أسباب الانسداد متعدّدة: حصوة الحالب وهي الأشيع عند البالغين في الأردن، وتضيّق في الحالب بعد التهاب أو عملية أو تفتيت حصوات، وضغط خارجي من كتلة أو تليّف، وتشوّه خلقي مثل تضيّق ملتقى حوض الكلية بالحالب عند الأطفال، وأورام المسالك. وهناك حالة قريبة يجب عدم خلطها به وهي \*\*الارتجاع البولي الحالبي\*\*، أي رجوع البول من المثانة صاعداً نحو الكلية، وهي تُشخَّص غالباً عند الأطفال بعد التهابات بولية متكرّرة وتُقيَّم في السياق نفسه، لكنها مشكلة اتجاه لا مشكلة انسداد، وعلاجها مختلف.

صورة الحالة تتراوح بين ألم خصر شديد متقطّع مع تقيّؤ في الانسداد الحادّ بحصوة، وغياب تامّ للأعراض في الانسداد المزمن التدريجي الذي يُكتشف بالتصوير أو بتحليل يُظهر خللاً في وظيفة الكلية. وهذه النقطة جوهرية: \*\*صمت الأعراض لا يعني السلامة\*\*، فبعض الحالات المزمنة تُكتشف بعد فقدان جزء من الوظيفة.

خطة العلاج تسير على مبدأين متتاليين: تصريف عاجل إذا كان هناك عدوى أو حمّى أو تدهور وظيفة أو ألم لا يُسيطر عليه، ويكون ذلك بدعامة حالبية أو بأنبوب كلوي عبر الجلد؛ ثم معالجة السبب نفسه بإزالة الحصوة أو توسيع التضيّق أو إعادة توصيل الحالب جراحياً. وحدود العلاج صريحة: \*\*الدعامة أو الأنبوب يصرّفان البول ولا يعالجان السبب\*\*، وقد يسبّبان انزعاجاً أو تكرار تبوّل أو دماً خفيفاً؛ \*\*وإزالة حصوة اليوم لا تمنع تكوّن حصوة جديدة\*\*، فالوقاية تحتاج تعديل سوائل وغذاء وربما تحليل استقلابي. والترشيح لأي خيار يُحدَّد بالفحص السريري والتصوير ووظيفة الكلية لا بالقراءة.

## خطوات الإجراء

### 1. التقييم وتحديد الشدّة

يقيّم الطبيب الألم وموضعه وانتشاره، ويقيس الحرارة والضغط والنبض، ويفحص الخصر والبطن، ويسأل عن كمية البول وعن الحمّى والتقيّؤ، ليحدّد إن كانت الحالة تحتاج تصريفاً عاجلاً أم تحمل المتابعة.

### 2. التصوير وتحاليل الوظيفة

يُبدأ عادة بالأمواج فوق الصوتية لتحديد وجود التوسّع ودرجته، وقد تُطلب طبقيّة محورية لتحديد موقع الانسداد وسببه بدقّة، مع تحليل بول وزراعة وتحاليل وظائف الكلى لتقييم الأثر الفعلي.

### 3. التصريف العاجل عند اللزوم

عند وجود عدوى أو حمّى أو تدهور وظيفة أو كلية وحيدة أو حمل، يُصرَّف الضغط سريعاً بدعامة حالبية تُركّب بالتنظير أو بأنبوب كلوي يوضع عبر الجلد بتوجيه التصوير، وذلك قبل معالجة السبب.

### 4. معالجة السبب

بعد استقرار الحالة يُعالج السبب: إزالة الحصوة أو تفتيتها، أو توسيع التضيّق أو قطعه، أو إعادة رأب ملتقى الحوض والحالب أو إعادة زرع الحالب في المثانة، والخيار يعتمد على الموقع والطول والوظيفة المتبقّية.

### 5. المتابعة والوقاية

تُعاد التصاوير والتحاليل بعد أسابيع للتأكد من زوال التوسّع وثبات الوظيفة، وتُنزع الدعامة في موعدها المحدّد بدقّة. وتُوضع خطة وقاية بالسوائل والتغذية، وقد يُطلب تقييم استقلابي عند تكرار الحصوات.

## Procedure steps

### 1. Assessment and grading severity

The doctor assesses the pain, its site and radiation, measures temperature, blood pressure and pulse, examines the flank and abdomen, and asks about urine volume, fever and vomiting, in order to decide whether the case needs urgent drainage or can be observed.

### 2. Imaging and function tests

Ultrasound is usually the first step to confirm dilatation and grade it, a CT scan may be requested to locate the obstruction and define its cause precisely, and urine analysis, culture and kidney function tests assess the actual impact.

### 3. Urgent drainage when required

When there is infection, fever, deteriorating function, a single kidney or pregnancy, the pressure is relieved quickly with an endoscopically placed ureteric stent or an image guided percutaneous nephrostomy tube, before the cause is addressed.

### 4. Treating the cause

Once stable, the cause is treated: stone removal or fragmentation, dilatation or incision of a stricture, reconstruction at the pelvi-ureteric junction, or reimplantation of the ureter into the bladder, the choice depending on site, length and remaining function.

### 5. Follow up and prevention

Imaging and blood tests are repeated after some weeks to confirm the dilatation has resolved and function is stable, and the stent is removed exactly on its scheduled date. A prevention plan for fluids and diet is agreed, with metabolic assessment when stones recur.

