تُجرى العملية عادة بتخدير موضعي حول العين مع تهدئة خفيفة، وتستغرق عادة من 30 إلى 60 دقيقة، ويخرج المريض في اليوم نفسه غالباً. لكن العمل الحقيقي يبدأ بعدها: الأسابيع الست إلى الثمانية الأولى هي المرحلة الحرجة، وتتطلّب زيارات قريبة ومتكرّرة لضبط الضغط، وقد تشمل فكّ خيط أو تدليك الفقاعة أو حقن دواء مضاد للتليّف أو تعديل القطرات. الغياب عن هذه المواعيد هو أكثر ما يُفشل عملية ناجحة تقنياً.

حدود الجراحة بصراحة: \*\*هي تحمي ما بقي من البصر ولا تُعيد ما فُقد\*\*، ولا تُحسّن حدّة النظر؛ بل قد تتذبذب الرؤية وتضبّب لأسابيع بعدها، وقد تتغيّر درجة النظارة. وهي ليست شفاءً نهائياً: قد تحتاج قطرات مرة أخرى إن ارتفع الضغط، وقد يحتاج بعض المرضى إجراءً ثانياً إن أُغلق مسار التصريف بالتليّف، والمتابعة مدى الحياة تبقى ضرورية. وهي لا تعالج المياه البيضاء (الساد) إلا إن أُجريت العمليتان معاً، وقد تُسرّع ظهور الساد لاحقاً.

كل جراحة عين تحمل مخاطر تُشرح قبل الموافقة: انخفاض الضغط أكثر من اللازم، رشح أو التهاب، نزف، تسرّب من الفقاعة، وفي حالات نادرة فقدان بصر إضافي. لذلك لا يُقارَن قرار الجراحة بين مريضين، والترشيح يُحدَّد بالفحص السريري وسجل مجال الرؤية وتصوير العصب البصري ومناقشة صريحة للبديل: الاستمرار على العلاج الدوائي.

## خطوات الإجراء

### 1. قرار الجراحة وتقييم ما قبلها

تُراجع سلسلة قياسات الضغط ومجال الرؤية وتصوير العصب لإثبات التقدّم، ويُفحص سطح العين والملتحمة وزاوية التصريف وحالة العدسة، ويُوثَّق ما استُخدم من قطرات وليزر سابقاً. يُشرح نوع العملية المقترح وأهدافها ومخاطرها والبديل عنها قبل التوقيع على الموافقة.

### 2. التحضير والتخدير

تُراجع أدويتك ومسيّلات الدم مع طبيب الجراحة والطبيب المعالج، وتُعطى تعليمات الصيام إن كانت هناك تهدئة. تُبدأ قطرات مضاد حيوي أو مضاد التهاب بحسب البروتوكول، ويُجرى تخدير موضعي حول العين مع تهدئة خفيفة؛ يبقى المريض مستيقظاً غالباً بلا ألم.

### 3. الإجراء الجراحي

يُفتح مسار تصريف جديد للسائل: قد يكون فتحة محكومة تحت سديلة من الصلبة مع فقاعة تصريف تحت الملتحمة، أو أنبوباً يوصل السائل إلى صفيحة تصريف، أو دعامة دقيقة تُوسّع مجرى التصريف الطبيعي. تُستخدم خيوط دقيقة قابلة للتعديل لضبط سرعة التصريف بعد العملية.

### 4. المرحلة الحرجة الأولى

زيارات متقاربة في الأسابيع الأولى لقياس الضغط وتقييم الفقاعة والالتهاب. قد يفكّ الطبيب خيطاً، أو يدلّك الفقاعة، أو يحقن دواءً مضاداً للتليّف، أو يعدّل الكورتيزون التدريجي. تُستخدم واقية للعين أثناء النوم، وتُمنع فرك العين والانحناء ورفع الأثقال والسباحة في هذه الفترة.

### 5. المتابعة طويلة الأمد

بعد استقرار الضغط تُباعَد الزيارات تدريجياً، ويعاد فحص مجال الرؤية وتصوير العصب لمقارنة الحالة بما قبل الجراحة. تبقى المتابعة مدى الحياة لأن الفقاعة قد تتليّف والضغط قد يرتفع مجدداً، وقد تُعاد القطرات أو يُقترح إجراء ثانٍ عند الحاجة، مع مراقبة ظهور الساد.

## Procedure steps

### 1. The surgical decision and preoperative assessment

The series of pressure readings, visual fields and nerve images is reviewed to establish progression, and the ocular surface, conjunctiva, drainage angle and lens are examined, with previous drops and laser documented. The proposed operation, its aims, risks and alternatives are explained before consent is signed.

### 2. Preparation and anaesthesia

Your medications and blood thinners are reviewed with the surgical and treating teams, and fasting instructions are given if sedation is planned. Antibiotic or anti-inflammatory drops are started per protocol, and local anaesthesia is given around the eye with light sedation; the patient usually stays awake and pain free.

### 3. The surgical procedure

A new outflow route is created: a controlled opening beneath a scleral flap with a drainage bleb under the conjunctiva, a tube conducting fluid to a drainage plate, or a micro-stent widening the natural outflow channel. Fine adjustable sutures are used so drainage speed can be tuned after surgery.

### 4. The critical early phase

Closely spaced visits in the first weeks measure pressure and assess the bleb and inflammation. The surgeon may release a suture, massage the bleb, inject anti-scarring medication, or adjust the tapering steroid. An eye shield is used during sleep, and rubbing the eye, bending, heavy lifting and swimming are avoided in this period.

