## Procedure steps

### 1. Patient selection and the prior diagnostic block

Clinical assessment of the pain pattern and its likely source, review of imaging, then a targeted diagnostic block. Clear temporary relief makes radiofrequency logical; absence of relief argues against it.

### 2. Preparation and fluoroscopic guidance

You usually lie face down, the skin is disinfected, monitoring is applied, and the fluoroscope angles are set to display the target precisely. You remain awake, with light sedation if needed, so you can respond during testing.

### 3. Needle placement and stimulation testing

The needle is advanced to the target, then gentle electrical stimulation is performed: sensory stimulation that reproduces your familiar pain confirms correct placement, and motor stimulation confirms the needle is clear of a motor nerve. This step is the key safety check.

### 4. Delivering controlled heat

The site is anaesthetised locally, then energy is delivered for a limited number of seconds at a controlled and monitored temperature, sometimes repeated at more than one point to cover the nerve's course. The pulsed mode is chosen when the target lies close to a nerve that must not be damaged.

### 5. Observation and linking the result to rehabilitation

A short observation period before discharge, documentation of pain levels and duration of relief over the following weeks, and starting or resuming a guided exercise programme, since unchanged faulty loading brings pain back however well the procedure went.

## About this service

Thermal radiofrequency treatment for disc-related pain is a non-surgical interventional procedure in which a special needle is advanced under X-ray guidance to a precise target near the source of pain. High-frequency current is then delivered through its tip, generating controlled heat that interrupts pain signal transmission in small sensory nerves or produces a limited change in the tissue of the disc's outer fibrous ring.

It is used in two distinct forms. Continuous radiofrequency raises the temperature enough to interrupt signal conduction and is widely used on the nerves supplying the facet joints, the medial branches, once a diagnostic block has shown the joint to be the pain source. Pulsed radiofrequency delivers energy in bursts at lower temperatures and is used cautiously near nerve roots that must not be damaged. When energy is applied inside the disc itself, the target is the annulus and its nerve endings, not the herniated fragment.

The limits need to be entirely explicit. Radiofrequency does not push a herniated disc back into place and does not remove the fragment pressing on a nerve. It does not widen a narrowed spinal canal, it does not repair spinal instability, and it does not regenerate cartilage. It is not the treatment for progressive muscle weakness or severe nerve compression, which require surgical assessment and are sometimes emergencies. Evidence for intradiscal application is also less established than evidence for medial branch treatment of facet joint pain, which makes careful patient selection the single most important factor in the outcome.

Relief is not permanent. After medial branch treatment the small nerves regenerate over time and pain typically returns after a period ranging from several months to around a year, at which point the procedure may be repeated. Above all it enables rehabilitation rather than replacing it: without trunk strengthening, load correction and weight management the underlying structural problem remains.

Candidacy is decided by clinical examination and by the result of a prior diagnostic block, not by an imaging report alone.

## Preparation

Tell your doctor about every medicine you take, especially blood thinners, anticoagulants and aspirin, and do not stop them yourself but ask about safe timing. Tell them if you have a pacemaker or any implanted electrical device, because this changes the settings used and must be known in advance. Mention diabetes, any allergy to contrast, local anaesthetic or antiseptics, and any active infection, fever, clotting disorder or possible pregnancy. Bring your images and reports along with the record of how you responded to the earlier diagnostic block. Ask in advance whether sedation will be used; if it will, follow the fasting instructions, arrange for someone to drive you, and do not drive yourself afterwards.

## Aftercare

Rest for the remainder of the day and move gently, avoiding heavy lifting, repeated bending and prolonged sitting for the first two days. Expect local soreness and a mild burning or sunburn-like sensation around the needle site for several days, sometimes up to a couple of weeks; this is common after radiofrequency and does not mean failure. Full benefit may take weeks to appear, so do not judge the result on day one. Monitor blood sugar if you are diabetic and steroid was added. Begin your exercise and trunk strengthening programme as pain allows, because the procedure enables rehabilitation and does not by itself treat the structural cause, and its effect is temporary by nature. \*\*Seek care immediately for new or worsening leg weakness, loss of bladder or bowel control, saddle numbness, fever and chills with worsening back pain, severe persistent numbness or burning along a nerve path, or redness and discharge at the needle site.\*\*