## التحضير قبل الإجراء

## العناية بعد الإجراء

اشرب سوائل كافية بحسب توجيه طبيبك، وسجّل كمية البول وأي تغيّر في لونه. إن كانت لديك دعامة حالبية فتوقّع انزعاجاً في الخصر أو أسفل البطن وتكرار تبوّل وأحياناً دماً خفيفاً، وهي أعراض شائعة تخفّ تدريجياً، لكن \*\*موعد نزع الدعامة إلزامي ولا يجوز تجاهله\*\* لأن بقاءها فترة أطول من اللازم قد يسبّب تكلّساً وانسداداً. وإن كان لديك أنبوب كلوي فحافظ على موضعه نظيفاً جافاً وثبّت الكيس ولا تشدّه. راجع فوراً عند \*\*حرارة أو رجفة مع ألم في الخصر، فهذا احتمال كلية مسدودة متعفّنة وهو طارئ\*\*، أو \*\*ألم خصر شديد لا يُسيطر عليه بالمسكّن\*\*، أو \*\*تقيّؤ يمنع تحمّل السوائل\*\*، أو \*\*قلّة البول الشديدة أو انقطاعه\*\*، أو \*\*دم غزير أو تخثّرات في البول\*\*، أو \*\*انزلاق الأنبوب الكلوي أو توقّف تصريفه\*\*، أو \*\*ألم مع كلية وحيدة أو في الحمل\*\*. ولا تؤجّل مراجعتك انتظاراً لتحسّن تلقائي.

## About this service

The ureter is a narrow tube that carries urine from the kidney to the bladder. Obstruction means this drainage is partly or completely halted, so pressure builds inside the kidney and its pelvis and calyces dilate, a state called hydronephrosis. It is important to understand that the dilatation is a consequence of the obstruction rather than a disease in its own right, so treating it means treating the cause. If obstruction persists without drainage the kidney may permanently lose part of its function, which is why obstruction accompanied by infection or by declining kidney function is treated as a situation needing prompt intervention.

The causes are varied: a ureteric stone, which is the commonest in adults in Jordan; a stricture following inflammation, surgery or stone fragmentation; external compression by a mass or by fibrosis; a congenital abnormality such as narrowing at the junction of the renal pelvis and ureter in children; and urinary tract tumours. A related condition should not be confused with it: \*\*vesicoureteral reflux\*\*, in which urine travels back up from the bladder towards the kidney. It is most often diagnosed in children after recurrent urinary infections and is assessed in the same setting, but it is a problem of direction rather than of blockage, and its treatment differs.

The clinical picture ranges from severe colicky flank pain with vomiting in acute obstruction by a stone, to a complete absence of symptoms in gradual chronic obstruction discovered on imaging or through a blood test showing impaired kidney function. This point is fundamental: \*\*the absence of symptoms does not mean safety\*\*, since some chronic cases are found only after part of the kidney function has been lost.

The treatment plan follows two sequential principles. Urgent drainage comes first if there is infection, fever, deteriorating function or uncontrolled pain, achieved with a ureteric stent or a percutaneous nephrostomy tube. The cause itself is then addressed by removing the stone, dilating the stricture or surgically reconnecting the ureter. The limits are explicit: \*\*a stent or nephrostomy drains urine, it does not treat the cause\*\*, and either may cause discomfort, urinary frequency or light bleeding; and \*\*removing a stone today does not prevent a new one forming\*\*, so prevention needs changes in fluid and diet and sometimes metabolic testing. Suitability for any option is decided by clinical assessment, imaging and kidney function, not by reading.

## Preparation

Bring all previous imaging and blood results on disc or in digital form, since comparing the degree of dilatation and the kidney function with earlier studies determines how urgent the situation is. Tell the doctor if you have a single kidney, a transplanted kidney or pre-existing impaired kidney function, if you are pregnant, if you take blood thinners or antiplatelet medicines, and about any known allergy to contrast media, iodine or antibiotics. Mention any previous urinary or pelvic surgery, stone fragmentation, or an earlier stent and when it is due to be removed if it is still in place. If a procedure under anaesthesia is planned, follow the fasting period the clinic specifies exactly and arrange a companion for the journey home. Note roughly how much urine you pass daily if the amount has fallen.

## Aftercare

Drink fluids as your doctor advises, and record your urine volume and any change in its colour. If you have a ureteric stent, expect flank or lower abdominal discomfort, urinary frequency and sometimes light bleeding; these are common and settle gradually, but \*\*the appointment to remove the stent is mandatory and must not be ignored\*\*, because leaving it longer than intended can lead to encrustation and blockage. If you have a nephrostomy tube, keep the exit site clean and dry, secure the bag and never pull on it. Seek care immediately for \*\*fever or chills with flank pain, which may mean an infected obstructed kidney and is an emergency\*\*, \*\*severe flank pain not controlled by analgesia\*\*, \*\*vomiting that prevents you keeping fluids down\*\*, \*\*markedly reduced or absent urine output\*\*, \*\*heavy bleeding or clots in the urine\*\*, \*\*a nephrostomy tube that has slipped or stopped draining\*\*, or \*\*pain in the setting of a single kidney or pregnancy\*\*. Do not postpone review hoping it will settle on its own.
