## Procedure steps

### 1. Clinical assessment, contacts and vaccination history

The doctor asks how long the cough has lasted, the shape of the fits and whether they end in a whoop or vomiting, whether they worsen at night, whether there is a baby or a pregnant woman at home, any contact with a confirmed case, and the vaccination status of you and your children, then examines breathing, ears and chest and assesses exhaustion and dehydration.

### 2. Laboratory swab

A fine swab is taken from the nasopharynx to confirm the bacterium, and it is most accurate during the earlier weeks of the illness. A chest image may be requested if a lung complication is suspected. A negative result taken late does not rule the illness out when the clinical pattern and contact history are clear.

### 3. Early antibiotic and isolation

An antibiotic suitable for the patient's age and condition is prescribed for a defined course. Its effect on symptoms is greatest when started early, while its certain benefit is halting transmission. You are asked to stay home, away from nursery, school and work, during the first days of treatment, with rooms ventilated and the mouth covered when coughing.

### 4. Protecting contacts and updating vaccination

The doctor identifies which household members need preventive medicine, prioritising infants, pregnant women and those who work with children. The vaccination schedule is then reviewed: completing a child's overdue immunisations, and a booster for the pregnant woman and those surrounding the newborn, since cocooning is the most effective way to protect a baby whose own immunity is incomplete.

### 5. Follow-up and managing the lingering cough

Follow-up is arranged to assess the fits, sleep, feeding and, in children, weight. It is explained that the cough may continue for weeks after the medicine ends, that this is expected, and that it does not mean a new antibiotic is needed. The fits are managed with adequate fluids, small frequent meals, and avoiding smoke, dust and cold dry air.

## About this service

Whooping cough (pertussis) is a highly contagious bacterial respiratory infection that causes bursts of coughing so tightly packed that breathing between them is difficult, often ending in a sharp intake of breath that produces the characteristic "whoop", or in vomiting after the fit. The illness usually starts like an ordinary cold for a week or so before the coughing fits escalate, which is why it is so often diagnosed late. Diagnosis is clinical, based on the distinctive cough pattern and contact history, and it is supported by a swab taken from the nasopharynx.

Treatment has two aims: breaking the chain of transmission to the people around you, and reducing the severity of the illness if the medicine is started early. A suitable antibiotic is prescribed for a defined course, alongside home isolation during its first days. \*\*Completing the prescribed course in full is essential: stopping at the first improvement keeps you contagious and breeds resistant bacteria\*\*, making the medicine less useful for you and for others in the future. Household contacts — especially infants, pregnant women, and anyone in close contact with a baby — may be given preventive medicine on a doctor's decision.

An important limit deserves plain explanation, because many people do not expect it: \*\*the antibiotic does not stop the cough quickly.\*\* It clears the bacteria, but the cough itself can persist for weeks afterwards because the airways have already been irritated. That is an expected course, not a sign of treatment failure, and it does not call for another antibiotic. Cough syrups and suppressants are not effective in this illness, and some are unsuitable for young children in the first place. Remember too that \*\*antibiotics do not treat viruses such as the common cold and influenza\*\*, which is why they are not prescribed for every cough, but only after an assessment that specifically points to pertussis.

The highest-risk group is the infant in the first year of life, who may not cough in the familiar way at all but instead stop breathing or turn blue around the lips, and may need monitoring in hospital. Real prevention is vaccination, which is part of the childhood immunisation schedule in Jordan; however, protection wanes over the years, so adolescents and adults do catch the illness and pass it on to a baby who has not yet completed the schedule. This is why updating vaccination in pregnancy and for those surrounding a newborn is recommended, an approach known as cocooning. \*\*Neither previous infection nor vaccination grants lifelong immunity\*\*, and eligibility for any treatment or preventive measure is decided by clinical examination and contact history.

## Preparation

Before the visit, record when the cough began and how many fits occur by day and by night, and whether a fit ends in a whoop, vomiting or a brief faint. If possible, film a short clip of a coughing fit, as this helps the doctor a great deal in telling pertussis apart from other causes. Bring your child's immunisation booklet or your own vaccination card, and mention whether there is a baby, a pregnant woman or someone with weakened immunity at home. Write down the medicines and syrups already tried and for how long. Tell the doctor if you have been in contact with a confirmed case and when, and whether the patient works or studies in a nursery, school or care centre. Wear a mask at the clinic and tell reception that the cough is severe and paroxysmal, so measures can be arranged to reduce transmission to others.

## Aftercare

Finish the antibiotic course in full and do not stop it because the cough has not yet settled — the medicine breaks transmission, it does not silence the cough immediately. Stay at home during the first days of treatment, ventilate rooms, cover your mouth with a tissue during a fit and wash your hands afterwards. Offer plenty of fluids and small frequent meals, since vomiting after a fit reduces intake, raise a child's head slightly for sleep, and allow no smoking anywhere nearby. Go to the emergency department immediately if any of the following occurs: \*\*pauses in breathing or blueness around the lips in an infant\*\* · \*\*a coughing fit that prevents breathing or drinking\*\* · \*\*rapid breathing, chest muscles pulling in, or confusion\*\* · \*\*a seizure or extreme lethargy\*\* · \*\*a high, persistent fever in a young child or in a person with weakened immunity\*\* · \*\*signs of dehydration: no urine or crying without tears\*\* · \*\*weight loss in a child or refusal to feed\*\*.
