تنبيه سلامة أساسي: \*\*لا تُعطَ أدوية السعال والبرد المركّبة الجاهزة للأطفال الصغار\*\*، فلا فائدة مثبتة لها وقد تسبّب نعاساً شديداً وتسرّع قلب واختلاجاً وحتى تسمّماً عند الخطأ في الكمية، ومركّبات الكودين ممنوعة عند الأطفال. \*\*العسل قد يخفّف السعال الليلي لمن أتمّ عاماً واحداً فقط، ويُمنع تماماً تحت عمر السنة\*\* لخطر التسمّم الوشيقي. ولا تستخدم مراهم الأنف والصدر المحتوية على كافور أو منثول للصغار، ولا تُدخّن في البيت أو السيارة فدخان التبغ من أقوى مفاقمات السعال المزمن عند الأطفال.

ومن حدود العلاج أن هدفه ليس إسكات السعال، لأن كتم السعال المنتج يمنع طرح المخاط ويؤخّر الشفاء. جهاز البخار والمنشقات لا تشفي عدوى فيروسية، والسعال بعد الزكام قد يستمر أسابيع بعد شفاء الطفل تماماً. وأي دواء يُوصف يُحدَّد بالفحص السريري وعمر الطفل ووزنه، لا بتجربة سابقة ولا بنصيحة من شخص آخر.

## خطوات الإجراء

### 1. التاريخ المرضي ونمط السعال

يُسأل عن بداية السعال ومدّته وطبيعته: جافّ أم مع بلغم، ليلي أم مع الجهد، هل بدأ فجأة أثناء الأكل أو اللعب، وهل هناك حرارة أو صفير أو تقيؤ بعد نوبة السعال أو نقص وزن. يُسأل عن التطعيمات وحساسية سابقة وربو في العائلة والتدخين في المنزل وأي دواء أُعطي للطفل.

### 2. الفحص السريري وقياس الأكسجة

يُقاس معدّل التنفّس والحرارة والوزن وتشبّع الأكسجين، ويُستمع للصدر بحثاً عن خرخرة أو صفير أو ضعف دخول هواء في جهة واحدة. يُفحص الأنف والبلعوم والأذن واللوزتان، وتُلاحظ علامات الجهد التنفّسي: سحب بين الأضلاع، توسّع فتحتي الأنف، أو تسارع تنفّس أثناء الراحة.

### 3. الفحوصات عند وجود داعٍ

لا يحتاج معظم الأطفال أي فحص. تُطلب صورة صدر عند الاشتباه بالتهاب رئوي أو جسم غريب، ومسحة أو تحاليل عند الاشتباه بالشاهوق أو عدوى محدّدة، وفحص وظائف التنفّس للأطفال الأكبر عند السعال المزمن، وتقييم للأنف والجيوب أو للارتجاع بحسب النمط، وتقييم موسّع عند السعال المستمر مع تأثّر النمو.

### 4. العلاج الموجّه للسبب

تُعالَج العدوى الفيروسية بالدعم فقط: سوائل، راحة، خفض حرارة، وغسول أنفي ملحي. يُوصف المضاد الحيوي فقط عند دليل عدوى بكتيرية كالالتهاب الرئوي. وإن كان السبب ضيق شعب أو تحسّساً فيُحوَّل الطفل لتقييم وخطة مستقلة. والجسم الغريب المُشتبه به يحتاج تدخّلاً عاجلاً بالتنظير.

### 5. التدابير المنزلية وإعادة التقييم

يُشرَح للأهل ما ينفع فعلاً: سوائل كافية، رفع الرأس قليلاً عند النوم، هواء غير جافّ، تجنّب الدخان والروائح النفّاذة، ومحلول ملحي للأنف. ويُشرَح ما يجب تجنّبه من أدوية السعال المركّبة والعسل تحت السنة. ويُحدَّد موعد إعادة التقييم إن لم يتحسّن الطفل خلال المدّة المتوقّعة أو ظهرت علامة خطر.

