السبب الأشيع هو انخفاض هرمون الإستروجين الذي يحافظ على سماكة النسيج ورطوبته: بعد انقطاع الطمث، وبعد الولادة وأثناء الرضاعة، وبعد استئصال المبيضين، وبعد العلاج الكيماوي أو الأدوية المضادة للإستروجين في علاج سرطان الثدي. وهناك أسباب أخرى: استخدام الصابون والغسول المعطّر والدوش المهبلي التي تُخرّب البيئة الطبيعية، وبعض الأدوية مثل مضادات الهيستامين، والتدخين، وأمراض المناعة التي تصيب الغدد المفرزة، وأمراض جلد المنطقة التناسلية التي يجب استبعادها لأن علاجها مختلف تماماً.

العلاج يبدأ بخطوات غير هرمونية وغير مكلفة وكثيراً ما تكفي: مزلقات تُستخدم قبل العلاقة لتقليل الاحتكاك، ومرطّبات مهبلية تُستخدم بانتظام عدة مرات أسبوعياً لتحسين رطوبة النسيج على المدى الأطول، وإيقاف كل ما يهيّج المنطقة من صابون معطّر وغسول ودوش داخلي ومناديل معطّرة، والغسل بالماء فقط من الخارج، واستخدام ملابس داخلية قطنية.

إذا كان السبب نقص الإستروجين ولم تكفِ التدابير الأولى، فالعلاج الموضعي بالإستروجين المهبلي فعّال لأنه يعالج النسيج مباشرة بامتصاص جهازي ضئيل، ويحتاج عادة من ستة إلى اثني عشر أسبوعاً لظهور أثره الكامل ثم جرعة صيانة. توجد أيضاً بدائل موصوفة غير إستروجينية لمن لا يناسبها الإستروجين، ويجب أن تُناقَش حالة من لديها تاريخ سرطان ثدي مع طبيبها المعالج قبل البدء. أما أجهزة الليزر والترددات الراديوية المهبلية فأدلّتها لا تزال محدودة ونتائجها متباينة، ولا تُعدّ خط علاج أول ولا بديلاً مثبتاً عن العلاج الموضعي.

وللعلاج حدود صريحة: المزلق يخفّف الاحتكاك وقت العلاقة ولا يعالج ضمور النسيج، والمرطّب لا يعيد الإستروجين، وعلاج الجفاف لا يحلّ وحده ضعف الرغبة الناتج عن الاكتئاب أو الإرهاق أو مشكلة في العلاقة الزوجية أو أثر دواء نفسي، وقد يحتاج الألم المزمن أو تشنّج عضلات قاع الحوض تأهيلاً مخصّصاً. الترشيح لأي علاج يُحدَّد بالفحص السريري لاستبعاد الالتهاب وأمراض جلد المنطقة، لا بالقراءة.

## خطوات الإجراء

### 1. استشارة وتاريخ مفصّل

يُسأل عن بداية الأعراض وشدّتها وعلاقتها بالعلاقة الزوجية والتبوّل، وعن انقطاع الطمث أو الرضاعة أو استئصال المبيضين، وعن العلاج الكيماوي أو أدوية سرطان الثدي، وعن الصابون والغسول والدوش المستخدم، والأدوية والتدخين وجفاف العين والفم.

### 2. الفحص واستبعاد الأسباب الأخرى

يُجرى فحص نسائي لطيف لتوصيف رقّة الأنسجة وشحوبها وفقدان مرونتها، ولاستبعاد الالتهاب الفطري أو البكتيري وأمراض جلد المنطقة التناسلية والتقرّحات والالتصاقات. تُؤخذ مسحة عند الشك، ويُقيَّم قاع الحوض إن كان هناك ألم أو تشنّج.

### 3. التدابير الأولى غير الهرمونية

تُوصف مزلقات للاستخدام قبل العلاقة ومرطّبات مهبلية للاستخدام المنتظم، ويُوقف الصابون المعطّر والغسول والدوش الداخلي والمناديل المعطّرة، ويُنصَح بالغسل بالماء من الخارج فقط وبملابس داخلية قطنية، ويُشجَّع الإقلاع عن التدخين.

### 4. العلاج الموضعي الموصوف

عند نقص الإستروجين وعدم كفاية التدابير الأولى يُوصف إستروجين مهبلي موضعي بامتصاص جهازي ضئيل، أو بديل موصوف غير إستروجيني عند وجود مانع، ويُبنى القرار على التاريخ الطبي وخصوصاً سرطان الثدي بالتنسيق مع الطبيب المعالج. يُشرَح أن الأثر الكامل يحتاج أسابيع.

### 5. متابعة وعلاج ما يرافقه

تُحدَّد مراجعة بعد ستة إلى اثني عشر أسبوعاً لتقييم الاستجابة وضبط جرعة الصيانة. يُعالَج ما يرافق الحالة: تشنّج قاع الحوض بتأهيل مخصّص، وضعف الرغبة بتقييم أسبابه النفسية والدوائية والعلائقية، والتهابات المسالك المتكرّرة بخطة وقائية.

## Procedure steps

### 1. Consultation and detailed history

You are asked about when symptoms began, their severity, and their relation to intercourse and passing urine, about menopause, breastfeeding or removal of the ovaries, about chemotherapy or breast cancer medication, about the soaps, washes and douches you use, and about medications, smoking and any dryness of the eyes and mouth.

### 2. Examination and excluding other causes

A gentle gynaecological examination describes thinning, pallor and loss of elasticity of the tissue, and excludes fungal or bacterial infection, genital skin disease, ulceration and adhesions. A swab is taken when infection is suspected, and the pelvic floor is assessed if there is pain or spasm.

