Thalassemia Treatment

## Procedure steps

### 1. Laboratory diagnosis and typing

A complete blood count with red cell indices and a blood film are performed, followed by hemoglobin electrophoresis or chromatography to define the type of thalassemia, with iron studies to distinguish it from iron deficiency. Genetic study of the patient and family members may be requested. This step prevents years of mistaken iron treatment.

### 2. Grading severity and the transfusion decision

The doctor assesses baseline hemoglobin, growth in children, the size of the spleen and liver, bone changes and symptoms, in order to decide whether this is a trait needing no treatment, an intermediate form transfused as required, or a regularly transfusion-dependent form. A target level before each transfusion is then set.

### 3. A safe transfusion programme

Careful blood grouping and cross-matching are performed, and leukocyte-reduced blood is preferred to lower reactions and antibody formation. Every session and the hemoglobin before and after it are recorded in the patient's log. The patient is observed during and after the transfusion for fever, chills, breathlessness or a change in urine colour.

### 4. Monitoring and treating iron overload

Iron stores are measured regularly and cardiac and hepatic iron are assessed with dedicated magnetic resonance imaging, because blood tests alone do not reveal cardiac loading. A chelating agent chosen by the doctor is prescribed with monitoring of kidney and liver function, hearing and vision. Adherence to chelation is the single most protective measure for the heart and must not be stopped by the patient.

### 5. Multidisciplinary follow-up

Endocrine function, growth, puberty, diabetes, bone density, cardiac and liver function, hepatitis serology and vaccinations are followed, and the state of the spleen and any need for splenectomy are reviewed. Genetic counselling is offered to the family, and stem cell transplantation or newer therapies are considered for selected patients within a specialist centre.

## About this service

Thalassemia is an inherited disorder of hemoglobin chain production that makes red blood cells small and short-lived, producing chronic anemia of varying degrees. The spectrum runs from the silent carrier of the trait, who usually has no complaints, through an intermediate form needing occasional transfusion, to the major transfusion-dependent form present from early childhood. There is therefore no single treatment for thalassemia, but a continuing care programme built on the type of defect, its severity and the results of follow-up.

Treatment begins with precise laboratory diagnosis: a blood count with red cell indices, hemoglobin electrophoresis or chromatography, and sometimes genetic study to identify the mutation. This step is essential because thalassemia trait resembles iron deficiency on the blood count and is frequently misdiagnosed, with the result that patients receive iron they do not need for years. Taking iron without laboratory proof of deficiency is a genuine error in thalassemia, because the problem is not lack of iron but faulty hemoglobin production, and iron accumulation is one of the most dangerous complications of the disease.

A care programme usually combines regular transfusion for those who need it, to protect growth and prevent bone deformity and an enlarged spleen; monitoring of iron overload and treating it with chelation therapy chosen by the doctor; and surveillance of the heart, liver, endocrine glands, bones, fertility and vaccinations. Folic acid is added when needed and the state of the spleen is assessed. In selected cases stem cell transplantation is discussed as a potentially curative option, and newer treatments can reduce transfusion needs in specific types. All of these are decisions for a specialist centre and cannot be settled by reading.

The limits are clear. Transfusion and iron chelation preserve health and prevent complications but do not correct the inherited defect. Carriers of the trait are not patients and need no treatment, but they do need genetic counselling before marriage and pregnancy, because two carriers together raise the chance of a child with the major form. Dietary supplements do not substitute for transfusion, and chelation must not be stopped because tests have improved, since iron stored in the heart is not always reflected in a simple blood test. Any plan is decided through clinical examination and laboratory and imaging follow-up with a hematologist.

## Preparation

Bring your transfusion log, blood group card, latest laboratory results and any magnetic resonance reports, together with a list of your medicines including your chelating agent and folic acid. Tell the team before the session if you have fever, cough, diarrhoea or any infection, as the transfusion may be postponed. Do not start iron supplements or iron-containing vitamins on your own, and do not take high-dose vitamin C without advice, as it can increase the effect of stored iron. Report any previous transfusion reaction or drug allergy. If you are planning marriage or pregnancy, ask for genetic counselling and joint carrier testing with your partner beforehand.

## Aftercare

Keep to your transfusion appointments and take your chelating agent every day as prescribed, because consistency is what protects the heart, liver and endocrine glands. Complete the periodic blood tests and the hearing and vision checks your doctor requests, and keep your record updated after every session. Expect mild tiredness after a transfusion, and a brief low-grade fever may occur. \*\*Contact the team immediately or attend the emergency department if you develop fever and chills during or after transfusion, low back pain, tea-coloured or bloody urine, breathlessness, chest pain or palpitations, rash with itching and facial swelling, deepening yellowing of the eyes, or severe abdominal pain with rapidly increasing pallor.\*\* If your spleen is enlarged or has been removed, treat any fever as an emergency without waiting, and carry a card describing your condition.
