## Procedure steps

### 1. Assessment of the tumour's impact

A full neurological examination is performed with contrast-enhanced MRI, adding visual field or hearing testing depending on the site. What matters at this stage is not size alone but what the tumour is pressing on, whether there is swelling around it, and whether there are seizures or genuine functional decline that call for prompt intervention.

### 2. Deciding between surveillance and intervention

The team discusses whether the tumour requires treatment now. The patient is given an explicit plan: defined imaging dates if surveillance is chosen, or a treatment date if intervention is needed. Warning signs that justify an earlier review are explained, and the patient is asked not to postpone imaging simply because they feel well.

### 3. Surgery and planned resection

When surgery is chosen, a defined corridor to the tumour is opened under image guidance, and the tumour is separated from the brain from the outside with an attempt to remove the involved membrane. Nerves are electrically monitored at sensitive sites, the surgeon stops at the safe limit, and the specimen is sent for histopathology to establish type and grade.

### 4. Radiotherapy when indicated

Targeted radiotherapy may be used instead of surgery for small tumours or those in hazardous locations, as a complement to surgery for a remnant after partial removal, or if the tumour returns. It is planned precisely on imaging to limit dose to healthy tissue, and its effect is assessed with scans over months rather than days.

### 5. Long-term follow-up and rehabilitation

Imaging follow-up continues for years after any treatment option, because some meningiomas return after a long interval. Remaining symptoms are addressed with physiotherapy, speech therapy or ophthalmic follow-up, anti-seizure medicines are reviewed with the doctor and never stopped independently, and every scan is archived for comparison over time.

## About this service

Meningiomas are tumours that arise from the membranes covering the brain and spinal cord rather than from brain tissue itself, so they grow from the outside and press on the brain instead of infiltrating it. Most are slow-growing and not malignant, and many are discovered incidentally on a scan ordered for another reason such as headache or a head injury. Symptoms depend on location: a tumour on the convex surface of the brain may cause a seizure or weakness of a limb, one near the optic nerve may cause declining vision, and one at the skull base may cause facial numbness or dizziness.

Not every meningioma needs treatment now, and this is one of the facts patients most often do not know. If the tumour is small, causing no symptoms and not pressing on a critical structure, the correct decision may be to monitor it with imaging at defined intervals. Surveillance is a considered treatment plan, not neglect and not procrastination, but it only works if the imaging appointments are kept exactly, because early growth produces no symptom at all.

When treatment is required, the main options are surgery, radiotherapy, or a combination, chosen according to location, size, symptoms, and the patient's age and general condition. Surgery aims to remove the tumour together with the involved membrane as far as the location allows, and it is often most suitable for accessible convexity tumours on the surface. For tumours adherent to large vessels or sensitive nerves at the skull base, deliberately partial removal followed by radiotherapy to the remnant may be the safer strategy.

The limits of treatment should be clear before it starts. Radiotherapy does not dissolve the tumour immediately; its aim is to stop growth, and the tumour may remain visible on scans for years while being inactive. Surgery does not remove the possibility of recurrence, particularly after partial removal or with a higher-grade tumour. Neither surgery nor radiation repairs neurological damage that was already established before treatment, such as a lost part of the visual field. Grade is never determined by imaging: it is determined by histopathology once tissue has been obtained, and it guides the decision on surveillance or additional treatment.

Decisions here are made by a multidisciplinary team of neurosurgeon, radiologist, radiation oncologist and pathologist, sometimes with an ophthalmologist or ENT surgeon depending on the site. Suitability for any option is decided by clinical examination, imaging and team discussion, not by comparing your case with someone else's.

## Preparation

Bring all previous scans as digital files together with their reports, because knowing how fast the tumour grew between two studies can change the decision from surveillance to treatment or the reverse. Tell your doctor about every medicine you take, mentioning blood thinners, aspirin, contraceptive pills or hormone therapy, and herbal supplements. Report any seizure or brief loss of awareness even if it happened once years ago, and any decline in vision, hearing or smell, or numbness of the face. Mention any previous radiotherapy to the head. Tell the team if you have a pacemaker, a nerve stimulator or metal implants, and about any contrast allergy or kidney problem. If surgery is planned, follow the fasting instructions to the hour, and arrange someone to stay with you and to drive you home. If you are pregnant or could be, say so before any imaging or treatment. Write your questions down in advance and ask directly: what is the goal of this treatment, and how likely is it that further treatment will be needed afterwards?

## Aftercare

After surgery expect headache, swelling and fatigue in the first days with gradual improvement; keep the wound clean and dry, take short frequent walks, and avoid heavy lifting, deep bending and air travel until your doctor allows it. After radiotherapy you may feel tired or nauseated or notice skin and scalp irritation, and it may take weeks before you notice a difference. Do not expect the tumour to disappear quickly from scans, because the aim of radiation is to stop growth. Never stop anti-seizure medicine or cortisone on your own, and keep ophthalmology appointments if the tumour lies near the optic nerve. \*\*Go to the emergency department immediately if you have: a severe headache that worsens rapidly or wakes you from sleep with repeated vomiting; a seizure; new weakness or numbness on one side; a sudden change in vision or speech; confusion or severe drowsiness; fever with a stiff neck; or clear fluid leaking from the wound or nose.\*\* Keep your imaging appointments for years after treatment: late recurrence is detected on a scan long before it causes any symptom.