## Procedure steps

### 1. History and cough pattern

The doctor asks about the onset, duration and character of the cough: dry or productive, night-time or exertional, whether it began suddenly while eating or playing, and whether there is fever, wheeze, vomiting after coughing bouts or weight loss. Vaccination history, previous allergy, family asthma, smoking at home and any medicine already given are all reviewed.

### 2. Examination and oxygen saturation

Respiratory rate, temperature, weight and oxygen saturation are measured, and the chest is auscultated for crackles, wheeze or reduced air entry on one side. The nose, throat, ears and tonsils are examined, and signs of increased work of breathing are noted: recession between the ribs, nasal flaring, or a fast respiratory rate at rest.

### 3. Investigations when indicated

Most children need no tests at all. A chest radiograph is requested when pneumonia or a foreign body is suspected, swabs or blood tests when whooping cough or a specific infection is likely, lung function testing in older children with chronic cough, assessment of the nose and sinuses or of reflux according to the pattern, and broader evaluation when a persistent cough is accompanied by faltering growth.

### 4. Cause-directed treatment

A viral infection is managed supportively with fluids, rest, fever control and saline nasal irrigation. Antibiotics are prescribed only when there is evidence of bacterial infection such as pneumonia. If airway narrowing or an allergic cause is identified, the child is referred for its own assessment and separate plan. A suspected inhaled foreign body requires urgent endoscopic intervention.

### 5. Home measures and reassessment

Parents are told what genuinely helps: adequate fluids, slightly raising the head during sleep, avoiding dry air, keeping away from smoke and strong fumes, and saline nasal drops. They are also told what to avoid, including combined cough preparations and honey under one year of age. A reassessment date is set if the child does not improve within the expected period or if a warning sign appears.

## التحضير قبل الإجراء

سجّل قبل الزيارة متى بدأ السعال بالضبط، وهل هو جافّ أم مع بلغم ولونه، وفي أي وقت يشتدّ، وهل بدأ فجأة أثناء الأكل أو اللعب فهذا يغيّر مسار التشخيص كلّياً. اكتب أعلى قراءة حرارة قِستها وكيف قِستها، وكم مرة تقيّأ الطفل، وكم مرة يبلّل الحفاض أو يتبوّل. أحضِر دفتر التطعيمات وكل الأدوية والشرابات التي أعطيتها للطفل بعبواتها، بما فيها ما وصفه غيرك أو ما تبقّى من مرض سابق، وأخبر الطبيب بها كلّها بصراحة. أبلِغ إن كان الطفل مخالطاً لمريض سعال طويل أو حالة سلّ أو شاهوق، وإن كان أحد أفراد البيت يدخّن، وإن للطفل ربو أو حساسية أو أمراض قلب أو نقص مناعة. لا تُعطِ طفلك دواء سعال مركّباً قبل الزيارة، وسجّل بمقطع صوتي قصير نوبة السعال إن أمكن فهو يساعد الطبيب كثيراً.

## العناية بعد الإجراء

أعطِ سوائل كافية وامنح الطفل راحة، واستخدم محلولاً ملحياً للأنف قبل النوم والرضاعة، وارفع رأس السرير قليلاً بدل رفع الوسادة تحت رأس الرضيع. أزِل الدخان والبخور والروائح النفّاذة من البيت والسيارة تماماً. أكمِل مدّة المضاد الحيوي إن وُصف ولا توقفه عند التحسّن، ولا تعطِ الطفل بقايا مضاد حيوي أبداً. لا تستخدم أدوية السعال والبرد المركّبة للصغار، ولا تُعطِ العسل قبل عمر سنة. توقّع أن يستمر السعال الجاف بعد الزكام فترة رغم شفاء الطفل، وتابع نشاطه وشهيته ونومه فهي أفضل مؤشّر على التحسّن.

## About this service

Treating a cough in children starts with identifying its cause, because coughing is not itself a disease but a protective reflex that clears mucus and inhaled particles from the airways. The most common cause in children is a viral upper respiratory infection that settles on its own, but a cough may also signal pneumonia, airway narrowing, post-nasal drip, gastro-oesophageal reflux, whooping cough, or an inhaled foreign body, and each of these needs entirely different management.

