## Procedure steps

### 1. Locating the source of pain from the history

The doctor asks exactly where the pain is felt — in the groin, over the outer bony prominence, or behind the thigh — whether it radiates down the leg with numbness, whether there is weakness or any change in bladder function, and whether it began after a fall or exertion.

### 2. Examining the hip and lower back

Hip range of motion is measured and pain on internal rotation tested, tendon and bursal sites are palpated, and the lower back, nerves, foot strength, sensation and gait are examined — because separating a hip source from a spinal source changes the treatment entirely.

### 3. Imaging directed by the examination

A hip radiograph is requested when osteoarthritis is suspected or after a fall to exclude a fracture, and MRI is reserved for selected situations such as suspected avascular necrosis, a labral tear, or a lumbar problem that warrants evaluation.

### 4. The non-surgical plan and rehabilitation

A strengthening programme for the hip and trunk muscles is built with range-of-motion exercises and load modification, weight reduction where relevant, use of a cane in the opposite hand when needed, and physician-directed pain relief for a limited period.

### 5. Discussing surgical options with realistic expectations

If the plan fails or osteoarthritis advances, the options including joint replacement are discussed, with an explanation of its limited service life, the role of rehabilitation before and after surgery, the lasting restrictions to expect, and that the goal is independent walking with less pain rather than a brand-new hip.

## About this service

Treating hip pain begins with pinpointing where the pain actually comes from, because what people call “hip pain” may arise from the joint itself, from the surrounding soft tissue, or from the lower back. True hip joint pain is usually felt in the groin and front of the thigh and may radiate to the knee; pain over the bony prominence on the outer side of the hip usually comes from tendon and bursal inflammation in that area; and pain running from the lower back down the back of the thigh and leg most often originates from the sciatic nerve.

One point must be stated clearly here: \*\*most cases of sciatica and low back pain improve without surgery\*\*, with graded movement, physiotherapy, activity modification and patience over some weeks, and there is no reason to rush to an operation in the absence of clear indications. There are, however, red flags that require emergency assessment immediately: \*\*sudden weakness in the leg or foot, loss of bladder or bowel control, or numbness in the saddle area between the thighs.\*\* These are never postponed and never put on a waiting list.

Hip osteoarthritis is treated first with a non-surgical plan: a strengthening programme for the hip and trunk muscles, range-of-motion work, load and activity modification, weight reduction where relevant, a cane held in the opposite hand when needed to reduce load, and physician-directed pain relief for a limited period. Complete rest is not treatment — \*\*prolonged lying down weakens muscle and increases stiffness and pain.\*\*

When osteoarthritis is advanced and the non-surgical plan has failed, hip replacement is discussed, and the whole truth must be told: \*\*an artificial joint has a limited service life\*\*, \*\*rehabilitation before and after surgery is half the result\*\*, and the goal is pain relief, reasonable function and independent walking — not a full return to the hip you had before the disease, and not a return to contact sports.

The limits of the other options are equally explicit: \*\*injections around the hip do not grow cartilage\*\*, and painkillers relieve without repairing. In hip fractures in older adults after a fall, casting is generally not an option at all, and the choice between fixation and replacement depends on the fracture type, its location and the patient’s condition rather than on preference.

## Preparation

Before the appointment, point with your finger to exactly where you feel the pain — that helps the doctor far more than a general description — and note whether it radiates to the knee or down the back of the leg. Write down when the pain began and what makes it worse: walking, stairs, rising from a chair, lying on that side, or prolonged sitting. Tell the doctor about any previous fall or injury, about long-term use of corticosteroid medicines because it raises certain possibilities, and about all your chronic conditions and medicines. Come in loose clothing that allows both hips and the back to be examined, and bring previous imaging with its reports. If you cannot bear weight on the leg after a fall, do not try to walk on it — go to the emergency department instead of waiting for a clinic appointment.

## Aftercare

Follow your exercise programme consistently, because strengthening the hip and trunk muscles is what reduces load on the joint and eases pain, and remember that results are measured in weeks. Modify activity rather than stopping it: reduce prolonged standing, repeated stairs and heavy lifting, keep walking with gradually increasing distances on level ground, and use a cane in the hand opposite the affected hip if advised — it genuinely reduces load and is not a sign of weakness. Avoid complete rest and prolonged lying down, which increase stiffness and muscle weakness. Reduce fall risks at home: loose rugs, poor lighting, a bathroom without a grab rail. \*\*Seek emergency care immediately if sudden weakness appears in the leg or foot, if you lose control of bladder or bowel, if you feel numbness in the saddle area between the thighs, if you cannot bear weight after a fall, or if fever develops with severe hip pain.\*\*
