## Procedure steps

### 1. Accurate measurement and risk assessment

The maximum diameter is measured by ultrasound or CT angiography, and the shape and position of the aneurysm and its relationship to the arteries arising from it are described. The measurement is compared with any earlier scan to judge the rate of growth, and the heart, lungs and kidneys are assessed to determine what intervention your body can tolerate.

### 2. Medical therapy and imaging surveillance

If the diameter is below the threshold for intervention, a surveillance programme of imaging at regular intervals is arranged, with strict blood-pressure control, cholesterol treatment and complete smoking cessation. No medicine shrinks an aneurysm, but these measures slow its growth and reduce the risk of other cardiac and arterial events.

### 3. Choosing and planning the method of repair

The team compares endovascular and open repair: the length of healthy aorta above the aneurysm, the diameter of the groin arteries and the angles of the route are measured to judge whether a supported graft will fit, and the faster recovery is weighed against the need for permanent imaging follow-up. The difference is explained to you before the decision.

### 4. Carrying out the repair

In endovascular repair the graft is introduced folded through the groin arteries and positioned precisely under X-ray guidance to line the aorta and shield the aneurysm from blood pressure. In open repair the abdomen or chest is opened, flow is temporarily interrupted, and the widened segment is replaced with a synthetic tube sewn to healthy wall above and below it.

### 5. Post-repair monitoring and lifelong follow-up

Kidney function, perfusion of the legs and bowel, and blood pressure are monitored after the procedure, and mobilisation begins early according to the method used. Imaging follow-up is mandatory for life after endovascular repair, to detect a leak around the graft or migration, and it is not cancelled simply because you feel well.

## About this service

An aortic aneurysm is a permanent widening of the largest artery in the body, caused by weakness of its wall; it may involve the segment passing through the abdomen or the part inside the chest. The main danger is not the widening itself but the possibility that the stretched wall tears — a sudden event that threatens life within minutes. Because an aneurysm usually grows without symptoms, many are found incidentally on imaging done for another reason.

Assessment begins with clinical examination and precise measurement of the diameter by ultrasound or CT, together with the shape and position of the aneurysm and its relationship to the arteries arising from it. Not every case needs an operation. A small aneurysm is followed with imaging at regular intervals, alongside tight blood-pressure control, treatment of cholesterol and complete cessation of smoking — smoking is the strongest modifiable factor in arterial disease and is linked to aneurysm growth more than any other factor.

Repair is considered when the diameter reaches the size your doctor defines for your case, when growth accelerates between two scans, or when symptoms appear. There are two routes: endovascular repair, in which a supported graft is delivered through the groin arteries to line the aorta from inside, and open repair, in which the widened segment is replaced with a synthetic tube. The choice depends on the shape of the aorta, the length of healthy vessel available, the condition of the heart, lungs and kidneys, and your age.

The limits must be stated. Repair prevents rupture; it does not cure arterial wall disease elsewhere in the body, and no medicine shrinks an existing aneurysm. Endovascular repair requires lifelong imaging follow-up because the graft can leak or move. The decision is made from examination and imaging — not from reading.

## Preparation

Stop smoking — it is the single most powerful thing you can do for your arterial wall, and the benefit starts before the operation. Bring your blood pressure under control and record daily readings, and continue your blood-pressure and cholesterol medicines as prescribed. Bring a written list of all your medicines and tell the doctor explicitly about blood thinners, antiplatelet drugs and diabetes treatment so they are adjusted by their decision, not yours. Say whether you have ever reacted to contrast or have kidney disease. Complete the requested heart and lung tests, and report any new chest pain, breathlessness or persistent back pain. Keep to the fasting time given, avoid heavy lifting and severe straining until your aneurysm has been assessed, and arrange an escort and help at home for recovery.

## Aftercare

Take your blood-pressure medicines exactly as prescribed and measure and record your pressure at home, because raised pressure is what strains the arterial wall or the graft. Avoid heavy lifting and forceful straining for the period your doctor specifies, and prevent constipation with fluids and fibre so that you do not need to strain. Keep the groin or abdominal wounds clean and dry, and start graded walking early to prevent venous clots. Complete your stop-smoking plan and keep every scheduled scan even when you feel perfectly well — a leak around the graft causes no sensation.

Call an ambulance immediately if: \*\*you develop sudden severe pain in the abdomen, back or chest, or a tearing sensation, with dizziness, fainting, cold sweat or pallor\*\* (possible rupture or dissection — do not wait, do not drive yourself, and do not press or massage the abdomen). Also seek urgent care for: bleeding or a pulsating swelling at the puncture site · fever or pus in a wound · a fall in urine output · \*\*a leg that suddenly becomes painful, swollen and warm\*\* (deep vein thrombosis — do not massage it) · \*\*chest pain or breathlessness\*\* (possible pulmonary embolism — emergency) · cold, pale or severely painful feet, or abdominal pain with blood in the stool.
