العلاج ليس دواءً «ينشّط الكلى»، فلا وجود لمثل هذا الدواء. العلاج هو تصحيح السبب سريعاً: تعويض السوائل بالقدر المحسوب، وإيقاف كل ما يضرّ الكلى، وتصريف الانسداد إن وُجد، ومعالجة العدوى. ومدرّات البول تُستخدم عند الحاجة لتخفيف احتباس السوائل، لا لحماية الكلى ولا لتسريع الشفاء. وقد يحتاج بعض المرضى إلى غسيل كلوي \*\*مؤقّت\*\* لعبور المرحلة الحرجة، وهذا لا يعني الغسيل مدى الحياة.

كثير من الحالات تتحسّن بدرجة جيدة إذا عُولجت مبكراً، لكن التحسّن قد يكون جزئياً، ومن تعرّض لفشل كلوي حاد يبقى أكثر عرضة لمرض كلوي مزمن لاحقاً، لذلك المتابعة بعد التحسّن ضرورية وليست تفصيلاً. وشدّة الحالة وخطة علاجها تُحدَّد بالفحص السريري والمختبر والتصوير، لا بالتقدير الشخصي.

## خطوات الإجراء

### 1. التعرّف السريع وتقييم الخطورة

يُقاس ضغط الدم والنبض وتُقيَّم حالة السوائل وكمية البول، وتُطلب وظائف الكلى والأملاح وتحليل البول بشكل عاجل. ويُراجَع تاريخ الأيام الأخيرة بحثاً عن إسهال أو قيء أو حرارة أو دواء جديد أو مسكّن أو خلطة عشبية أو تصوير بصبغة، لأن الإجابة غالباً في هذه التفاصيل.

### 2. إيقاف المسبّبات فوراً

تُوقف أو تُعدَّل بإشراف الطبيب كل الأدوية والمواد التي قد تكون سبّبت الحالة أو تُفاقمها، وقد يشمل ذلك تعليقاً مؤقتاً لبعض أدوية الضغط أو السكر. لا توقف دواءك من تلقاء نفسك ولا تستمر عليه دون إبلاغ الطبيب، فالقرار يوازن بين حماية الكلى وحاجتك للدواء.

### 3. تصحيح السوائل ومعالجة السبب

إن كان السبب جفافاً تُعطى سوائل وريدية بكمية محسوبة ومراقبة، وإن كان انسداداً يُصرَّف البول بقسطرة أو دعامة أو تصريف عبر الجلد بحسب موضع الانسداد، وإن كانت عدوى تُعالج فوراً. تصحيح السبب هو العلاج الحقيقي، أما إعطاء سوائل بلا حساب فقد يسبّب احتباساً وضيق نفس.

### 4. مراقبة المضاعفات والغسيل المؤقّت عند اللزوم

تُراقَب مستويات البوتاسيوم والحماض وكمية البول وحالة الرئتين عن قرب، لأن ارتفاع البوتاسيوم قد يكون مهدّداً للحياة. وإذا لم تكفِ التدابير الدوائية، يبدأ غسيل كلوي مؤقّت لسحب السوائل والسموم حتى تتعافى الكلى، ويُوقف عند استعادة الوظيفة.

### 5. المتابعة بعد التحسّن

بعد استقرار الحالة تُعاد الفحوص بعد أسابيع لمعرفة إن كانت الوظيفة رجعت كاملة أم بقي نقص، ويُعاد تقييم الأدوية الموقوفة واحداً واحداً. ويُنصح بمتابعة ضغط الدم وتحليل الزلال دورياً، لأن نوبة الفشل الحاد ترفع خطر المرض المزمن لاحقاً.

## Procedure steps

### 1. Rapid recognition and severity assessment

Blood pressure and pulse are measured, fluid status and urine output are assessed, and kidney function, electrolytes and a urine test are requested urgently. The history of the last few days is reviewed for diarrhoea, vomiting, fever, a new medicine, a painkiller, a herbal mixture or contrast imaging, because the answer usually lies in these details.

### 2. Stopping the offending agents immediately

All medicines and substances that may have caused or are worsening the injury are stopped or adjusted under medical supervision, which may include temporarily holding some blood pressure or diabetes medicines. Do not stop a medicine on your own and do not continue it without telling your physician; the decision balances kidney protection against your need for the drug.

### 3. Correcting fluids and treating the cause

If dehydration is the cause, intravenous fluid is given in a calculated and monitored amount; if obstruction is the cause, urine is drained with a catheter, a ureteric stent or percutaneous drainage depending on the site; if infection is present it is treated at once. Correcting the cause is the real treatment, whereas uncalculated fluid can cause overload and breathlessness.

