ليس كل تليف واحداً، والفرق مهمّ للعلاج: بعض الأنماط سببها معروف كالتعرّض المهني للغبار والسيليكا، أو أمراض المناعة الذاتية، أو بعض العلاجات الدوائية والإشعاعية، أو التهاب رئوي تحسّسي من الطيور والعفن والرطوبة في المنزل، وبعضها مجهول السبب. لذلك يقوم التشخيص على مجموعة معاً: التاريخ المهني والدوائي والعائلي المفصّل، تصوير مقطعي عالي الدقّة للصدر، فحص وظائف الرئة وقياس سعة انتشار الغازات، تحاليل مناعية، وأحياناً غسل قصبي أو خزعة رئة عند الحاجة إلى حسم النمط.

وهنا يجب أن تكون الحدود صريحة: النسيج المتليّف لا يُعكَس ولا «يُنظَّف» ولا يعود سليماً، ولا يوجد دواء ولا عشبة ولا جلسة تُذيب التندّب. ما هو متاح فعلاً هو إبطاء التقدّم في أنماط معيّنة بأدوية مضادّة للتليف تُوصف وتُراقَب مختبرياً بقرار أخصائي، ومعالجة السبب حين يكون قابلاً للعلاج مثل إبعاد المريض عن مصدر التعرّض أو علاج مرض المناعة، ثم تحسين الأعراض والقدرة البدنية بالتأهيل الرئوي والأكسجين عند نقصه. أما زرع الرئة فخيار محدود جداً لحالات مختارة تُقيَّم في مراكز متخصّصة.

ومن الخرافات التي تؤذي مريض التليف: أن حبس النفس أو نفخ البالونات «يقوّي الرئة» ويفتح التليف. هذا غير صحيح ولا يفيد شيئاً، وقد يسبّب دوخة وإجهاداً. التمارين المفيدة هي برنامج تأهيل رئوي منظّم يجمع تمارين المشي والقوة مع تدريب على أنماط تنفّس تقلّل الإحساس بالضيق، تحت إشراف ومتابعة.

والإقلاع عن التدخين ضروري لكل مريض تليف، كما أن ضبط الأمراض المصاحبة مهمّ: ارتجاع الحمض، انقطاع النفس النومي، ارتفاع ضغط الشريان الرئوي، ومشاكل القلب. والتطعيمات الموصى بها تقلّل نوبات العدوى التي قد تسبّب تدهوراً حاداً خطيراً. الترشيح لكل هذه الخيارات يُحدَّد بالفحص السريري والصور والوظائف، لا بالمقارنة مع تجربة مريض آخر.

## خطوات الإجراء

### 1. تاريخ مرضي ومهني مفصّل

يسأل الطبيب عن مدّة ضيق النفس وتطوّره، وعن كل عمل عملته وتعرّض للغبار أو السيليكا أو الأسبستوس أو الأبخرة، وعن الطيور والعفن والرطوبة في المنزل، وعن كل دواء وعلاج إشعاعي سابق، وعن أمراض المناعة في العائلة. هذه الأسئلة تحدّد النمط وقد تكشف سبباً يمكن إبعاده.

### 2. التصوير المقطعي عالي الدقّة

صورة الصدر العادية لا تكفي لتشخيص التليف ولا لتحديد نمطه، لذا يُطلب تصوير مقطعي عالي الدقّة يُظهر توزّع التندّب وشكله. النمط الظاهر على هذا التصوير هو أحد أهمّ محدّدات التشخيص والعلاج، ويُقرأ مع بقيّة المعطيات في لجنة أو مراجعة متخصّصة عند الحاجة.

### 3. وظائف الرئة وقياس الأكسجين بالمجهود

يُقاس حجم الرئة وسعة انتشار الغازات لتحديد شدّة التقييد ومدى تأثّر انتقال الأكسجين، ويُجرى اختبار مشي لقياس هبوط الأكسجين مع المجهود. هذه الأرقام هي المرجع الذي تُقاس عليه المتابعة لاحقاً، ولهذا تُكرَّر بفواصل منتظمة لمعرفة إن كان المرض مستقرّاً أم متقدّماً.

### 4. تحاليل مناعية وخزعة عند الحاجة

تُطلب تحاليل مناعية للبحث عن مرض نسيج ضامّ خلف التليف، وقد يُجرى غسل قصبي أو خزعة رئة إذا بقي النمط غير محدّد، لأن العلاج يختلف بين التليف المجهول السبب والتليف المناعي والالتهاب الرئوي التحسّسي. القرار بالخزعة يوازن قيمة المعلومة مقابل مخاطر الإجراء.

### 5. خطة العلاج والتأهيل والمتابعة

تُبنى خطة تجمع علاج السبب أو إبعاد التعرّض، ودواءً مضادّاً للتليف بقرار أخصائي مع مراقبة مختبرية لأنماط معيّنة، وتأهيلاً رئوياً منظّماً، وأكسجيناً عند نقصه، وتطعيمات، وضبط الارتجاع وانقطاع النفس النومي. تُحدَّد متابعة دورية، ويُقيَّم الزرع في حالات مختارة بمركز متخصّص.

## Procedure steps

### 1. Detailed medical and occupational history

The physician asks how long breathlessness has been present and how it has progressed, about every job you have held and exposure to dust, silica, asbestos, or fumes, about birds, mould, and damp at home, about every medication and any past radiotherapy, and about autoimmune disease in the family. These questions define the pattern and may reveal a removable cause.

