## Procedure steps

### 1. Locating the infection and its likely type

The doctor gathers the history of symptoms, their duration and progression, travel, contacts, chronic conditions, previous surgery, implanted devices and drug allergies, then examines the suspected site and general clinical signs. From this it becomes clear whether the cause is likely viral, bacterial, or not an infection at all — and that determines everything that follows.

### 2. Samples and tests before starting medicine

Samples appropriate to the site are collected — blood, urine or sputum culture, a wound swab or abscess fluid — before the first dose, so the result is not falsely negative, together with blood tests, inflammatory markers and imaging where needed. Susceptibility testing is what allows the most precise medicine to be chosen instead of guessing.

### 3. A treatment plan matched to the organism

A viral infection is managed with supportive care: fluids, rest, fever-reducing medicine, nasal rinsing and pain relief, with antiviral medicines only for specific groups and only early in the illness. A bacterial infection is treated with a targeted antibiotic for the shortest effective course. \*\*An antibiotic is never prescribed “just in case” for a viral illness.\*\*

### 4. Source control when required

If there is an abscess, a collection of pus, dead tissue, a catheter, an implanted device or an infected tooth, the decisive step is a procedure to drain, clean or remove the source. \*\*Without it the medicine fails and the infection returns no matter how many antibiotic courses are given\*\* — and this is one of the most commonly overlooked points in practice.

### 5. Follow-up, response and reducing recurrence

Response is assessed clinically at a set appointment, culture results are reviewed so the medicine can be narrowed, changed or stopped, and any adverse reaction is documented. Factors that predispose to recurrence are then addressed: diabetes control, dental health, skin and wound care, recommended vaccinations, and hand hygiene together with food and water safety.

## About this service

Infection treatment is a clinical pathway that begins by identifying the site of infection and the type of organism responsible — bacterial, viral, fungal or parasitic — and only then choosing the treatment that fits that specific type. The first step is not writing a prescription; it is reaching a diagnosis, because a medicine that works powerfully against one class of organisms has no benefit at all against another, and may cause harm.

\*\*Antibiotics do not treat viruses.\*\* The common cold, influenza, most acute sore throats and bronchitis, and a runny nose with yellow or green phlegm are usually viral, and the colour of phlegm is not evidence that you need an antibiotic. In these illnesses an antibiotic does not shorten the illness or prevent its complications, yet it exposes you to real harm: drug allergy, diarrhoea caused by disruption of gut bacteria that can become severe, effects on the kidneys or liver, and interactions with the long-term medicines you already take.

When the cause genuinely is bacterial, three rules must not be broken. \*\*First: stopping the antibiotic as soon as you feel better, before the prescribed course is finished, leaves the strongest bacteria alive to grow resistant, so the illness returns more severely and becomes harder to treat.\*\* \*\*Second: buying antibiotics without a prescription, using leftovers from an old pack, or passing on medicine prescribed for someone else causes harm far beyond you — it strips these medicines of their effectiveness for everybody, until a routine operation or an ordinary chest infection becomes a genuine danger for another patient.\*\* Third: samples and cultures are taken before the medicine starts, so the organism and its susceptibility are known, and treatment can then be narrowed to the most precise and shortest effective course.

Drug treatment also has explicit limits. \*\*Some infections do not resolve with medicine alone\*\*: they need an abscess drained, a wound cleaned, a catheter or implanted device removed, or an infected tooth treated — and without that step even the strongest medicine fails. Treatment does not prevent a future infection either, and there is no such thing as a "strong antibiotic" that removes the need for a diagnosis. Response is measured by clinical improvement rather than laboratory numbers alone, and those numbers should not be interpreted by patients on their own. Whether a medicine is needed at all, and which one and for how long, is determined by clinical examination with your doctor — not by how severe the symptoms feel, and not by what worked in a previous illness.

## Preparation

Before the visit, write down when your symptoms began and how they progressed, any temperature readings you took, and the site of pain, redness or swelling. Bring a complete list of your medicines, supplements and chronic conditions, and state any drug allergy and exactly what happened during it. Most importantly, tell the doctor openly if you have taken any antibiotic in recent weeks, including its name, duration and where it came from, even if it was without a prescription — this changes both the interpretation of cultures and the choice of medicine. Do not start an antibiotic before the visit or use leftovers from an old pack. Bring reports of any operation or implanted device and your previous test results. If you have a wound, do not apply household substances or powders to it before the doctor sees it.

## Aftercare

If an antibiotic was prescribed, take it at the scheduled times and complete the full course even if you feel entirely better after two days; do not keep leftovers for later and do not give them to anyone else. If your illness is viral, understand that supportive care is the treatment and that recovery takes time. Tell your doctor about a rash, itching or facial swelling, or severe or repeated watery diarrhoea during or after the medicine. Keep any wound and dressing clean exactly as explained to you. Go to the emergency department immediately if any of the following appears: \*\*rapid breathing or confusion with fever — possible sepsis\*\* · \*\*fever with a stiff neck or light sensitivity — possible meningitis\*\* · \*\*a rash that does not fade when pressed\*\* · \*\*a high, persistent fever in a young child or a person with weakened immunity\*\* · \*\*redness spreading quickly, or pain far more severe than the wound looks\*\* · \*\*reduced urine output or dizziness on standing\*\*.
