## Procedure steps

### 1. Identifying the headache type from the history

The doctor asks how long attacks last, where the pain is, its character and severity, and what accompanies it - nausea, light or sound sensitivity, tearing or nasal blockage - along with triggers, sleep pattern, caffeine intake and menstrual timing, plus the number of headache days and painkiller days per month. This history separates the primary headache types and exposes secondary causes.

### 2. Examination and screening for red flags

Blood pressure is measured and a neurological examination is performed, together with assessment of the neck, sinuses, jaw and teeth and, where indicated, the back of the eye, while medicines that can provoke headache are reviewed. Red flags are excluded: sudden explosive onset, fever with neck stiffness, head injury, weakness or seizure, a new headache after fifty, or a clear change in a long-standing pattern.

### 3. Tests and imaging only when indicated

No test diagnoses a primary headache, though blood tests may be requested for contributing factors such as anaemia or thyroid disorders. Imaging is ordered when a red flag is present, when the pattern changes, or when the neurological examination is abnormal - not for every patient. Scanning without an indication increases anxiety and turns up incidental findings unrelated to the headache.

### 4. Acute plan and a painkiller ceiling

A medicine suited to the headache type is prescribed and taken early in the attack, since delay reduces its benefit, and \*\*a clear ceiling on painkiller days per month\*\* is set to avoid medication-overuse headache. A back-up plan is agreed for when the first dose fails, along with what to record in the diary and when to return.

### 5. Prevention, trigger work and follow-up

For frequent or disabling headaches a regular preventive treatment is started and needs weeks before its effect can be judged, supported by regular sleep and meals, hydration, gradual caffeine reduction, physiotherapy for neck tension, correction of screen posture, an eye test and stress management. The diary is reviewed at follow-up to count headache days and painkiller days.

## About this service

Treating a headache starts with diagnosing which headache it is, because the single word covers conditions that differ completely in cause and treatment. Primary headaches are those in which the headache itself is the disease, including tension-type headache, migraine and cluster headache. Secondary headaches are a sign of something else: a sinus, dental, neck or eye problem, blood pressure, a medicine, or an intracranial process that needs urgent attention. No single painkiller suits all of these, which is why so many self-directed attempts fail.

Diagnosis depends on the history far more than on any test: how long attacks last, where the pain sits and how it feels, whether a visual warning precedes it, whether it comes with light and sound sensitivity or nausea, what worsens and relieves it, how many headache days you have each month, how many days you take painkillers, and your sleep, stress, caffeine intake and menstrual pattern. \*\*No blood test diagnoses a primary headache\*\*, and imaging is not requested for everyone: it is ordered when there are worrying features or when a long-standing headache pattern changes, and a normal scan does not mean the headache is imaginary.

Treatment has two arms. Acute treatment uses a medicine appropriate to the headache type, taken early and limited to a defined number of days. Preventive treatment is taken regularly by people whose headaches are frequent or disabling. Both are combined with trigger management: regular sleep, meals that are not skipped, adequate hydration, gradual reduction of caffeine, treatment of neck tension and poor desk posture, an eye test, and stress control. Keeping a headache diary for a few weeks is one of the most useful tools available, because it reveals patterns the patient has not noticed.

The most important misconception to correct is this: \*\*overusing painkillers is itself a common cause of chronic daily headache\*\*. Someone who regularly takes headache painkillers on more than roughly two to three days a week can enter a cycle in which the medication drives the headache, and the way out is a planned, supervised reduction combined with a preventive strategy, not a higher dose. The limits are clear too: prevention reduces the number and severity of attacks rather than abolishing headache altogether, it does not replace trigger management, and it may take weeks before its effect shows. Go to the emergency department immediately for a sudden, explosively severe headache, headache with fever and a stiff neck, headache with weakness, slurred speech, seizure or altered awareness, headache after a head injury, headache that worsens with coughing or lying down or wakes you with vomiting, a new headache after the age of fifty, or headache with a painful red eye and changed vision. Suitability for any medicine is decided by clinical examination, not by reading.

## Preparation

The most valuable preparation is a headache diary: for two to four weeks record the date of each attack, its duration and severity out of ten, where the pain sat, what accompanied it, what medicine you took and how many tablets and whether it helped, your hours of sleep, your coffee and stimulant intake, and, for women, the timing of your period. Bring a list of all your medicines, including contraceptive pills, supplements and over-the-counter painkillers, and count your painkiller days per month honestly, because that figure is pivotal. Tell the doctor about any recent head injury, high blood pressure, vision problems, neck tension or teeth grinding, and any family history of headache, and bring previous reports and scans if you have them.

## Aftercare

Take acute medication early rather than after hours of suffering, and stay within the monthly limit you were given: \*\*taking painkillers on more than two to three days a week can turn your headaches into a chronic daily headache\*\*. If you are already in that cycle, the way out is a gradual plan with your doctor, not abrupt stopping and not a higher dose. Give preventive treatment enough time before judging it and never stop it on your own. Keep your sleep, meals and water intake regular, reduce caffeine gradually to avoid withdrawal headache, continue neck exercises and screen-posture correction, and keep the headache diary up to date so results can be measured.

Go to the emergency department immediately for a \*\*sudden, extremely severe headache that peaks within seconds\*\*, \*\*headache with fever and neck stiffness or a rash\*\*, \*\*weakness or numbness on one side, slurred speech, seizure or altered awareness\*\*, headache after a head injury, headache that worsens with coughing or lying flat or wakes you with repeated vomiting, headache with a painful red eye and blurred vision, or a new or different headache in anyone over fifty or with cancer, reduced immunity or pregnancy.
