الأنواع تختلف وتغيّر خطة العلاج: استرواح عفوي أوّلي يحدث بلا مرض رئوي معروف وأكثر ما يُصيب شباباً نحيفين طوال القامة، والتدخين عامل خطر رئيسي فيه؛ واسترواح عفوي ثانوي على خلفية مرض رئوي مزمن؛ واسترواح رضحي بعد كسر ضلع أو طعنة أو حادث، وقد يصحبه تهتّك في نسيج الرئة أو تجمّع دم في الحيّز البلوري؛ واسترواح توتّري وهو حالة طارئة تهدّد الحياة يزداد فيها الضغط داخل الصدر بسرعة فينهار الدوران.

العلاج يتبع الحجم والأعراض وحالة الرئة لا الاسم فقط: كمية هواء صغيرة عند شخص مستقرّ قد تُدار بالمراقبة والأوكسجين وتصوير متكرّر، وحالة أكبر تحتاج شفطاً بالإبرة أو تركيب أنبوب صدري يُصرّف الهواء ويسمح للرئة بالتمدّد. أما التدخّل الجراحي بالتنظير — بإزالة الفقاعات الهوائية وإحداث التصاق بين غشاءي الرئة — فيُطرح عند التكرار أو تسريب هواء مستمرّ أو حالات مهنية خاصة.

خلال وجود الأنبوب تُراقب فقاعات التسريب وتمدّد الرئة بالصور، ويُشجَّع التنفّس العميق والحركة، ويُزال الأنبوب حين يتوقّف التسريب وتتمدّد الرئة، ثم تُؤخذ صورة تأكيد قبل الخروج.

وحدود العلاج يجب أن تكون صريحة: الأنبوب يحلّ النوبة الحالية ولا يمنع تكرارها، وخطر التكرار حقيقي خصوصاً في الأشهر الأولى؛ والالتصاق البلوري أو الجراحة يقلّلان الخطر ولا يُلغيانه. والإقلاع عن التدخين هو أقوى إجراء يخفّض احتمال التكرار وهو بيدك أنت. ولا يُعالج استرواح الصدر بالراحة في البيت ولا بالأعشاب ولا بشدّ الصدر برباط — \*\*وضيق النفس الشديد أو الإغماء أو ازرقاق الشفاه حالة طارئة تُنقل بالإسعاف لا تُنتظر\*\*. ويُؤجَّل السفر بالطائرة لفترة يحدّدها الطبيب، ويُمنع الغوص بأسطوانات عادةً بشكل دائم إلا بقرار متخصص بعد إجراء جراحي نهائي.

## خطوات الإجراء

### 1. التشخيص السريع

يُقيَّم التنفّس والنبض وضغط الدم وتشبّع الأوكسجين أولاً، ويُفحص الصدر، وتُطلب صورة صدر وقد يُستخدم التصوير بالموجات فوق الصوتية أو التصوير المقطعي عند الشكّ أو في حالات الرضح. سرعة هذه الخطوة هي ما يفرق بين حالة تُدار بهدوء وحالة تتدهور.

### 2. تحديد الشدّة وقرار العلاج الأوّلي

يُقرَّر المسار بحسب كمية الهواء وشدّة الأعراض ووجود مرض رئوي مزمن: مراقبة وأوكسجين مع تصوير متكرّر، أو شفط بالإبرة، أو تركيب أنبوب صدري. الاسترواح التوتّري يُعالج فوراً قبل أي تصوير لأنه لا يحتمل التأخير.

### 3. تركيب الأنبوب الصدري ومراقبة التسريب

يُخدَّر الجلد موضعياً ويُدخل أنبوب رقيق بين الأضلاع إلى الحيّز البلوري ويُوصَل بجهاز تصريف بمانع رجوع. تُراقب الفقاعات وتمدّد الرئة يومياً بالصور، وتُعطى مسكّنات، ويُشجَّع التنفّس العميق والمشي مع الأنبوب.

