## Procedure steps

### 1. Diagnosis and staging

Range of motion is measured in all directions and the phase determined — painful, stiff or recovering — because treatment differs fundamentally between them.

### 2. Excluding other causes and screening predisposing factors

Other causes of shoulder pain such as tendon tear, arthritis or impingement are excluded by examination and imaging, and thyroid and glucose are checked as they affect the course.

### 3. Pain control in the painful phase

Prescribed analgesia and anti-inflammatories with compresses, and gentle movement within pain limits — no vigorous stretching, which aggravates inflammation at this stage.

### 4. Physiotherapy and graded stretching

A structured programme to restore range in all directions with daily home exercises — the backbone of treatment in the stiff phase, extending over months.

### 5. Injection or procedures in resistant cases

An intra-articular injection may reduce inflammation and enable stretching; in resistant cases hydrodilatation or capsular release is discussed, followed by intensive rehabilitation.

## About this service

Frozen shoulder (adhesive capsulitis) is a condition in which the capsule surrounding the joint becomes inflamed and fibrotic, contracting and thickening, so range of motion progressively narrows and lifting, rotating or reaching behind becomes difficult. It most often affects middle age, is commoner in people with diabetes and thyroid disorders, and may follow a period of arm immobilisation after injury or surgery.

What characterises it — and must be understood to accept treatment — is that \*\*its course is long and passes through three phases\*\*: a painful phase in which pain escalates as motion begins to reduce, then a stiff phase where pain relatively eases but movement is at its most restricted, then a phase of gradual recovery of motion. The whole course is usually measured in months, not weeks, and knowing this prevents patients moving between doctors seeking an instant fix.

Treatment differs by phase, and this is the key to outcome: in the painful phase the focus is pain control and gently preserving remaining motion — vigorous exercise here increases inflammation and does harm. In the stiff phase, structured physiotherapy with sustained graded stretching becomes the backbone of treatment. A targeted intra-articular injection may be offered to reduce inflammation and enable stretching, and in resistant cases procedures such as hydrodilatation or capsular release are discussed.

Blood-sugar control is part of treatment, not a side detail, because uncontrolled diabetes prolongs the course. Daily adherence to home exercises is what separates recovering good motion from being left with lasting limitation.

## Preparation

Before the visit note when the pain began and how it evolved, which movements became harder, and whether the arm was immobilised or there was injury or surgery. Tell your physician if you have diabetes or a thyroid disorder and bring your results, since controlling them is part of treatment. Bring any previous shoulder imaging or reports. Ask explicitly: which phase am I in, which exercises are allowed now and which are forbidden, how long is this expected to take, and when will the plan be reassessed — accurate expectations prevent frustration and stop-start treatment.

## Aftercare

Do the home exercises daily — short daily consistency beats long intermittent sessions and is the most important factor in regaining motion. Do not force the shoulder into painful stretching in the painful phase, and progress in the next phase as your therapist directs. Use heat before exercises to ease stretching and cold afterwards if pain increases. Do not keep the arm in a sling for long periods without medical advice, as immobilisation increases stiffness. Control blood sugar seriously. Expect fluctuating rather than linear progress. Contact your physician if pain worsens sharply without cause, motion is suddenly lost, weakness or numbness descends the arm, or the joint becomes hot, swollen and red.