The doctor therefore focuses on the pattern and duration of the cough: whether it is worse at night or during the day, dry or productive, whether it began suddenly during play or gradually with a runny nose, whether it comes with wheeze, a harsh noise on breathing in, fever, breathlessness or weight loss, and whether it has lasted longer than four weeks. Examination includes listening to the chest, measuring oxygen saturation, and inspecting the nose, throat and ears. A chest radiograph, blood tests or further investigations are requested only when there is a clear reason.

Treatment targets the cause rather than the sound. A viral infection is managed with fluids, rest, fever control and saline nasal irrigation. Bacterial pneumonia requires a prescribed antibiotic. A cough linked to airway narrowing or allergic disease needs its own assessment and plan with the doctor. An inhaled foreign body needs urgent intervention. Antibiotics do not help a viral cough and do not shorten it, and giving them unnecessarily causes bacterial resistance, diarrhoea and allergic reactions.

A core safety warning: \*\*over-the-counter combined cough and cold preparations should not be given to young children\*\*. They have no proven benefit and can cause heavy drowsiness, rapid heartbeat, agitation and even poisoning if the amount is mistaken, and codeine-containing preparations are contraindicated in children. \*\*Honey may ease a night-time cough in a child over one year of age only, and is absolutely forbidden under one year\*\* because of the risk of infant botulism. Do not use camphor or menthol rubs on young children, and do not smoke in the house or car, since tobacco smoke is among the strongest aggravators of chronic cough in children.

As for the limits of treatment, silencing the cough is not the goal: suppressing a productive cough prevents mucus clearance and delays recovery. Steam devices and inhalers do not cure a viral infection, and a post-viral cough may persist for weeks after the child is otherwise completely well. Any medicine prescribed is chosen on the basis of clinical examination and the child's age and weight, not on a previous course or someone else's advice.

## Preparation

Before the visit, note exactly when the cough started, whether it is dry or productive and the colour of any sputum, at what times it worsens, and whether it began suddenly while eating or playing, since that completely changes the diagnostic path. Write down the highest temperature you measured and how you measured it, how many times the child vomited, and how often they pass urine or wet a nappy. Bring the vaccination record and every medicine or syrup you have given, in its original box, including anything prescribed elsewhere or left over from a previous illness, and disclose all of it openly. Report whether the child has been in contact with someone with a prolonged cough, tuberculosis or whooping cough, whether anyone at home smokes, and whether the child has asthma, allergy, heart disease or immune deficiency. Do not give a combined cough preparation before the visit, and if possible record a short audio clip of a coughing bout, as this helps the doctor considerably.

## Aftercare

Give plenty of fluids and let the child rest, use saline nasal drops before sleep and feeds, and raise the head of the bed slightly rather than placing a pillow under an infant's head. Remove smoke, incense and strong fumes from the house and car completely. Complete the full antibiotic course if one was prescribed and do not stop it on improvement, and never give a child leftover antibiotics. Do not use combined cough and cold preparations in young children, and do not give honey before one year of age. Expect a dry post-viral cough to linger for some time even after the child is well, and judge recovery by activity, appetite and sleep, which are the best indicators.

Go to the emergency department immediately if there is: \*\*difficulty breathing, recession between the ribs, nasal flaring or blue lips\*\*; \*\*a cough that started suddenly with choking or a harsh noise on breathing in, suggesting an inhaled object\*\*; \*\*marked lethargy, refusal to drink, reduced urine output or a dry mouth\*\*; \*\*fever in an infant under three months\*\*; \*\*coughing up blood or rust-coloured sputum\*\*; \*\*repeated coughing bouts ending in a gasp or vomiting\*\*; \*\*severe chest pain or fast breathing at rest\*\*. Book a clinic review if the cough lasts more than four weeks, or comes with weight loss, night sweats or recurrent fever.