### 4. إزالة الأنبوب والتأكّد من تمدّد الرئة

يُزال الأنبوب حين يتوقّف تسريب الهواء وتتمدّد الرئة على الصورة، ثم تُعاد صورة تأكيد بعد الإزالة قبل الخروج. تُغطّى فتحة الأنبوب، وتُشرح لك علامات عودة الاسترواح وموعد المتابعة والصورة التالية.

### 5. الوقاية من التكرار والجراحة عند الحاجة

يُقيَّم خطر التكرار من نوع الاسترواح والنتائج والتصوير. عند تكرار الحالة أو استمرار التسريب أو وجود فقاعات واضحة يُطرح إجراء بالتنظير يُزيل الفقاعات ويُحدث التصاقاً بلورياً. والإقلاع عن التدخين جزء من الخطة لا نصيحة عابرة.

## Procedure steps

### 1. Rapid diagnosis

Breathing, pulse, blood pressure and oxygen saturation are assessed first, the chest is examined, and a chest X-ray is requested, with ultrasound or CT used when there is doubt or after trauma. The speed of this step is what separates a case managed calmly from one that deteriorates.

### 2. Deciding severity and initial treatment

The route is chosen according to the volume of air, symptom severity and the presence of chronic lung disease: observation with oxygen and repeat imaging, needle aspiration, or chest drain insertion. A tension pneumothorax is treated immediately, before any imaging, because it cannot wait.

### 3. Chest drain insertion and air-leak monitoring

The skin is numbed with local anaesthetic and a slim tube is inserted between the ribs into the pleural space and connected to a drainage system with a one-way seal. Bubbling and lung expansion are checked daily on imaging, pain relief is given, and deep breathing and walking with the drain are encouraged.

### 4. Drain removal and confirming re-expansion

The drain is removed once the air leak has stopped and the lung is expanded on imaging, and a confirmatory image is repeated after removal before discharge. The drain site is dressed, and the signs of recurrence, your follow-up appointment and the next image are explained to you.

### 5. Preventing recurrence and surgery when needed

Recurrence risk is judged from the type of pneumothorax, the course of the episode and the imaging. If it recurs, if the leak persists or if clear blebs are seen, a keyhole procedure is offered to remove the blebs and create pleural adhesion. Stopping smoking is part of the plan, not a passing suggestion.

## التحضير قبل الإجراء

إن كان الألم مفاجئاً وضيق النفس واضحاً فلا تحضّر شيئاً ولا تنتظر: اتصل بالإسعاف أو اذهب إلى أقرب طوارئ. أما إن كان الإجراء مخطّطاً — تركيب أنبوب أو جراحة تنظير — فأعطِ الفريق قائمة كاملة بأدويتك ومكمّلاتك، وأخبرهم بأي نوبة استرواح سابقة وبأي مرض رئوي مزمن أو ربو أو اضطراب تخثّر أو حساسية دوائية. \*\*لا تُوقف مضادات التصفيح أو مضادات التخثّر من تلقاء نفسك؛ توقيت إيقافها قبل تركيب الأنبوب أو الجراحة قرار مشترك بين الفريق الجراحي وطبيب القلب، وينطبق ذلك حتى قبل قلع سنّ أو تنظيف الأسنان.\*\* التزم بتعليمات الصيام إن كان هناك تخدير عام، ورتّب مرافقاً وإقامة، وأخبر الفريق إن كنت تعمل غوّاصاً أو طيّاراً أو في مهنة تتضمّن تغيّر ضغط لأن هذا يغيّر توصيات ما بعد العلاج، وأقلع عن التدخين من اليوم.