### 3. First-line non-hormonal measures

Lubricants are advised for use before intercourse and vaginal moisturisers for regular use, perfumed soap, washes, internal douching and scented wipes are stopped, washing the outside with water only and wearing cotton underwear are recommended, and stopping smoking is encouraged.

### 4. Prescribed local treatment

Where oestrogen is low and first measures are insufficient, local vaginal oestrogen with very low systemic absorption is prescribed, or a prescribed non-oestrogen alternative when there is a contraindication. The decision rests on medical history, especially breast cancer, in coordination with the treating specialist, and it is explained that full effect takes weeks.

### 5. Follow-up and treating what accompanies it

A review is set after six to twelve weeks to assess response and adjust the maintenance dose. Accompanying problems are addressed: pelvic floor spasm with dedicated rehabilitation, low desire by assessing psychological, medication-related and relationship causes, and recurrent urinary infections with a preventive plan.

## العناية بعد الإجراء

استخدمي المرطّب المهبلي بانتظام وليس عند الأعراض فقط، والمزلق قبل العلاقة بكمية كافية، واختاري منتجات بسيطة بلا عطور ولا كحول ولا مواد مهيّجة. اغسلي المنطقة الخارجية بالماء فقط ودون دوش داخلي، والبسي قطناً، وتجنّبي الغسول المعطّر والمناديل المعطّرة والصابون العادي. إن وُصف لك علاج موضعي فالتزمي بجرعة البداية ثم جرعة الصيانة كما حُدِّد لك، ولا تتوقّعي نتيجة كاملة قبل ستة إلى اثني عشر أسبوعاً، ولا توقفيه من تلقاء نفسك عند التحسّن لأن الأعراض تعود عادة بعد التوقّف. الجماع المنتظم أو استخدام موسّعات موصوفة يساعد على حفظ مرونة النسيج، والإقلاع عن التدخين يفيد.

## About this service

Vaginal dryness is a loss of the normal moisture of the vaginal tissue, along with thinning and loss of elasticity. It causes burning, itching and tightness, pain or light bleeding during intercourse, and sometimes urinary urgency or repeated urinary infections. The condition is very common and genuinely distressing, yet it is often left unmentioned out of embarrassment, and in most cases it responds to simple steps once the cause is identified.

The commonest cause is a fall in oestrogen, the hormone that keeps the tissue thick and moist: after menopause, after childbirth and during breastfeeding, after removal of the ovaries, and after chemotherapy or anti-oestrogen medication used in breast cancer treatment. Other causes matter too: soaps, perfumed washes and vaginal douching that damage the natural environment, some medications such as antihistamines, smoking, autoimmune conditions affecting secretory glands, and genital skin conditions that must be excluded because their treatment is entirely different.

Treatment begins with non-hormonal, inexpensive measures that are often enough on their own: lubricants used before intercourse to reduce friction, vaginal moisturisers used regularly several times a week to improve tissue hydration over the longer term, stopping everything that irritates the area including perfumed soap, washes, internal douching and scented wipes, washing the outside with water only, and wearing cotton underwear.

Where the cause is low oestrogen and first measures are not enough, local vaginal oestrogen is effective because it treats the tissue directly with very little systemic absorption. It usually takes six to twelve weeks for its full effect, followed by a maintenance dose. Prescribed non-oestrogen alternatives also exist for women in whom oestrogen is unsuitable, and any woman with a history of breast cancer should discuss treatment with her treating specialist before starting. As for vaginal laser and radiofrequency devices, the evidence remains limited and results inconsistent; they are not a first-line treatment and not a proven replacement for local therapy.

The limits are explicit. A lubricant reduces friction during intercourse but does not treat tissue atrophy, and a moisturiser does not restore oestrogen. Treating dryness alone does not resolve low desire caused by depression, exhaustion, a relationship difficulty or the side effect of a psychiatric medication, and chronic pain or pelvic floor muscle spasm may need dedicated rehabilitation. Whether a treatment suits you is decided by clinical examination, which also excludes infection and genital skin disease, not by reading.

## Preparation

Write your symptoms down clearly before the appointment: whether the dryness is constant or appears only with intercourse, how long it has lasted, and whether it comes with burning, itching, discharge, bleeding after intercourse or urinary symptoms. Note your hormonal situation: the date of your last period, or when menopause occurred, or childbirth and breastfeeding, or removal of the ovaries. Tell the clinician about all your medications, including breast cancer treatment, chemotherapy, antihistamines and psychiatric medicines, and about chronic conditions and any dryness of the eyes or mouth. Describe in detail what you use in the area: the soap or wash, vaginal douching, scented wipes, any cream, oil or herbal mixture. Avoid douching and unprescribed creams before the examination because they distort its findings, and never use a friend's or relative's prescription.

## Aftercare

Use the vaginal moisturiser regularly rather than only when symptoms flare, and use enough lubricant before intercourse, choosing simple products free of perfume, alcohol and irritants. Wash the outside with water only and never douche internally, wear cotton underwear, and avoid perfumed washes, scented wipes and ordinary soap. If a local treatment has been prescribed, follow the starting dose and then the maintenance dose exactly as set out, do not expect the full effect before six to twelve weeks, and do not stop it on your own once you improve, because symptoms usually return after stopping. Regular intercourse or prescribed dilators help maintain tissue elasticity, and stopping smoking helps.

Seek care immediately if you have: \*\*any vaginal bleeding after menopause or repeated bleeding after intercourse, offensive or blood-stained discharge, ulceration, splitting, a change in the colour of the skin or adhesions, all of which need examination to exclude genital skin disease and tumours, severe pain that prevents examination or intercourse, fever with pelvic pain, or burning and urinary urgency with blood\*\*. After childbirth, seek care immediately for \*\*heavy bleeding, fever, offensive discharge, or increasing pain at the site of your stitches\*\*.