### 5. Long-term follow-up

Once pressure stabilises, visits are gradually spaced out and visual fields and nerve imaging are repeated to compare with the preoperative baseline. Monitoring continues for life, because the bleb can scar and pressure can rise again; drops may be reintroduced or a second procedure proposed, while cataract formation is watched for.

## التحضير قبل الإجراء

أخبر الفريق بكل أدويتك، وخصوصاً مسيّلات الدم ومضادات الصفيحات وأدوية البروستاتا، ولا توقف أيّاً منها إلا بقرار مشترك بين طبيب العيون وطبيبك المعالج. أبلغ عن أي حساسية دوائية أو لليود أو للاصقات، وعن الربو أو أمراض القلب أو السكري وكيف تضبطه. التزم بتعليمات الصيام إن قيل لك إن هناك تهدئة. رتّب من يوصلك ويعيدك ومن يساعدك في تقطير الدواء أول أسبوع، وخطّط لعدم القيادة ولا العمل الشاق لعدة أسابيع، واحصل على إجازة إن كان عملك بدنياً أو مغبّراً. أحضر جميع قطراتك يوم العملية، ولا تضع مكياجاً على العين، واسأل مسبقاً عن عدد زيارات المتابعة القريبة لأنها ليست اختيارية.

## العناية بعد الإجراء

التزم بجدول القطرات بدقّة، وخصوصاً الكورتيزون ومضاد الحيوي، ولا توقف أياً منها ولا تنقص عددها من تلقاء نفسك؛ التدرّج في إنقاص الكورتيزون يقرّره الطبيب. لا تفرك العين إطلاقاً، والبس الواقية أثناء النوم كما وُصف، ولا تنم على جهة العين المصابة في الأيام الأولى. تجنّب رفع الأثقال والانحناء والحزق والسعال العنيف والسباحة والبخار والغبار لعدة أسابيع، وعالج الإمساك لتجنّب الحزق. توقّع رؤية ضبابية متذبذبة وإحساساً بجسم غريب ودموعاً في البداية. \*\*راجع فوراً عند\*\* ألم شديد أو متزايد، أو هبوط مفاجئ في الرؤية، أو احمرار متزايد مع رشح أو صديد أو التصاق الجفون صباحاً، أو رؤية ظل أو بقع أو ومضات متزايدة، أو خروج سائل مائي غزير من العين، أو حساسية شديدة للضوء مع صداع — فبعض هذه علامات التهاب داخل العين وهو طارئ يهدّد البصر.

## About this service

Glaucoma surgery creates a new drainage route for the fluid inside the eye, or reduces its production, in order to lower pressure when drops and laser are not enough, when optic nerve and visual field damage keep progressing despite them, or when medication is not tolerated or cannot realistically be taken as prescribed. The decision rests on the direction of damage over time, not on a single pressure number.

Several operations exist and the surgeon selects among them according to the glaucoma type, the degree of nerve damage, the state of the eye and previous surgery: trabeculectomy, which forms a drainage bleb under the conjunctiva; implantation of a drainage tube or valve; minimally invasive glaucoma procedures often combined with cataract surgery; and procedures that reduce fluid production in advanced disease. Anti-scarring medication may be used to help keep the new drainage pathway open.

The operation is usually performed under local anaesthesia around the eye with light sedation, typically takes 30 to 60 minutes, and most patients go home the same day. The real work begins afterwards: the first six to eight weeks are the critical phase and require frequent, closely spaced visits to fine-tune pressure, which may include releasing a suture, massaging the bleb, injecting anti-scarring medication or adjusting drops. Missing these appointments is the single commonest way a technically successful operation fails.

The limits, stated honestly: \*\*surgery protects the sight that remains and does not restore what has been lost\*\*, and it does not sharpen vision. Vision may fluctuate and blur for weeks afterwards and the spectacle prescription can change. It is not a definitive cure either: drops may be needed again if pressure rises, some patients require a second procedure if scarring closes the drainage route, and lifelong monitoring remains essential. It does not treat cataract unless both operations are combined, and it can hasten cataract formation later.

Every eye operation carries risks that are explained before consent: pressure falling too low, leakage or infection, bleeding, bleb leak, and in rare cases further loss of vision. For that reason one patient's decision cannot be copied by another. Eligibility is determined by clinical examination, the record of visual fields and nerve imaging, and a frank discussion of the alternative of continuing medical treatment.

## Preparation

Tell the team about all your medicines, especially blood thinners, antiplatelet agents and prostate drugs, and do not stop any of them except by joint decision between your ophthalmologist and your treating physician. Report drug, iodine or adhesive allergies, and any asthma, heart disease or diabetes and how it is controlled. Follow fasting instructions if sedation is planned. Arrange transport in both directions and someone to help instil drops in the first week, plan not to drive or do heavy work for several weeks, and take leave if your job is physical or dusty. Bring all your drops on the day, wear no eye make-up, and ask in advance how many early follow-up visits are needed, because they are not optional.

## Aftercare

Follow the drop schedule exactly, especially the steroid and antibiotic, and never stop or reduce them yourself; the steroid taper is decided by your surgeon. Do not rub the eye at all, wear the shield during sleep as instructed, and avoid sleeping on the operated side in the first days. Avoid heavy lifting, bending, straining, forceful coughing, swimming, steam and dust for several weeks, and treat constipation to avoid straining. Expect fluctuating blurred vision, a foreign-body sensation and watering early on. \*\*Seek care immediately if\*\* you develop severe or increasing pain, a sudden drop in vision, increasing redness with discharge, pus or lids stuck together in the morning, new or increasing shadows, floaters or flashes, a copious watery leak from the eye, or marked light sensitivity with headache, as some of these indicate infection inside the eye, a sight-threatening emergency.
