العلاج يقوم على \*\*مسارين منفصلين لا يُخلط بينهما\*\*:
\*\*علاج النوبة\*\* — يُؤخذ عند بدايتها مبكراً وبجرعة كافية، لأن التأخير يُضعف الاستجابة. ويشمل مسكّنات محدّدة وأدوية خاصة بالشقيقة يصفها الطبيب مع دواء للغثيان.
\*\*علاج وقائي\*\* — يُؤخذ يومياً بانتظام لتقليل عدد النوبات وشدّتها، ويُطرح عندما تكون النوبات متكرّرة أو معطّلة، ويحتاج \*\*أسابيع قبل أن يُقاس أثره\*\* — وهذا ما يجعل كثيرين يتركونه ظلماً بعد أيام.

وهنا أخطر فخّ في الشقيقة، وهو شائع جداً: \*\*صداع فرط استخدام الدواء\*\*. الإكثار من مسكّنات النوبة أكثر من عدد أيام محدود شهرياً يُحوّل الشقيقة العرضية إلى \*\*صداع يومي مزمن ينتج عن الدواء نفسه\*\*، فيزيد المريض الجرعة فيسوء أكثر في حلقة مغلقة. كسر هذه الحلقة يحتاج خطة طبية لسحب المسكّن.

والخطة الكاملة ليست دوائية فقط: \*\*مفكّرة صداع\*\* لتحديد المُحرّضات (قلّة نوم أو نوم زائد، تفويت وجبات، جفاف، توتّر، كافيين ونقصه المفاجئ، دورة شهرية، روائح، إضاءة)، وانتظام النوم والوجبات والسوائل والرياضة المعتدلة.

وتبقى \*\*علامات إنذار\*\* لا تُدار كشقيقة: صداع مفاجئ شديد كالانفجار، أو صداع مع حرارة وتصلّب رقبة، أو مع ضعف أو تشوّش أو تشنّج، أو صداع جديد بعد سنّ الخمسين، أو يسوء بالسعال والاستلقاء، أو يوقظ من النوم دائماً — هذه تستوجب تقييماً عاجلاً.

## خطوات الإجراء

### 1. التشخيص واستبعاد علامات الإنذار

يُشخَّص الصداع النصفي بنمط النوبة وأعراضها المصاحبة، وتُستقصى علامات الإنذار التي تستوجب تصويراً — فالتصوير ليس روتيناً لكل صداع.

### 2. مفكّرة الصداع وتحديد المُحرّضات

تُسجَّل النوبات ومدّتها وشدّتها والدواء المتناول والمُحرّض المحتمل — وهذا السجل هو ما يقيس فاعلية العلاج ويكشف فرط استخدام الدواء.

### 3. ضبط علاج النوبة

يُوصف دواء النوبة ليُؤخذ \*\*مبكراً\*\* بجرعة كافية مع دواء للغثيان، ويُحدَّد عدد الأيام الشهرية المسموح بها لتفادي صداع فرط الاستخدام.

### 4. إضافة علاج وقائي عند التكرار

يُبدأ دواء وقائي يومي عند تكرار النوبات أو تعطيلها للحياة، بجرعة متدرّجة، ويُقيَّم أثره بعد أسابيع لا أيام بمقارنة مفكّرة الصداع.

### 5. معالجة فرط استخدام الدواء إن وُجد

إن تبيّن فرط استخدام مسكّنات النوبة، تُوضع خطة سحب تدريجي بإشراف طبي مع دعم وقائي — لأن استمرار الاستخدام يُبقي الصداع مزمناً.

## Procedure steps

### 1. Diagnosis and excluding red flags

Migraine is diagnosed from the attack pattern and accompanying symptoms, with red flags screened for that would warrant imaging — imaging is not routine for every headache.

### 2. Headache diary and trigger identification

Attacks, their duration and severity, medication taken and possible triggers are recorded — this diary measures treatment effectiveness and reveals medication overuse.

### 3. Optimising acute treatment

Acute medication is prescribed to be taken \*\*early\*\* at an adequate dose with an antiemetic, and a monthly limit on treatment days is set to avoid medication-overuse headache.

### 4. Adding prevention when attacks recur

A daily preventive drug is started when attacks are frequent or disabling, titrated gradually, and assessed after weeks rather than days by comparing the headache diary.

### 5. Treating medication overuse if present

If overuse of acute medication is identified, a supervised tapering plan is set with preventive support — because continuing use keeps the headache chronic.

## التحضير قبل الإجراء

\*\*احمل مفكّرة صداع لشهر قبل الزيارة\*\*: تاريخ كل نوبة ومدّتها وشدّتها، وما تناولته من دواء \*\*وعدد أيام تناوله في الشهر\*\* (هذا الرقم تحديداً هو ما يكشف فرط الاستخدام)، والمُحرّض المحتمل، وعلاقتها بالدورة الشهرية. أحضر \*\*كل\*\* ما تتناوله للصداع بما فيه ما تشتريه بلا وصفة والمسكّنات المركّبة والكافيين — كثير من المرضى لا يذكرها فيُغفل السبب. اذكر أي ضغط مرتفع أو حمل أو تخطيط له أو أدوية منع حمل، ووجود أورة (تُغيّر بعض الخيارات). اذكر تاريخ الشقيقة في العائلة. وإن كان صداعك جديداً أو تغيّر نمطه فقل ذلك صراحة — فهو ما يوجّه للاستقصاء.

