## Procedure steps

### 1. Clinical assessment and growth evaluation

Assessment begins with personal and family history, and in children with height and weight measurement plotted on a growth chart, examination of bones, legs and teeth, and evaluation of muscle strength, thirst and urine volume. In adults, questions cover medications, occupational exposure, recurrent stones and immune disease.

### 2. Blood and urine tests for acid-base status and electrolytes

Tests are requested for electrolytes, potassium, sodium, chloride, calcium, phosphate and kidney function, together with blood acid-base measurement and a urine test examining acidity and the presence of glucose, protein and salts. Comparing what is in the blood with what is in the urine is what reveals which part of the tubule is affected.

### 3. Specialised functional tests and imaging

A timed urine collection may be needed, or functional tests measuring the kidney's ability to acidify or concentrate urine, along with ultrasound to look for calcification or stones and bone imaging when rickets is suspected. Immune tests or heavy-metal screening are requested in adults according to the history.

### 4. Replacement plan and treating the cause

A precise replacement plan is set for alkali, deficient salts and vitamin D according to the results, with defined fluid volumes. If the cause is acquired, the responsible medicine is stopped under supervision or the underlying disease is treated. The patient or family is taught how to keep to the daily regimen, because consistency of replacement is what protects bones and growth.

### 5. Regular monitoring and genetic counselling

Blood and urine tests are repeated at intervals to fine-tune the doses, since both excess and deficiency are harmful, while growth, bone health, kidney function and imaging for calcification are followed. When an inherited cause is likely, genetic counselling is offered to the family and siblings are screened, because early diagnosis in them prevents bone damage.

## About this service

Renal tubular disorders are a group of conditions in which the fault lies in the fine tubules that follow the filters inside the kidney. These tubules reabsorb water, salts, bicarbonate, glucose and small proteins, and excrete surplus acid. Their distinguishing feature is that the kidney's filtering capacity may be normal or near-normal while the problem sits in the sorting stage that follows, so substances that should have returned to the body are lost in the urine.

Because the site of the fault differs between conditions, the presentation varies widely: a child who fails to gain weight and height as expected; soft bones, rickets and bowing of the legs; muscle weakness and lethargy with low potassium; thirst and frequent urination not explained by diabetes; kidney stones and calcification; or a persistent acid-base abnormality on blood tests. In adults, the complaint may be no more than fatigue and repeated stones.

The best known of these disorders are the types of renal tubular acidosis; Fanconi syndrome, in which several substances are lost together in the urine such as glucose, phosphate, amino acids and bicarbonate; salt-wasting syndromes; and conditions in which the kidney fails to respond to the water-regulating hormone, producing large urine volumes. Causes are either inherited, usually appearing in childhood, or acquired in adults through certain medicines, heavy metals, immune disease or blood disorders.

Treatment is fundamentally replacement and correction rather than cure in the inherited types: alkali is given to correct acidosis, deficient salts such as potassium and phosphate are replaced, vitamin D deficiency and bone softening are treated, fluid balance is managed, and stones and calcification are addressed. If the cause is acquired, whether a medicine, a toxic metal or an immune disease, stopping or treating it may restore function partly or fully. Never adjust the amount of alkali or supplements yourself and do not buy supplements off the shelf: too much causes kidney calcification and too little allows bone damage, and the balance is set by periodic blood and urine testing.

Be cautious with herbal products, herbal stone-dissolving mixtures and non-steroidal anti-inflammatory painkillers, because they multiply the load on tubules that are already diseased and can cause acute deterioration. Suitability for any treatment or specialised test is determined by clinical examination and laboratory results, not by similarity of symptoms to another case.

## Preparation

Bring all your previous tests, especially electrolytes, potassium, calcium, phosphate and acid-base results, because the pattern of the abnormality over time is the key to diagnosis. Write an exact list of everything you take: medicines, calcium or vitamin D supplements, painkillers, herbal products and sports powders, since some of these cause these disorders or mask the results. Tell your physician about any occupational exposure to chemicals or metals. If the patient is a child, bring the growth chart, vaccination record and photographs of the legs if there is bowing. Mention any family history of rickets, stones, muscle weakness or marriage between relatives. If a timed urine collection is requested, ask for detailed instructions and the correct container before you leave the clinic.

## Aftercare

Take the alkali, salt and vitamin doses exactly as prescribed and at the right times, and never increase, reduce or stop them on your own, because both directions do harm: too much causes kidney calcification and too little allows bone and growth problems. Drink the recommended amount of fluid and avoid dehydration in hot weather, during fasting or with diarrhoea. Repeat laboratory tests on schedule, since doses are adjusted from them. In children, follow growth, teeth and walking with your physician. Seek urgent care if there is: \*\*a sharp drop in urine output\*\*, \*\*sudden swelling of the face and feet\*\*, \*\*blood in the urine\*\*, \*\*fever with flank pain\*\*, severe muscle weakness, palpitations or tingling around the mouth, vomiting and diarrhoea preventing fluid intake, or unusual lethargy in a child.