### 4. Monitoring complications and temporary dialysis when needed

Potassium, acid-base status, urine output and the lungs are monitored closely, because a high potassium level can be life-threatening. If medical measures are not enough, temporary dialysis is started to remove fluid and waste until the kidneys recover, and it is stopped once function returns.

### 5. Follow-up after recovery

Once the episode settles, tests are repeated after some weeks to see whether function returned fully or a deficit remains, and each withheld medicine is reassessed one at a time. Periodic monitoring of blood pressure and urine albumin is advised, because an episode of acute injury raises the later risk of chronic kidney disease.

إن كنت في الطريق إلى الطوارئ أو العيادة، لا تشرب كميات كبيرة من السوائل دفعة واحدة ولا تتناول مدرّاً للبول من تلقاء نفسك. اكتب على ورقة كل دواء ومسكّن ومكمّل وعشبة تناولتها في الأيام الأخيرة بأسمائها كما هي على العلبة، واحمل العلب معك إن أمكن. أخبر الطبيب بأي إسهال أو قيء أو حرارة أو تعرّض لحرارة الشمس أو صيام طويل أو مجهود شديد، وبأي تصوير بصبغة أو عملية أجريتها حديثاً. قدّر كمية البول في اليوم الأخير ووقت آخر تبوّل، فهذه معلومة مهمة للتشخيص. أحضر تقارير فحوصك السابقة إن توفّرت، وأخبر الطبيب إن كنت مصاباً بالسكري أو الضغط أو تضخّم البروستاتا أو حصى متكرّرة.

## العناية بعد الإجراء

## About this service

Acute kidney injury is a sudden fall in the kidneys' filtering capacity that develops over hours to days, usually detected by a rapid rise in kidney function results or a clear drop in urine output. It is an urgent condition and fundamentally different from chronic kidney failure: here time is decisive, and every delay increases the chance of permanent damage.

The commonest causes are severe fluid loss from diarrhoea, vomiting, fever, outdoor heat exposure or simply drinking too little; then medicines and substances that are toxic to the kidneys; then obstruction of urine flow by a stone or an enlarged prostate; then acute inflammation of kidney tissue or the glomeruli; and severe infection that reduces blood flow to the kidneys. Many of these causes are reversible if identified quickly.

One warning deserves emphasis: taking non-steroidal anti-inflammatory painkillers while dehydrated, or alongside certain blood pressure medicines, is among the best known avoidable causes of acute kidney injury. The same applies to herbal products, supplements and herbal stone-dissolving mixtures. Some plants are directly toxic to the kidney tubules, and patients have needed dialysis within days of taking a herbal preparation. Such products should never be given to a child, an older adult or a person with diabetes under any label.

Treatment is not a drug that activates the kidneys, because no such drug exists. Treatment means correcting the cause quickly: measured fluid replacement, stopping everything that harms the kidneys, relieving obstruction if present, and treating infection. Diuretics are used when needed to relieve fluid overload, not to protect the kidneys or speed recovery. Some patients need temporary dialysis to get through the critical phase, and that does not mean dialysis for life.

Many cases recover well when treated early, but recovery can be partial, and anyone who has had acute kidney injury remains at higher risk of chronic kidney disease later, so follow-up after recovery is essential rather than optional. Severity and the treatment plan are determined by clinical examination with laboratory and imaging results, not by personal judgement.

## Preparation

If you are on the way to an emergency department or clinic, do not drink very large volumes of fluid at once and do not take a diuretic on your own initiative. Write down every medicine, painkiller, supplement and herbal product you have taken in recent days, exactly as named on the package, and bring the packages if you can. Tell the treating team about any diarrhoea, vomiting, fever, heat exposure, prolonged fasting or heavy outdoor work, and about any recent contrast scan or operation. Estimate how much urine you passed in the last day and when you last passed urine, because this is important diagnostic information. Bring previous test reports if available, and mention diabetes, hypertension, prostate enlargement or recurrent stones.

## Aftercare

After the acute phase, keep the follow-up appointments your physician sets for repeat testing even if you have returned to normal activity, because recovery of function is measured in the laboratory and not by feeling better. Drink the amount of fluid you are advised to take and avoid dehydration, especially in hot weather, during fasting or with diarrhoea. Do not return to any non-steroidal painkiller, herbal mixture or supplement without explicit approval from your physician. Seek urgent care if you develop: \*\*a sharp drop in urine output or none at all\*\*, \*\*sudden swelling of the face and feet\*\*, \*\*blood in the urine\*\*, \*\*fever with flank pain\*\*, breathlessness or inability to lie flat, unrelenting nausea and vomiting, confusion or unusual drowsiness, or palpitations with severe muscle weakness.
