## Procedure steps

### 1. Characterising the pattern of deviation

The clinician asks when the squint appears, how often and when it worsens, separating a controlled intermittent deviation from one present most of the time, since management differs.

### 2. Refraction and visual acuity

The refractive error and the acuity of each eye are measured, because reduced vision in one eye changes the plan and calls for amblyopia treatment first.

### 3. Assessing control and binocular vision

The angle is measured with prisms at near and distance, and the brain's ability to fuse the two images and the eyes' convergence during reading are evaluated.

### 4. Non-surgical management

Glasses are prescribed and, when indicated, targeted convergence exercises, part-time patching or prisms are used, with regular review to measure any improvement in control.

### 5. Surgery when monitoring is no longer enough

The eye muscles are adjusted when the deviation becomes large, frequent or damaging to binocular vision, with long follow-up because it can recur.

## About this service

Exotropia is an outward deviation of one or both eyes away from the nose, so that one eye fixes on the target while the other drifts to the side. It is less common than esotropia in childhood but frequent in its intermittent form, and it can also appear in adults.

The commonest presentation is intermittent exotropia: the eyes are straight most of the time, then one drifts outward with tiredness, daydreaming, illness or when looking into the distance. Characteristic signs include a child closing one eye or screwing up both eyes in bright sunlight, and parents noticing the deviation at the end of the day rather than at the start. A separate pattern is linked to weak convergence during reading, causing headache, eye strain and letters appearing to double during close work.

Treatment is built on measuring how large the deviation is, how often it occurs and how well the brain controls it. The plan begins with correcting any refractive error, and may include targeted exercises or part-time patching for poor convergence or to control a small deviation, prisms incorporated into glasses, and surgery when the deviation becomes large, occurs most of the time, or threatens binocular vision. Regular monitoring without active treatment is a legitimate option in mild, well-controlled cases, and that decision is made by clinical examination rather than by reading.

The limits should be stated plainly. Exercises help weak convergence but do not correct a large or constant exotropia, and glasses will not straighten a deviation unrelated to a refractive error. \*\*Surgery repositions the eye but does not raise the vision of an amblyopic eye, and it cannot guarantee perfect alignment from one procedure; the deviation may recur or reverse inward and need further intervention.\*\* Most importantly, \*\*a new outward squint appearing suddenly with double vision, headache, a drooping eyelid or a change in pupil size needs urgent assessment, because it may point to a neurological cause.\*\*

## Preparation

Before the appointment, note your observations precisely: at what times the deviation appears, which eye drifts, whether it increases with tiredness, at the end of the day or in sunlight, and whether the child closes one eye. Bring photographs or short video clips showing the deviation if you can, as these are very useful in intermittent exotropia, which may not appear during the examination itself. Tell the doctor about headache, eye strain or letters doubling while reading, about any previous head injury, about neurological or thyroid disease, and about every regular medicine. Mention any family history of squint or lazy eye, and bring the current glasses, their prescription and previous reports. Expect dilating drops that leave vision blurred for hours, so arrange a driver or an adult to accompany your child.

## Aftercare

Wear prescribed glasses all day and do not limit them to reading unless your ophthalmologist says so. If convergence exercises are prescribed, perform them regularly and exactly as demonstrated, because unsupervised, irregular exercises can give a false sense of improvement. Watch the deviation and record when it appears and how long it lasts, since this information is what the clinician uses to decide between continued monitoring and surgery. After an operation use the drops on schedule, avoid rubbing the eye, swimming and dusty environments until permitted, and expect redness lasting weeks and possibly temporary double vision. \*\*Seek care immediately for severe pain, a sudden drop in vision or coloured discharge, and for any new sudden squint accompanied by double vision, headache, vomiting, a drooping eyelid or unequal pupil sizes.\*\*