### 2. High-resolution computed tomography

A plain chest X-ray is not enough to diagnose fibrosis or define its pattern, so high-resolution computed tomography is requested to show how the scarring is distributed and shaped. The pattern seen on that scan is among the strongest determinants of diagnosis and treatment, and it is interpreted alongside the other findings, in a multidisciplinary review when needed.

### 3. Lung function and exertional oxygen testing

Lung volumes and gas transfer capacity are measured to grade how restricted the lungs are and how far oxygen transfer is affected, and a walk test records how much oxygen falls with exertion. These numbers become the baseline against which follow-up is judged, which is why they are repeated at regular intervals to see whether the disease is stable or advancing.

### 4. Immune tests and biopsy when needed

Immune blood tests look for a connective tissue disease behind the fibrosis, and bronchial lavage or a lung biopsy may be performed if the pattern remains unresolved, because treatment differs between idiopathic fibrosis, autoimmune-related fibrosis, and hypersensitivity pneumonitis. The decision to biopsy weighs the value of the information against the risk of the procedure.

### 5. Treatment, rehabilitation and follow-up plan

The plan combines treating the cause or removing the exposure, antifibrotic medication prescribed by a specialist with laboratory monitoring for specific patterns, structured pulmonary rehabilitation, oxygen when saturation is low, vaccinations, and control of reflux and sleep apnoea. Regular follow-up is scheduled, and transplant assessment is considered for selected patients at a specialised centre.

## About this service

Pulmonary fibrosis is progressive scarring of the thin tissue that separates the air sacs from the blood capillaries. That tissue becomes thicker and stiffer, so oxygen crosses less easily and the lung expands less freely. Typical symptoms are breathlessness that begins on exertion and advances over time, a persistent dry cough, and fatigue, often with a characteristic crackling sound on chest examination and changes in the fingertips.

Not all fibrosis is the same, and the difference matters for treatment. Some patterns have a known cause: occupational exposure to dust and silica, autoimmune disease, certain medications and radiotherapy, or hypersensitivity pneumonitis from birds, mould, and damp housing. Others have no identified cause. Diagnosis therefore rests on a combination: a detailed occupational, drug, and family history, high-resolution computed tomography of the chest, lung function testing with gas transfer measurement, immune blood tests, and sometimes bronchial lavage or a lung biopsy when the pattern must be settled definitively.

The limits deserve plain language. Scarred lung tissue cannot be reversed, cleaned out, or restored, and no medication, herb, or session dissolves fibrosis. What genuinely exists is slowing progression in specific patterns using antifibrotic medication prescribed and monitored by a specialist, treating the cause where it is treatable — removing the patient from the exposure or managing autoimmune disease — and then improving symptoms and physical capacity through pulmonary rehabilitation and oxygen when saturation is low. Lung transplantation is a very limited option for carefully selected patients assessed in specialised centres.

One myth harms these patients in particular: the belief that holding your breath or blowing up balloons strengthens the lungs and opens up fibrosis. It does not, it achieves nothing, and it can cause dizziness and exhaustion. The exercise that helps is a structured pulmonary rehabilitation programme combining walking and strength training with breathing patterns that reduce the sensation of breathlessness, all under supervision.

Stopping smoking is essential for every patient with fibrosis, and controlling coexisting conditions matters too: acid reflux, sleep apnoea, pulmonary hypertension, and cardiac disease. Recommended vaccinations reduce the infections that can trigger a dangerous acute deterioration. Suitability for any of these options is determined by clinical assessment, imaging, and lung function, never by comparison with another patient's experience.

## Preparation

Before the visit, write a complete occupational history covering every job you have held, how many years, and what you were exposed to — dust, stone, sand, fumes, or paints — because that history can be the key to the diagnosis. List all medications and supplements and any previous radiotherapy to the chest or breast, plus any drug you took long term. Tell the physician if you keep birds or pigeons, if there is damp, mould, or an old air-conditioning unit at home, or feathers in your bedding. Mention autoimmune symptoms such as dry mouth and eyes, skin tightening, joint pain, and colour change in the fingers with cold, and report smoking history in years including waterpipe. Bring all previous scans on a disc together with earlier lung function reports, since comparing old with new establishes how fast the disease is progressing.

## Aftercare

Stick to the plan and to your blood test appointments if you are on antifibrotic medication, because monitoring liver function and tolerance is part of the treatment rather than an optional extra. Stay completely away from smoking and from whichever exposure was identified for you, fix damp and mould at home, and remove birds. Follow your pulmonary rehabilitation programme and keep moving daily within your tolerance, since prolonged sitting weakens muscles and worsens the sensation of breathlessness. If oxygen has been prescribed, use it at the set flow rate and never increase it yourself. Have the recommended vaccinations. \*\*Go to the emergency department immediately if breathlessness worsens over days or hours, which may signal an acute exacerbation, infection, or pulmonary embolism.\*\* \*\*Seek emergency care for sharp chest pain with sudden breathlessness, blue lips, breathlessness that stops you finishing a sentence, or coughing up blood.\*\* Contact your doctor for fever or a change in sputum colour, leg swelling with rapid weight gain, or dizziness and palpitations on exertion.