## العناية بعد الإجراء

استمرّ في تمارين التنفّس العميق وامشِ يومياً، وخذ المسكّن بانتظام في الأيام الأولى، وحافظ على موضع الأنبوب نظيفاً جافّاً حتى يُغلق تماماً. تجنّب رفع الأثقال والمجهود العنيف لعدة أسابيع بحسب تعليمات الطبيب، ولا تسافر بالطائرة إلا بعد أن يسمح لك بذلك بناءً على صورة متابعة، وتجنّب الغوص بالأسطوانات. \*\*عُد إلى الطوارئ فوراً عند عودة ألم صدر مفاجئ في الجهة نفسها، أو ضيق نفس متزايد، أو تسارع نبض، أو ازرقاق الشفاه، أو إغماء، أو انتفاخ تحت الجلد في الصدر أو الرقبة مع إحساس بفرقعة عند اللمس، أو حرارة مرتفعة مع احمرار وقيح في موضع الأنبوب.\*\* والإقلاع عن التدخين ليس نصيحة عامة هنا بل هو الإجراء الأهمّ لتقليل تكرار الحالة. التزم بموعد المتابعة والصورة التالية حتى إن اختفت الأعراض تماماً.

## About this service

What people call a punctured lung is, in medical terms, a pneumothorax: air leaking into the space between the lung and the chest lining, where it presses on the lung and prevents it from expanding. The typical picture is sudden sharp one-sided chest pain with breathlessness, and the pain often worsens on breathing in.

The type matters because it changes the plan. A primary spontaneous pneumothorax occurs without known lung disease and most often affects tall, slim young people, with smoking as a major risk factor. A secondary spontaneous pneumothorax happens on a background of chronic lung disease. A traumatic pneumothorax follows a rib fracture, a stab wound or an accident, and may be accompanied by contusion of the lung tissue or blood collecting in the pleural space. A tension pneumothorax is a life-threatening emergency in which pressure inside the chest rises rapidly and the circulation collapses.

Treatment follows the size, the symptoms and the state of the lung rather than the label alone. A small volume of air in a stable person may be managed with observation, oxygen and repeat imaging. A larger one needs needle aspiration or a chest drain that evacuates the air and lets the lung re-expand. Keyhole surgery — removing air blebs and creating adhesion between the two pleural layers — is offered for recurrence, for a persistent air leak, or for particular occupations.

While a drain is in place, bubbling and lung expansion are monitored on imaging, deep breathing and mobility are encouraged, and the drain is removed once the leak has stopped and the lung is expanded, with a confirmatory image before discharge.

The limits must be stated plainly. A drain resolves the current episode but does not prevent another, and the recurrence risk is real, particularly in the first months; pleurodesis or surgery reduces that risk without abolishing it. Stopping smoking is the single most powerful step that lowers recurrence, and it is in your hands. A pneumothorax is not treated by resting at home, by herbal remedies or by strapping the chest — \*\*severe breathlessness, fainting or blue lips is an emergency that needs an ambulance, not a wait-and-see approach\*\*. Air travel is deferred for a period your doctor defines, and scuba diving is usually permanently avoided unless a specialist clears it after definitive surgery.

## Preparation

If the pain came on suddenly and you are clearly breathless, do not prepare anything and do not wait: call an ambulance or go to the nearest emergency department. If the procedure is planned — a drain or keyhole surgery — give the team a full list of your medicines and supplements, and tell them about any previous episode, chronic lung disease, asthma, clotting disorder or drug allergy. \*\*Do not stop antiplatelet or anticoagulant medication on your own; when to pause it before a drain or an operation is a joint decision between the surgical team and your cardiologist, and the same holds even before a tooth extraction.\*\* Follow fasting instructions if general anaesthesia is planned, arrange an escort and an inpatient stay, and tell the team if you work as a diver or pilot or in any job involving pressure changes, because that changes the advice afterwards. Stop smoking from today.

## Aftercare

Keep doing deep breathing exercises and walk daily, take pain relief regularly in the first days, and keep the drain site clean and dry until it has closed fully. Avoid heavy lifting and strenuous effort for several weeks as instructed, do not fly until you are cleared on a follow-up image, and avoid scuba diving. \*\*Return to the emergency department immediately if sudden chest pain returns on the same side, if breathlessness increases, if your pulse races, if your lips look blue, if you faint, if the chest or neck swells with a crackling feeling under the skin, or if you develop fever with redness and pus at the drain site.\*\* Stopping smoking is not a general recommendation here — it is the single most important step to reduce recurrence. Keep your follow-up appointment and the next image even if your symptoms have completely gone.
