## Procedure steps

### 1. Urgent assessment and defining the type

The patient is assessed as an urgent case: temperature, pulse, blood pressure, blood sugar and markers of bloodstream infection, together with examination of the borders of the dead tissue and a search for spread through the tissues or gas beneath them. Samples and blood tests are taken and the area is imaged to establish whether the gangrene is dry or wet, because that difference changes the whole order of treatment.

### 2. Controlling infection and stabilising the patient

Fluids are given, blood sugar is controlled and metabolic disturbances are corrected, while treatment for infection is started on the doctor's decision and later guided by culture results. Any collection of pus is drained immediately, and wet or gas gangrene is taken to the operating theatre the same day, because waiting widens the loss.

### 3. Perfusion study and revascularisation

Limb pulses and pressures are measured and the arteries are imaged; if there is a treatable blockage, flow is restored by balloon and stent or by surgical bypass. Revascularisation defines the border of salvageable tissue and, in many cases, lowers the level of amputation that is needed.

### 4. Removing dead tissue and choosing the level

Dead tissue is removed in the operating theatre, and more than one session may be needed before the borders settle. The level of amputation — toe, forefoot or higher — is chosen according to perfusion, infection and the tissue's ability to heal, and both the decision and its alternative are explained to you beforehand.

### 5. Healing, rehabilitation and preventing recurrence

Wounds are followed with regular dressings and correct offloading, physiotherapy starts early to preserve strength and balance, and therapeutic footwear or a prosthesis is assessed when needed. A permanent plan is set for blood sugar, cholesterol, blood pressure and stopping smoking, with regular foot checks and surveillance of perfusion.

## About this service

Gangrene is the death of body tissue, either because its blood supply has been cut off or because a severe infection has destroyed it. It appears most often in the toes, foot or leg, and sometimes in the hand or in internal tissue. It is not a single diagnosis. Dry gangrene develops slowly on the background of chronic arterial blockage, leaving tissue that is black, dry and fairly well demarcated. Wet and gas gangrene are accompanied by infection that spreads rapidly through the tissues and threatens life, not only the limb; these are surgical emergencies that cannot wait.

Treatment works along four lines at once: stabilising the patient, including fluids and blood sugar; controlling infection with treatment decided by the doctor; restoring blood flow to whatever tissue is still alive, by catheter or surgical bypass where that is feasible; and removing dead tissue. Removal may be limited debridement, amputation of a toe, or amputation at a higher level when the tissue cannot be salvaged — and the aim is always the lowest level that will heal and allow the best possible function.

The limits must be stated with complete frankness: tissue that has died does not come back to life. No dressing, ointment, oil, herbal mixture or oxygen session revives dead tissue — this myth costs people their limbs, because it delays intervention by weeks. Treatment stops the spread and preserves as much living tissue as possible. Smoking is the strongest modifiable factor in arterial disease and must be stopped at once.

If you have diabetes, remember that \*\*the absence of pain is false reassurance\*\* because of nerve damage, and that regular foot examination is non-negotiable. Real assessment comes from clinical examination and perfusion studies — not from reading.

## Preparation

Do not wait for a distant appointment: gangrene with fever, a foul smell or spreading redness is a same-day emergency. Do not apply oils, herbal preparations, powders or coloured solutions to the area, do not try to cut away black tissue or drain anything yourself, and do not tie anything tightly around the limb. Do not warm the foot with hot water or a hot bottle, and do not walk on it if the wound lies on a weight-bearing area. Bring a written list of your medicines and tell the doctor about blood thinners, diabetes treatment and any drug allergy, and bring previous imaging reports, culture results and blood-sugar records. Stop smoking immediately, keep to the fasting instruction if you are told theatre is likely, and come with someone to accompany you.

## Aftercare

Keep to the timing and technique of dressing changes as you were taught, and put nothing of your own on the wound. Elevate the limb only in the way your team specified, and continue offloading with the therapeutic shoe, cast or wheelchair as instructed. Start physiotherapy on schedule to preserve strength and prevent stiff joints. Control your blood sugar and record readings, continue cholesterol and blood-pressure medicines, and stay off cigarettes. Inspect the other foot daily as well — it now carries more load. Remember: \*\*dead tissue is not revived by ointments or herbs\*\*, and delay is what raises the level of amputation.

Seek care immediately if: \*\*redness spreads, or there is swelling, a foul smell or pus\*\* · \*\*you develop fever, shivering, low blood pressure or confusion\*\* (possible bloodstream infection — emergency now) · there is crackling or swelling under the skin · the black area enlarges or new blackening appears · \*\*the limb becomes cold, pale, painful and pulseless\*\* (acute arterial occlusion — hours decide whether the limb survives) · the wound bleeds · \*\*a leg suddenly becomes painful, swollen and warm\*\* (deep vein thrombosis — do not massage it) · \*\*chest pain or breathlessness\*\* develops (possible pulmonary embolism — emergency).
