## Procedure steps

### 1. Perfusion assessment first

The doctor feels the foot pulses and measures the ankle-to-arm pressure ratio, adds toe pressure or skin oxygen measurement when needed, and then requests duplex ultrasound or CT angiography. This step separates an ischaemic wound from other wounds and determines everything that follows.

### 2. Assessing the wound, infection and bone

The wound is measured and its depth, edges and tissue type described, and the team looks for pus, odour, spreading redness or exposed bone. A swab or tissue sample is taken if infection is suspected, the bone is imaged if the wound is deep or long-standing, and blood sugar, nutrition and kidney function are reviewed.

### 3. Restoring blood flow before anything else

If ischaemia is confirmed, revascularisation is planned — balloon and stent by catheter, or a surgical bypass — according to the site and length of the blockage and your general condition. Postponing this step for weeks of dressings is the commonest reason wounds deteriorate and toes are lost.

### 4. Debridement and local wound care

Dead tissue and accumulated slough are removed in stages, in clinic or in the operating theatre depending on the amount, and any collection of pus is drained. Dressings are chosen to balance moisture and changed at the recommended intervals, and treatment for infection is added on the doctor's decision, not by self-experiment.

### 5. Offloading and preventing recurrence

Pressure is taken off the wound with therapeutic footwear, a cast, a frame or a stick, and foot deformity or gait problems are corrected where needed. A permanent plan is put in place: daily foot inspection, professional nail care, control of blood sugar, cholesterol and blood pressure, stopping smoking, and imaging follow-up of perfusion.

## About this service

Treatment of ischaemic wounds is a coordinated plan for a wound that will not heal because the blood reaching it is not enough to build new tissue or fight bacteria. The governing rule is simple and unforgiving: no wound heals without blood. Care therefore begins with assessing perfusion, not with choosing a dressing, and if the arterial supply is inadequate, restoring flow — by catheter or by surgery — comes before everything else.

Assessment starts with feeling the foot pulses and measuring the ankle-to-arm pressure ratio, adding toe pressures or skin oxygen measurement where needed, followed by arterial imaging to locate the blockage. At the same time the contributing causes are sought: repeated pressure over the wound site, foot deformity, infection in the bone, venous congestion, poor nutrition, uncontrolled blood sugar, and smoking. Smoking is the strongest modifiable factor in arterial disease, and continuing to smoke cancels out the effect of any local treatment.

Once perfusion is secured, local work follows: removing dead tissue, draining infection, dressings that keep a balanced level of moisture, and taking the load off the wound with special footwear, a cast or a walking aid. Bone infection is assessed by imaging and may require longer treatment or an operation.

The limits must be said plainly. Dressings, ointments and antibiotics alone do not heal an ischaemic wound and are no substitute for restoring blood flow. If you have diabetes, remember that \*\*the absence of pain is false reassurance\*\*: nerve damage hides both the wound and the infection until they are advanced, which is why regular foot inspection is non-negotiable. What treatment suits your wound is decided by clinical examination and perfusion studies — not by reading.

## Preparation

Come to the appointment with the wound covered only by a simple dressing that can be removed easily, and do not apply coloured solutions, oils, herbal preparations or powders that hide what the doctor needs to see. Write down when and how the wound started, which antibiotics or dressings have been used and for how long, and bring reports of any previous imaging or wound culture. Bring a complete list of your medicines and mention blood thinners, diabetes treatment and any drug allergy. Bring the shoes you normally wear — in many cases they explain the wound. Measure and record your blood sugar for several days beforehand, stop smoking now, and do not walk barefoot or warm the foot with hot water or heating devices.

## Aftercare

Change the dressing in the way and at the frequency you were taught, and do not improvise with products of your own. Keep the wound out of shower water unless you are told otherwise, and keep offloading it: wear the therapeutic shoe, cast or stick even indoors and even for a few steps. Inspect both feet every day with a mirror or with help, and run your hand inside your shoes before putting them on. Do not cut your own nails or dead skin and do not use corn-removing plasters. Control your blood sugar and complete your stop-smoking plan. Remember that \*\*the absence of pain does not mean improvement\*\* if your sensation is reduced.

Seek care immediately if: redness spreads, or there is swelling or warmth around the wound · pus, a foul smell, or a rapid increase in wound size appears · you develop fever, shivering or a sudden rise in blood sugar readings · \*\*a toe turns black, or becomes numb or cold\*\* · \*\*the limb becomes cold, pale, painful and pulseless\*\* (possible acute arterial occlusion — hours decide whether the limb survives) · \*\*a leg suddenly becomes painful, swollen and warm\*\* (possible deep vein thrombosis — do not massage the leg and do not wait) · \*\*chest pain or breathlessness\*\* follows a leg clot (possible pulmonary embolism — emergency).
