وحدود العلاج يجب أن تُفهم بصراحة: عودة النظم إلى انتظامه لا تُلغي بنفسها قرار منع التخثّر، والجذ القثطاري ليس شفاءً مضموناً دائماً وقد يحتاج جلسة ثانية، وتخطيط قلب طبيعي في العيادة لا ينفي وجود رجفان يأتي ويذهب، فيلزم تسجيل ممتدّ لالتقاطه. وهناك اعتقاد شائع خطير يجب تصحيحه: أن «الأسبرين يكفي لمنع الجلطة الدماغية في الرجفان الأذيني» — مضادات الصفيحات ليست بديلاً مكافئاً لمضادات التخثّر في هذا الغرض، والقرار للطبيب المعالج وحده ولا يُبنى على تجربة أحد الأقارب ولا يُوقَف الدواء عند أول تحسّن.

والترشيح لكل خيار يُحدَّد بالفحص السريري وتخطيط القلب والتسجيل الممتدّ وتصوير القلب بالأمواج فوق الصوتية والتحاليل، لا بالقراءة. والالتزام بالمواعيد والتحاليل الدورية جزء من العلاج نفسه، وأي دواء يُوقَف أو يُبدَّل أو تُغيَّر كميته بأمر الطبيب فقط.

## خطوات الإجراء

### 1. تثبيت التشخيص وتوثيق النظم

يُسجَّل تخطيط القلب أثناء الأعراض إن أمكن، وإن كان الرجفان يأتي ويذهب يُستخدم تسجيل ممتدّ لأيام أو أسابيع. توثيق النظم ضروري لأن الخفقان قد يكون من أسباب أخرى، ولأن نوع الرجفان ومدّته يغيّران الخطة.

### 2. تقييم خطر السدّة الدماغية

يُحسب خطر الجلطة الدماغية بعوامل محدّدة، ويُوازن مع خطر النزف، ثم يُقرَّر منع التخثّر ونوعه. هذا القرار مستقلّ عن الأعراض وعن انتظام النبض، ويُعاد تقييمه دورياً لأن الخطر يتغيّر مع العمر والحالات المصاحبة.

### 3. البحث عن المحرّضات وعلاجها

تُطلب تحاليل الغدة الدرقية والدم، ويُقيَّم الضغط والوزن وانقطاع النفس النومي والكحول والكافيين المفرط، ويُصوَّر القلب بالأمواج فوق الصوتية لتقييم الصمّامات وعضلة القلب. علاج هذه العوامل يقلّل تكرار النوبات ويحسّن استجابة أي خطة لاحقة.

### 4. ضبط النظم أو ضبط السرعة

يُختار بين إعادة النظم بالأدوية أو بالصعق الكهربائي المنظّم تحت تهدئة قصيرة أو بالجذ القثطاري، وبين الاكتفاء بضبط سرعة النبض إذا كان ذلك أنسب لعمرك وحالتك. الاختيار يوازن بين الأعراض ووظيفة القلب واستجابتك السابقة للأدوية.

### 5. المتابعة طويلة الأمد

تُتابع الأعراض والنبض والضغط والوزن ووظيفة الكلى والتحاليل اللازمة لمانع التخثّر، ويُعاد تقييم الخطة إن تغيّرت الحالة. تعلّم قياس نبضك بنفسك وتسجيل الأعراض ومواعيدها يساعد الطبيب على قرار أدقّ.

## Procedure steps

### 1. Confirming the diagnosis and documenting the rhythm

An ECG is recorded during symptoms when possible, and if the fibrillation comes and goes, extended monitoring over days or weeks is used. Documenting the rhythm matters because palpitations can have other causes, and because the type and duration of the fibrillation change the plan.

### 2. Assessing stroke risk

Stroke risk is calculated from defined factors and weighed against bleeding risk, then anticoagulation and its type are decided. This decision is independent of symptoms and of whether the pulse is regular, and it is reassessed periodically because risk changes with age and coexisting conditions.

