في المرحلة الأولى يظهر التسوّس كبقعة بيضاء أو بنية على السطح بلا تجويف، وهذه المرحلة قابلة للتوقّف والانتكاس إلى الشفاء عبر الفلورايد المهني والمعاجين المخصّصة وتنظيف الفم وتقليل تكرار السكريات. أما إذا تكوّن تجويف فعلي في المينا أو العاج فلا يعود التمعدن كافياً، ويصبح الترميم بالحشوة هو المعالجة. وإذا امتدّ التسوّس إلى لبّ السن ظهر ألم تلقائي ونابض ويحتاج علاج جذور، وإذا تكوّن خُرّاج فقد يلزم تصريفه، وإذا فُقد جزء كبير من السن فقد يكون الخلع ثم التعويض هو الخيار الواقعي.

ما لا يفعله علاج التسوّس مهمٌ بالقدر نفسه: الحشوة تُصلح الضرر ولا تُزيل السبب، فمن يُحشى له سن ويستمر على نفس نمط التنظيف والسكريات يعود إليه التسوّس في سن آخر أو حول حافة الحشوة. والفلورايد يوقف التسوّس المبكر ولا يُغلق تجويفاً موجوداً. ولا يوجد غسول ولا معجون ولا وصفة منزلية تُعيد بناء نسيج سن مفقود، ولا يشفي التسوّس بالمضمضة أو بالأعشاب. كما أن غياب الألم لا يعني غياب التسوّس؛ فأخطر أنواعه ينمو صامتاً بين الأسنان ويُكتشف بالأشعة.

وهنا خرافة شائعة وخطيرة يجب تصحيحها: وضع حبة الأسبرين أو مسحوق دوائي كاوٍ مباشرة على السن أو اللثة لا يُسكّن الألم بل يُحرق نسيج اللثة كيميائياً ويزيد المشكلة. كذلك زوال الألم من تلقاء نفسه ليس دليل شفاء، فقد يعني موت لبّ السن وانتقال المشكلة صامتة إلى ما حول الجذر.

خطة علاجك تُحدَّد بالفحص السريري وصور الأشعة وتقييم عوامل الخطر عندك (جفاف الفم، تناول المشروبات السكرية، تقويم أو تعويضات، أمراض مزمنة)، لا بقراءة صفحة. وفي حالات التسوّس المتعدد تُرتَّب الأسنان بالأولوية: يُوقف الأشدّ خطراً على العصب أولاً ثم تُستكمل البقية.

## خطوات الإجراء

### 1. التقييم وتحديد المرحلة

يفحص الطبيب كل الأسنان ويستعين بصور الأشعة لتحديد مرحلة كل تسوّس: بقعة مبكرة، تجويف في المينا أو العاج، أو وصول إلى اللبّ، لأن كل مرحلة لها علاج مختلف تماماً.

### 2. ضبط عوامل الخطر

يُراجع نمط التنظيف وتكرار السكريات والمشروبات الغازية وجفاف الفم وأثر بعض الأدوية، وتُعطى تعليمات تنظيف وخيط وفلورايد، لأن إصلاح الأسنان دون ضبط السبب يعني تكرار التسوّس.

### 3. إيقاف التسوّس المبكر

في البقع المبكرة تُطبَّق مواد إعادة التمعدن والفلورايد المهني وتُغلق الشقوق العميقة في الأسنان الخلفية بمادة سادّة عند الحاجة، ثم تُتابع البقعة في مواعيد لاحقة للتأكد من ثباتها.

### 4. ترميم التجاويف

تُزال الأنسجة المتسوّسة ويُرمَّم السن بحشوة مباشرة أو غير مباشرة أو بتاج إذا كان الفقد كبيراً، ويُرتَّب العمل على جلسات حسب عدد الأسنان وحالة اللثة.

### 5. علاج اللبّ أو الخلع والمتابعة

إذا وصل التسوّس إلى العصب يُجرى علاج جذور ثم ترميم، وإذا كان السن غير قابل للترميم يُناقَش الخلع والتعويض؛ ثم تُحدَّد مواعيد متابعة دورية مع صور أشعة للكشف عن تسوّس جديد أو ثانوي مبكراً.

## Procedure steps

### 1. Assessment and staging

The dentist examines all teeth and uses radiographs to stage each lesion — early spot, cavitation in enamel or dentine, or pulp involvement — because each stage has a completely different treatment.

### 2. Controlling risk factors

Cleaning technique, frequency of sugar and soft drinks, dry mouth and the effect of certain medicines are reviewed, and brushing, flossing and fluoride advice is given — repairing teeth without controlling the cause simply repeats the disease.

### 3. Arresting early lesions

For early lesions, remineralising agents and professional fluoride are applied and deep grooves in back teeth may be sealed. The lesion is then reviewed at later visits to confirm it has stabilised.

### 4. Restoring cavities

Decayed tissue is removed and the tooth is restored with a direct or indirect filling, or a crown when tooth loss is extensive. Work is spread over visits according to the number of teeth and gum condition.

### 5. Pulp treatment or extraction, then follow-up

If decay has reached the nerve, root canal treatment followed by a restoration is performed; if the tooth is unrestorable, extraction and replacement are discussed. Regular reviews with radiographs then follow to detect new or secondary decay early.

