delays recovery, and choose soft lukewarm or cool foods while avoiding spicy, acidic and crunchy items. Complete any prescribed antibiotic in full and take analgesia on a regular schedule rather than only when pain peaks. Stop smoking and waterpipe use entirely during treatment. After adult tonsillectomy, expect pain referred to the ears and a white layer over the tonsil beds, which is normal, and follow the fluid, analgesia and activity restrictions your surgeon sets. Seek immediate care for any bleeding from the mouth or spitting of blood after surgery, however small; inability to swallow or to open the mouth; a muffled voice with drooling and breathing difficulty; painful one-sided neck swelling; persistent fever or no improvement after three days of antibiotics; a rash, joint pain or dark urine after the infection; or one enlarged tonsil or an ulcer that does not heal.