## العناية بعد الإجراء

خُذ دواء النوبة \*\*في أول بداية الأعراض\*\* لا بعد أن يستقرّ الألم — التأخير يُضعف الاستجابة كثيراً. \*\*والأهمّ: لا تتجاوز عدد أيام تناوله الشهرية التي حدّدها لك الطبيب\*\*؛ إن احتجت أكثر فهذه إشارة لمراجعة الخطة لا لزيادة الجرعة، لأن الإكثار يُحوّل الصداع إلى يومي مزمن. أكمل الدواء الوقائي \*\*أسابيع\*\* قبل الحكم عليه ولا توقفه لأنك «لم تشعر بفرق» في أيام. حافظ على نوم منتظم بلا زيادة ولا نقص، ولا تفوّت الوجبات، واشرب سوائل كافية، وثبّت الكافيين ولا تقطعه فجأة، ومارس رياضة معتدلة منتظمة. استعمل غرفة مظلمة هادئة أثناء النوبة. \*\*راجع عاجلاً عند صداع مفاجئ شديد جداً، أو مع حرارة وتصلّب رقبة، أو مع ضعف أو تشوّش أو صعوبة كلام أو تشنّج، أو صداع بدأ بعد سنّ الخمسين، أو يسوء بالسعال والانحناء، أو تغيّر نمطه تماماً.\*\*

## About this service

Migraine is a chronic neurological disease, not merely a “bad headache”. A typical attack is throbbing pain, often one-sided, moderate to severe, worsened by movement, accompanied by nausea or vomiting and marked sensitivity to light and sound. It may be preceded by an \*\*aura\*\* (visual disturbance such as zigzag lines or spots, or spreading numbness), and attacks last hours to three days.

Treatment rests on \*\*two separate tracks that must not be confused\*\*:
\*\*Acute treatment\*\* — taken early at onset and at an adequate dose, since delay weakens the response. It includes specific analgesics and migraine-specific drugs prescribed by a physician, with an antiemetic.
\*\*Preventive treatment\*\* — taken daily to reduce attack frequency and severity, offered when attacks are frequent or disabling, and requiring \*\*weeks before its effect can be judged\*\* — which is why many abandon it unfairly after a few days.

Here lies migraine’s most dangerous and very common trap: \*\*medication-overuse headache\*\*. Using acute painkillers on more than a limited number of days per month converts episodic migraine into \*\*daily chronic headache caused by the medication itself\*\*, so the patient takes more and worsens further in a closed loop. Breaking that loop requires a medically supervised withdrawal plan.

The full plan is not only pharmacological: a \*\*headache diary\*\* to identify triggers (too little or too much sleep, skipped meals, dehydration, stress, caffeine and its sudden withdrawal, menstruation, odours, lighting), plus regular sleep, meals, hydration and moderate exercise.

And there remain \*\*red flags\*\* not managed as migraine: sudden explosive severe headache; headache with fever and neck stiffness; headache with weakness, confusion or seizure; new headache after age fifty; headache worsened by coughing or lying down; or one that consistently wakes you from sleep — these require urgent assessment.

## Preparation

\*\*Bring a headache diary covering a month before the visit\*\*: the date, duration and severity of each attack, what medication you took \*\*and on how many days that month\*\* (that figure specifically reveals overuse), the possible trigger, and any relation to menstruation. Bring \*\*everything\*\* you take for headache including over-the-counter products, combination analgesics and caffeine — many patients omit these so the cause is missed. Report high blood pressure, pregnancy or plans for it, and contraceptive use, and whether you get aura (it changes some options). Mention family history of migraine. And if your headache is new or its pattern has changed, say so explicitly — that is what directs investigation.

## Aftercare

Take acute medication \*\*at the very onset of symptoms\*\* rather than after the pain establishes — delay markedly weakens the response. \*\*Most importantly: do not exceed the monthly number of treatment days your physician set\*\*; needing more is a signal to review the plan, not to increase the dose, because overuse converts headache into daily chronic pain. Continue the preventive drug for \*\*weeks\*\* before judging it and do not stop because you “felt no difference” within days. Keep regular sleep — neither too little nor too much — do not skip meals, drink enough fluids, keep caffeine steady rather than stopping abruptly, and take regular moderate exercise. Use a dark quiet room during attacks. \*\*Seek prompt review for a sudden extremely severe headache, headache with fever and neck stiffness, with weakness, confusion, speech difficulty or seizure, a headache starting after age fifty, one worsened by coughing or bending, or a complete change in pattern.\*\*
