## Procedure steps

### 1. Comprehensive assessment and defining the type

A detailed history is taken from the patient and from an informant who knows their daily life, covering when symptoms began, how they progressed and how they affect everyday tasks. Standardised cognitive testing of memory, language, attention and executive function follows, with a neurological and mood examination, defining the severity and the pattern that points to the most likely type.

### 2. Excluding treatable causes

Blood tests are requested including thyroid function, vitamin B12, glucose, electrolytes, liver and kidney function; the brain is imaged by MRI or CT; and every medicine and supplement is reviewed so that anything harmful to cognition can be stopped. Hearing, vision, sleep and mood are assessed, because much apparent decline has a fully treatable cause.

### 3. Building the drug and non-drug plan

The doctor decides whether medication is indicated according to the type and stage, explaining what it can and cannot be expected to do, and blood pressure, sugar and lipids are optimised. Alongside it, regular physical activity and balance work, social participation, meaningful activities linked to the person's own history and former occupation, and occupational or speech therapy are prescribed as needed.

### 4. Safety, environment and behaviour management

The home is simplified with a stable routine, a clear calendar, night lighting and organised medication, hazards such as gas, hot water, sharp tools and medicines are secured, and continuing to drive is discussed honestly. When agitation or wandering appears, the first search is for pain, infection, urinary retention, a new medicine, noise or a change of surroundings, before any sedating drug is considered.

### 5. Caregiver support and follow-up

The caregiver is trained in short, calm communication and in avoiding argument and repeated correction, tasks are shared within the family, respite is arranged, and the caregiver's strain and mood are screened regularly. Legal and financial planning is completed early, and the person is reviewed every few months to reassess function, medication and goals.

## About this service

Dementia is not the name of a single disease. It is a set of symptoms caused by progressive damage to brain cells, affecting memory, language, planning, judgement and behaviour to a degree that disrupts daily life. Alzheimer's disease is its commonest cause, followed by vascular dementia from disease of the brain's arteries, then other types such as Lewy body dementia and frontotemporal dementia, and two causes can coexist in the same person.

Distinguishing normal age-related forgetfulness from dementia is the first step in treatment. It is normal to be slow to recall a name or where the keys are and then remember, and to remain able to run the day, handle money and manage medicines. The warning sign is \*\*loss of everyday function\*\*: forgetting the way home, repeating the same question minutes later, becoming unable to manage medication, money or bills, leaving food on the stove, declining personal hygiene, confusion about time and place, and personality change with social withdrawal. \*\*These signs call for early assessment\*\* and should not be dismissed as simply old age.

Management begins with a structured assessment that excludes treatable conditions which mimic dementia, such as depression, an underactive thyroid, vitamin B12 deficiency, the effect of sedative and anticholinergic medicines, sleep apnoea, infection, normal-pressure hydrocephalus and a subdural bleed after a head knock. The assessment includes standardised cognitive testing, a neurological examination, blood tests and brain imaging, together with an account from someone who knows the person's daily routine, because the patient may not perceive the extent of the change.

\*\*No treatment available today cures dementia.\*\* The stated aims are to slow functional decline, improve quality of life, and preserve safety and independence for as long as possible. That rests on medication chosen by the doctor according to the type and stage, on controlling blood pressure, blood sugar and lipids and treating hearing and vision loss, sleep disturbance and depression, on regular physical activity, social contact and meaningful occupation, on a stable routine in a simplified, safe environment, and on a plan for agitation and wandering that looks first at physical and environmental causes before any sedating medication.

There are no proven memory boosters, supplements or devices that treat dementia, and anything marketed with a promise of cure deserves caution and a question to the doctor before purchase. Support for the caregiver is part of the treatment plan rather than an add-on: teaching communication techniques, sharing tasks among family members, arranging regular respite, screening the caregiver for strain and depression, and addressing legal, financial and driving decisions early, with the patient involved while they can still express their wishes. Suitability and the plan are decided by clinical assessment, not by reading.

## Preparation

Bring someone who knows the daily details to the assessment visit, because an informant's account matters as much as the examination itself. Before the appointment, write down specific examples of what has changed, when it began and how it progressed: episodes of forgetting, repeated questions, getting lost, mistakes with medicines or money, and changes in mood, sleep or behaviour. Bring every medicine and supplement in its box, especially sleeping tablets, sedatives, antihistamines and bladder or anti-nausea drugs, since some impair cognition. Bring previous blood tests, any brain imaging, diabetes and blood pressure records, and hearing and vision test results. Make sure the person wears their hearing aid and glasses during testing, because hearing loss invalidates the result. Write down your questions, and tell the team about the home arrangements and who provides care and for how many hours.

## Aftercare

Keep the daily routine fixed in timing and place, break tasks into short steps, use a large calendar, a clear clock and a task board, and organise medicines in a weekly box with a reminder. Make the home safe: turn the gas off when not in use, limit hot water temperature, put medicines and sharp or toxic materials out of reach, add night lighting to the bathroom, and post emergency numbers. Put an identification card in the person's wallet, or a tracker if they tend to wander. Maintain daily walking, social contact and regular sleep, do not correct every mistake or argue, and redirect the conversation towards reassurance and the next task. Attend review every three to six months for reassessment.

\*\*Seek medical care immediately if there is: sudden decline in cognition over hours or days; acute confusion or new hallucinations (which may be delirium from infection, medication, dehydration or urinary retention); fever, burning on urination or a cough; a fall with a head injury, especially in someone taking a blood thinner; a seizure; sudden weakness on one side or slurred speech; refusal to eat or drink; thoughts of harming self or others; or severe caregiver strain.\*\* Delirium is a medical emergency that is often mistaken for the dementia simply progressing.
