## Procedure steps

### 1. Assessment and diagnosis

Head circumference is measured and plotted against growth charts, a full neurological examination is performed including fundoscopy, and MRI or CT defines ventricle size and the level of obstruction. In small infants an ultrasound through the fontanelle is often added because it is quick, safe and repeatable.

### 2. Choosing the procedure and family discussion

The neurosurgeon explains why a shunt or an endoscopic opening is preferred for this particular child, the expected benefit, the risks, the alternative, and what may be needed later. This is the moment to ask every question and to request clarification before the consent form is signed.

### 3. Anaesthesia and theatre preparation

The child is placed under general anaesthesia with monitoring of breathing, blood pressure and temperature. Incision sites are marked and the skin is prepared, prophylactic antibiotics are given per protocol, and the valve or the endoscope is checked and set up before the operation starts.

### 4. The operation

For a shunt, a small scalp incision and an abdominal incision are made and a fine catheter is tunnelled under the skin to connect the ventricle to the abdominal cavity through a flow-regulating valve. For the endoscopic route, a fine scope passes through a small skull opening and the floor of the third ventricle is opened so fluid finds an internal path without an implanted device.

### 5. Recovery and long-term follow-up

The child is observed in hospital for one to three days, imaging is repeated to confirm that pressure has fallen, and the family is trained to recognise shunt failure and infection. Regular follow-up then tracks head circumference, growth, development and vision, with occupational or speech therapy referral when needed.

## About this service

Hydrocephalus is an abnormal build-up of cerebrospinal fluid inside the ventricles of the brain, caused either by an obstruction along its pathway or by impaired absorption. The extra fluid raises pressure inside the skull and presses on brain tissue that is still developing. In children the problem is mechanical, so it is managed surgically; medication does not correct it, and waiting for a child to grow out of it is not a treatment plan.

In infants the usual causes are congenital malformations of the fluid pathway, bleeding in the brain after premature birth, meningitis, and tumours blocking the flow. Presentation depends on age. A baby may show a head circumference crossing upward through growth percentiles, a full or tense fontanelle, separated skull sutures, eyes deviated downward, poor feeding, vomiting and unusual drowsiness. An older child more often has headache, morning vomiting, double vision or squint, unsteadiness, and a drop in school performance.

Two operations form the backbone of treatment. A ventriculoperitoneal shunt drains fluid from the ventricle into the abdominal cavity through a valve that regulates flow. Endoscopic third ventriculostomy creates an internal route for the fluid and suits selected obstructive patterns. The choice depends on the cause, the age of the child and the MRI or CT findings, and imaging is repeated after surgery to confirm the response.

The limits matter as much as the benefits. Diverting fluid stops the rising pressure and protects the remaining tissue, but it does not reverse injury that happened before treatment, does not treat an underlying tumour or malformation, and cannot guarantee a particular level of learning or movement. A shunt is a device: it can block, become infected or disconnect as the child grows, so long-term follow-up and occasional revision surgery are part of the plan. Candidacy and the type of procedure are decided by clinical examination and imaging, never by reading.

## Preparation

Tell the doctor about every medicine and supplement your child takes, any drug allergy, any bleeding or clotting problem, and any history of meningitis, previous brain surgery or an existing shunt. Bring earlier imaging reports, the head circumference record and the vaccination card, and report any recent fever, cough, diarrhoea or skin rash, because an active infection may postpone surgery. Follow the fasting time given by the anaesthetist exactly and do not allow water or sweets during it. Wash your child's hair and skin the evening before as instructed, and do not apply creams or oils to the head or abdomen. Arrange an overnight hospital stay with one consistent companion, and before admission ask which valve will be used, where the incisions will be, and which warning signs you will be watching for after discharge.

## Aftercare

Keep the wounds clean and dry, put no ointments or herbal preparations on them, and keep the dressing-change and wound-review appointments. Give pain relief as prescribed, return your child gradually to a normal diet and routine, and avoid rough play, jumping and swimming until the surgeon allows them. Do not press along the catheter track under the skin or fiddle with the valve site, and protect the head from knocks. Record head circumference and weight at every visit, and continue developmental assessment sessions if they were requested.

\*\*Go to the emergency department immediately if there is: repeated vomiting, especially in the morning; a headache that wakes the child from sleep; marked drowsiness or difficulty waking; a seizure; separating skull sutures, a bulging fontanelle or a rapid jump in head circumference in an infant; eyes deviating downward; fever with neck stiffness; redness, swelling or leaking fluid along the catheter track or wound; or abdominal distension with vomiting.\*\* These suggest raised intracranial pressure, shunt failure or infection and must not wait for a clinic appointment.
