## Procedure steps

### 1. Clinical assessment and hydration status

The doctor assesses how long the diarrhoea has lasted, its frequency, the presence of blood or fever, the suspected food and any affected household contacts, then judges the degree of dehydration from pulse, blood pressure, dry mouth, urine output and, in children, weight. That judgement decides whether rehydration is oral at home or intravenous.

### 2. Samples and laboratory tests

A stool sample for culture is taken when there is fever, blood, prolonged symptoms, or as part of investigating a food-borne outbreak, and a blood culture is requested when spread of infection or typhoid fever is suspected, together with electrolytes and kidney function if dehydration is present. Samples are collected before any antibiotic starts, so the result is not falsely negative.

### 3. Rehydration and supportive care

The core treatment is replacing fluids and salts: oral rehydration solution taken in small, frequent sips, and intravenous fluids when vomiting is severe or dehydration advanced. Light food is restarted early, and medicines that could add harm — such as bowel-slowing agents when fever or blood is present — are reviewed and avoided.

### 4. The targeted antibiotic decision

An antibiotic is prescribed only for eligible cases based on examination, culture and susceptibility testing, not for every patient. \*\*You are told the medicine, its duration, and why the course must be completed, because stopping as soon as you feel better breeds resistance that strips the drug of its effect for you and for others.\*\* Drug allergy, pregnancy and breastfeeding are reviewed before prescribing.

### 5. Follow-up and preventing spread

A follow-up visit is arranged to assess the response, and culture results are reviewed so the medicine can be adjusted if needed. Measures to prevent household spread are explained: handwashing after the toilet and before food, not preparing food for others until the diarrhoea has stopped, separating towels, and specific arrangements for people working in restaurants, care settings or nurseries before they return to work.

## About this service

Salmonella infection is an illness caused by Salmonella bacteria, usually transmitted through contaminated food or water. It appears in different forms: an acute gastroenteritis with diarrhoea, vomiting, abdominal cramps and fever, or the more serious typhoid fever, which affects the whole body and requires targeted treatment and follow-up. Identifying which form is present is the first step, because the management of the two differs substantially.

In ordinary gastroenteritis, treating dehydration is the real treatment rather than an antibacterial medicine: oral rehydration solution in small, frequent amounts, followed by a gradual return to light food. \*\*Most cases of Salmonella diarrhoea improve with fluids alone, and an antibiotic is not a routine treatment for every episode of diarrhoea.\*\* Using one unnecessarily may actually prolong how long the bacteria stay in the gut and increase the chance of resistance. Antibiotics are reserved for specific situations decided by clinical examination, such as infants, older adults, pregnant women and people with weakened immunity, or when there is evidence that the infection has spread to the bloodstream, or in typhoid fever.

Typhoid fever follows a different route: a climbing fever, headache, abdominal pain and marked exhaustion, requiring an antibiotic chosen according to the culture result and susceptibility testing. \*\*Completing the full course is a necessity, not an option — stopping at the first sign of improvement allows the illness to return and breeds resistant bacteria that are far harder to treat later.\*\* Antibiotics must never be bought without a prescription, taken from an old leftover pack, or borrowed from a medicine prescribed for someone else, because this causes harm that extends to the entire community.

There are limits worth knowing. Treatment does not grant lasting immunity, and reinfection from contaminated food is possible. Bacteria may remain in the stool for weeks after symptoms disappear without this meaning the treatment failed, and people who handle food or work in care settings may need special arrangements before returning to work. Medicines that stop bowel movement can be harmful when diarrhoea comes with fever or blood. Prevention remains the strongest tool: handwashing, cooking chicken and eggs thoroughly, avoiding raw egg in desserts and sauces, and sticking to pasteurised dairy. Anyone travelling to an area where typhoid is common should see a travel clinic early — vaccination and preventive advice are arranged well ahead of departure, not at the last minute.

## Preparation

Before the visit, write down when the diarrhoea began, how many times a day it occurs, whether there is blood, mucus or fever, which food you suspect and when you ate it, and who else among your contacts fell ill. Do not take an antibiotic on your own, as it can invalidate the culture result and delay diagnosis. Do not take a medicine to stop diarrhoea before speaking to the doctor, especially with fever or blood. Start oral rehydration solution now in small, frequent sips and keep track of how often you pass urine. Bring a list of your medicines and chronic conditions, and tell the doctor if you are pregnant or breastfeeding, or if the patient is a young child, an older adult, or someone with weakened immunity. If a stool sample is requested, follow the instructions for collecting it in a clean dedicated container and deliver it promptly.

## Aftercare

Keep taking fluids and salts until your urine is pale again and you are passing water normally, and return to light food gradually. Wash your hands thoroughly after the toilet and before preparing any food, and do not prepare food for others until the diarrhoea has completely stopped. If an antibiotic was prescribed, finish the whole course and do not stop at the first improvement. Go to the emergency department immediately if any of the following appears: \*\*rapid breathing or confusion with fever — possible sepsis\*\* · \*\*a high, persistent fever in a young child or in someone with weakened immunity\*\* · \*\*fever with a stiff neck or light sensitivity\*\* · \*\*a rash that does not fade under pressure\*\* · \*\*signs of severe dehydration: passing no urine, dizziness on standing, crying without tears or lethargy in a child\*\* · \*\*visible blood in the stool, worsening severe abdominal pain, or vomiting that prevents drinking\*\*.