## التحضير قبل الإجراء

دوّن قبل الموعد متى بدأ الألم، وما يزيده (البرودة، الحرارة، الحلويات، العضّ)، وهل يأتي تلقائياً أو يوقظك ليلاً، وهل تناولت مسكّنات ومتى؛ فهذه التفاصيل تغيّر التشخيص فعلياً. أخبر الطبيب بأمراضك المزمنة وخاصة السكري ونقص المناعة، وبأدويتك ومنها مسيّلات الدم وأدوية هشاشة العظام، وبحملك أو إرضاعك، وبأي حساسية من التخدير الموضعي. أحضر صوراً أو تقارير أسنان سابقة إن كانت متوفّرة. لا تضع أسبرين ولا مساحيق على السن أو اللثة قبل الموعد، ولا تفتح دواء مضاد حيوي من تلقاء نفسك. تناول طعاماً خفيفاً قبل الجلسة، ونظّف أسنانك، وتوقّع أن تُطلب صورة أشعة قبل وضع أي خطة.

## العناية بعد الإجراء

بعد أي جلسة ترميم انتظر انتهاء التخدير قبل الأكل، وتوقّع حساسية خفيفة للبرودة أو ضغط العضّ لأيام إلى أسبوعين. التزم بمواعيد الجلسات المتبقية حتى لو زال الألم، فوقف العلاج في منتصفه يترك أسناناً مكشوفة لتسوّس أسرع. نظّف مرتين يومياً بمعجون يحتوي فلورايد، واستخدم الخيط أو الفرشاة البينية، وقلّل تكرار السكريات والمشروبات الحمضية أكثر من تقليل كميتها، واشرب الماء بعد الوجبات. إن كان فمك جافاً بسبب دواء أو حالة مزمنة فأخبر الطبيب لأن ذلك يرفع خطر التسوّس كثيراً. \*\*راجع فوراً عند: ألم نابض مستمر أو يوقظك من النوم، تورّم في اللثة أو الوجه، ارتفاع حرارة، طعم كريه أو خروج قيح، أو ألم شديد عند العضّ على سن مُعالَج.\*\* أما تورّم الوجه المنتشر أو صعوبة البلع أو التنفّس أو انتفاخ تحت العين فحالة طارئة تُراجَع في الطوارئ فوراً.

## About this service

Dental caries is a progressive disease in which acids produced by plaque bacteria dissolve the minerals of enamel and then dentine, creating a cavity that does not heal on its own. Treating decay is therefore not one procedure but a pathway chosen according to the stage of disease: remineralising an early white spot, restoring an established cavity, treating the nerve, or extracting a tooth that has collapsed beyond repair.

At the earliest stage decay appears as a white or brown spot on the surface with no cavitation. This stage can be arrested and reversed with professional fluoride, prescribed pastes, effective cleaning and a reduction in how often sugar is consumed. Once a true cavity has formed in enamel or dentine, remineralisation is no longer sufficient and a restoration is the treatment. If decay reaches the pulp, spontaneous throbbing pain appears and root canal treatment is required; if an abscess has formed it may need drainage; and if too much of the tooth is destroyed, extraction followed by a replacement may be the realistic option.

What decay treatment does not do matters just as much. A filling repairs damage but does not remove the cause: if cleaning habits and sugar frequency stay the same, decay returns in another tooth or at the margin of the restoration. Fluoride arrests early lesions but does not close an existing cavity. No mouthwash, toothpaste or home remedy rebuilds lost tooth structure, and decay is not cured by rinsing or by herbal preparations. Absence of pain is also not absence of disease — some of the most damaging lesions grow silently between teeth and are found only on radiographs.

One dangerous myth deserves direct correction: placing an aspirin tablet or a caustic powder directly on the tooth or gum does not relieve pain — it chemically burns the gum tissue and makes matters worse. Similarly, pain that disappears by itself is not proof of healing; it may mean the pulp has died and the problem has moved silently to the tissues around the root.

Your treatment plan is set by clinical examination, radiographs and an assessment of your personal risk factors — dry mouth, sugary drinks, orthodontic appliances or prostheses, chronic illness — not by reading a page. When many teeth are affected, they are prioritised: the lesions that most threaten the nerve are stabilised first and the rest are completed in sequence.

## Preparation

Before the visit, note when the pain started, what triggers it (cold, heat, sweets, biting), whether it comes on spontaneously or wakes you at night, and which painkillers you have taken and when — these details genuinely change the diagnosis. Tell your dentist about chronic conditions, especially diabetes and reduced immunity, about your medications including blood thinners and bone medication, about pregnancy or breastfeeding, and about any reaction to local anaesthetic. Bring previous dental images or reports if you have them. Do not place aspirin or powders on the tooth or gum, and do not start an antibiotic on your own. Eat a light meal beforehand, brush your teeth, and expect a radiograph before any plan is made.

## Aftercare

After any restorative visit, wait for the anaesthetic to wear off before eating, and expect mild sensitivity to cold or biting pressure for a few days up to two weeks. Keep the remaining appointments even if the pain has gone — abandoning treatment midway leaves teeth exposed to faster decay. Brush twice daily with fluoride toothpaste, use floss or interdental brushes, and reduce how often rather than only how much sugar and acidic drinks you take; drink water after meals. If your mouth is dry because of medication or a chronic condition, tell your dentist, as this markedly increases decay risk. \*\*Seek care promptly if you develop: persistent or night-waking throbbing pain, gum or facial swelling, fever, a bad taste or pus, or severe pain on biting a treated tooth.\*\* Spreading facial swelling, difficulty swallowing or breathing, or swelling below the eye requires emergency care immediately.
