## Procedure steps

### 1. Early recognition and urgent assessment

In the emergency department vital signs are measured immediately: pulse, breathing rate, blood pressure, temperature, level of consciousness and oxygen saturation, while the likely source of infection is sought. The patient is triaged by severity and placed on a fast-track pathway, because every hour of delay counts in this illness.

### 2. Immediate samples and tests

Blood cultures are drawn from two sites, along with samples from the suspected source such as urine, sputum, abscess fluid or a wound swab, plus blood tests, organ function, inflammatory markers and lactate, and targeted imaging to find the source. Samples are taken quickly so they never delay the start of treatment.

### 3. Urgent fluids and intravenous antibiotics

Broad-spectrum intravenous antibiotics are started as soon as possible after samples are taken, together with intravenous fluids to support the circulation and oxygen where needed, while urine output, blood pressure and consciousness are monitored. Medicines to raise blood pressure are added and the patient is moved to intensive care if deterioration continues.

### 4. Source control

The site from which the infection started is identified and treated with a procedure when needed: draining an abscess, relieving an obstruction in the urinary tract, removing an infected catheter or implanted device, debriding dead tissue, or abdominal surgery. \*\*Without this step, medicine alone does not succeed.\*\*

### 5. Narrowing treatment, follow-up and rehabilitation

Once culture and susceptibility results are back, the antibiotic is narrowed to the most precise option and the length of the course is defined. A recovery phase is then planned: gradual mobilisation, nutrition, breathing and muscle support and monitoring of organ function, with an explanation that fatigue and poor concentration may persist for weeks or months after discharge.

## About this service

Sepsis, commonly called blood poisoning, is an excessive and dysregulated response by the body to an infection that began in a specific place — the lungs, urinary tract, skin, abdomen or a wound — turning a local infection into a whole-body reaction that damages the organs themselves and drops blood pressure and blood flow to them. It is a time-critical emergency managed in hospital. It cannot be managed at home, in a clinic, or over the phone.

\*\*In this illness, time is the treatment.\*\* The pathway starts with rapid recognition in the emergency department, followed by immediately drawing blood cultures and samples from the suspected site, and starting broad-spectrum intravenous antibiotics as fast as possible, alongside intravenous fluids to support the circulation, oxygen, medicines to raise blood pressure when needed, and close monitoring in intensive care if the patient deteriorates. Antibiotics are then narrowed once culture and susceptibility results are available.

The decisive step that is frequently overlooked is source control: \*\*if the cause is an abscess, an obstruction in the urinary tract, an infected catheter or dead tissue, no antibiotic however powerful will cure the patient without a procedure to drain or remove that source.\*\* In the same way, \*\*stopping antibiotics early or adjusting the dose yourself opens the door to resistance that makes any future episode far harder to treat\*\*, and treatment is continued to its end by medical decision rather than by how well the patient feels.

There is also a dangerous myth that must be corrected plainly: \*\*waiting at home for the fever to "break", or taking an antibiotic tablet bought at a pharmacy, is the most dangerous mistake in this illness.\*\* Sepsis can present with a normal or even low temperature, particularly in older adults and people with weakened immunity, and the first sign may be confusion or rapid breathing rather than fever at all. Another limit worth knowing is that surviving sepsis does not mean an immediate return to your previous state: profound fatigue, muscle weakness, and disturbed concentration and sleep can persist for weeks or months. This is an expected course that needs rehabilitation and follow-up, and a previous episode grants no immunity against another. Eligibility for every step, and the treatment itself, are determined by urgent clinical assessment.

## Preparation

This is an emergency: if the warning signs appear, go to the emergency department immediately, do not wait for an appointment and do not try a medicine at home. Take a list of your medicines, chronic conditions and drug allergies, plus any report of recent surgery, a catheter, an implanted device, dialysis sessions or chemotherapy. Tell the emergency team from the first moment which symptom appeared first and when it started, whether there is a wound, pain on passing urine, a cough, diarrhoea or abdominal pain, and whether any antibiotic was taken in recent days. Mention if the patient is pregnant, an older adult, immunocompromised or has had the spleen removed, and do not give food or drink to someone who is drowsy. Bring a companion who knows the medical history.

## Aftercare

After discharge, complete the antibiotic course exactly as prescribed and do not adjust it yourself, and attend all follow-up appointments and organ-function tests. Expect a gradual recovery: increase activity daily without exhausting yourself, drink enough fluids, pay attention to nutrition and protein, and keep a regular sleep pattern — fatigue and poor concentration for weeks or months are expected after sepsis. Care for any wound or drain site exactly as explained, and update your vaccinations as your doctor advises. Go to the emergency department immediately — not to a clinic — if any of the following occurs: \*\*rapid or laboured breathing\*\* · \*\*confusion or unusual drowsiness\*\* · \*\*a high fever, or a low temperature with heavy shivering\*\* · \*\*a rash that does not fade under pressure\*\* · \*\*reduced urine output or dizziness on standing\*\* · \*\*cold, mottled or bluish skin\*\* · \*\*worsening severe pain or spreading redness around a wound\*\*.