### 3. Finding and treating the triggers

Thyroid and blood tests are requested, blood pressure, weight, sleep apnoea, alcohol and excessive caffeine are assessed, and an echocardiogram evaluates the valves and heart muscle. Treating these factors reduces how often episodes recur and improves the response to any later plan.

### 4. Rhythm control or rate control

The choice is made between restoring rhythm with medication, planned electrical cardioversion under brief sedation, or catheter ablation, and simply controlling the heart rate when that suits your age and condition better. The choice balances symptoms, heart function and your previous response to medication.

### 5. Long-term follow-up

Symptoms, pulse, blood pressure, weight, kidney function and the tests required for anticoagulation are followed, and the plan is reassessed if your condition changes. Learning to check your own pulse and keeping a record of symptoms and when they occur helps your doctor make a more accurate decision.

## التحضير قبل الإجراء

## About this service

Atrial fibrillation is a disorder of the heart's electrical system in which the atria lose their organised activation, so the pulse becomes irregular and often fast. It may show itself as palpitations, unexplained tiredness, breathlessness on exertion or dizziness, and it may cause nothing noticeable at all and be found by chance when the pulse is checked or an ECG is recorded. For that reason the success of treatment is never judged by how the patient feels alone.

Management runs along two independent tracks, and neither replaces the other. The first is reducing the risk of stroke with anticoagulation, decided by a formal risk assessment based on factors such as age, high blood pressure, diabetes, heart failure and any previous clot — not by whether the pulse is regular and not by whether symptoms have gone. The second is rhythm control or rate control, using medication, planned electrical cardioversion, or catheter ablation, together with a search for treatable triggers such as an overactive thyroid, sleep apnoea, alcohol, anaemia, obesity and uncontrolled blood pressure.

The limits must be understood plainly. Restoring a regular rhythm does not by itself cancel the decision about anticoagulation; catheter ablation is not a guaranteed permanent cure and may require a second procedure; and a normal ECG in clinic does not rule out fibrillation that comes and goes, so extended monitoring is needed to capture it. One widespread and dangerous belief needs correcting: that aspirin is enough to prevent stroke in atrial fibrillation. Antiplatelet drugs are not an equivalent substitute for anticoagulants for this purpose, the decision belongs to the treating physician alone, it is not based on a relative's experience, and treatment is not stopped at the first sign of improvement.

Suitability for each option is determined by clinical examination, an ECG, extended rhythm monitoring, an echocardiogram and blood tests, not by reading. Keeping appointments and doing the periodic blood tests is part of the treatment itself, and any medicine is stopped, changed or adjusted only on your doctor's instruction.

## Preparation

Bring a written list of all your medicines and supplements, along with any previous ECG, monitor report or echocardiogram. Give a full account of your age and coexisting conditions: high blood pressure, diabetes, heart failure, kidney or liver disease, any previous stroke or bleeding event, and any past stomach bleeding, because all of these change the anticoagulation decision. Report your caffeine, alcohol and smoking honestly, and mention snoring, pauses in breathing during sleep or daytime sleepiness. Do not stop or adjust any medicine yourself before the appointment. If the appointment is for planned cardioversion or catheter ablation, follow the fasting period set by the team and arrange for someone to drive you.

## Aftercare

Take your medicines at fixed times, never stop or change your anticoagulant on your own even if your pulse becomes regular again, and keep up the periodic blood tests. Control your blood pressure and weight, cut down alcohol and excessive caffeine, and treat sleep apnoea if it is confirmed. Keep a record of your pulse, your symptoms and when they occur to help your doctor. Tell every doctor and dentist that you are on an anticoagulant before any procedure or tooth extraction. \*\*Seek care immediately — or call an ambulance — for sudden fainting with no warning at all, since a dangerous rhythm disorder can cause it and not merely low blood pressure; for palpitations with chest pain or breathlessness; for a very fast pulse that will not settle; for sudden weakness on one side, facial drooping, slurred speech or sudden loss of vision, which are signs of a stroke; and for unusual bleeding, black stools or blood in the urine.\*\*
