## Procedure steps

### 1. Urgent assessment and excluding pulmonary embolism

The circumference of both legs is measured and swelling, pain, warmth, colour and pulses examined, key questions are asked about travel, surgery, pregnancy, hormones and cancer, and any symptom suggesting a clot has reached the lungs is excluded first because it takes priority over everything else.

### 2. Venous Doppler and blood tests

A deep vein Doppler shows the clot's site and extent and whether it reaches the thigh or pelvis, and blood tests including kidney and liver function, full blood count and clotting studies are requested to determine the safe option, with the clot-breakdown test used where appropriate.

### 3. Starting anticoagulation and choosing the care setting

Anticoagulation is usually started the same day, with the agent chosen according to kidney function, pregnancy, cancer, bleeding risk and other medicines. It is then decided whether treatment can be given at home with follow-up or admission is required if the clot is extensive or the patient unstable.

### 4. Compression, mobilisation and symptom relief

Compression stockings of the correct size and grade are prescribed to ease swelling and heaviness, early walking and elevating the leg when seated are encouraged instead of prolonged bed rest, and safe analgesia that does not add to bleeding risk is explained.

### 5. Setting duration, seeking the cause and preventing recurrence

Duration is set according to the cause: a clot after surgery or travel is treated differently from an unprovoked one. An underlying cause is investigated where indicated, and a prevention plan is agreed for high-risk situations such as future surgery, long-haul travel and pregnancy.

## About this service

A blood clot in the leg — medically deep vein thrombosis — is the formation of a thrombus inside a deep vein in the calf, thigh or pelvis that obstructs the return of blood from the limb to the heart. Treatment has three aims: stopping the clot extending, preventing part of it travelling to the lungs, and reducing the chance of recurrence while limiting its long-term effects on the leg.

It usually affects one leg: swelling that makes it visibly different from the other, pain or tightness in the calf that worsens on walking or with pressure, warmth, a red or bluish change in colour, and prominent surface veins. Risk rises with prolonged immobility such as long-haul travel or bed rest after surgery or a plaster cast, after major operations and particularly orthopaedic surgery, in pregnancy and the weeks after delivery, with hormones and contraceptive pills, with cancer, obesity, smoking, acute illness, and with an inherited clotting tendency or a previous clot in you or your family.

Diagnosis is never based on appearance alone. The clinician combines an assessment of clinical probability with a venous Doppler scan, and uses the blood test for clot-breakdown products in selected situations. The mainstay of treatment is anticoagulation, with the agent and duration prescribed individually — usually months, and longer if the cause persists or the clot has recurred — together with compression stockings to ease swelling, and early movement and walking once treatment has begun rather than prolonged bed rest. In selected cases catheter-directed clot dissolution or clot removal is considered, and rarely a filter is placed in the vena cava when anticoagulation is contraindicated.

Do not massage a swollen leg, do not apply hot compresses, and do not resort to cupping, traditional massage, herbal remedies or tight bandaging on your own initiative — massaging a vein containing a clot is a dangerous thing to do. The limits of treatment must also be understood: anticoagulants stop the clot enlarging and recurring and allow the body to dissolve it gradually; they do not dissolve it immediately. Stockings relieve symptoms and do not dissolve the clot. And chronic swelling, heaviness and skin discolouration can persist despite entirely correct treatment, a condition known as post-thrombotic syndrome.

\*\*The most dangerous complication is the clot travelling to the lungs: sudden breathlessness, chest pain that worsens on breathing in, a racing pulse, coughing blood, dizziness or fainting — this is an immediate emergency that cannot wait for an appointment.\*\* Conversely, not every swollen leg is a clot; it may be cellulitis, chronic venous insufficiency, lymphatic swelling, or oedema from the heart, kidneys, liver or a medicine. That distinction is made by clinical examination and Doppler, not by reading, which is why you must never start or stop any blood thinner on your own.

## Preparation

If the swelling or pain started recently, go to the emergency department or arrange urgent same-day assessment rather than waiting for a clinic appointment. Do not start any blood thinner, aspirin or anti-inflammatory painkiller on your own before assessment, and do not massage the leg. Bring a list of your medicines and supplements, and state exactly when symptoms began. Tell the doctor about any long-haul travel, surgery, fracture, plaster cast or bed rest in recent weeks, about pregnancy or the post-delivery period, hormone or contraceptive use, any cancer or chemotherapy, any previous clot in you or your family, and any history of bleeding, stomach ulcer, brain haemorrhage, liver or kidney disease, as all of these determine the safe option. Bring previous reports, blood results and Doppler images if you have them, do not fast unless asked, and mention any contrast allergy or kidney disease.

## Aftercare

Take your anticoagulant precisely and at the same time each day, never stop it or change the amount yourself, and do not stop because the swelling has gone — the duration is set by your doctor. Tell every doctor, dentist and pharmacist that you are on an anticoagulant before any procedure or new medicine, and avoid anti-inflammatory painkillers and herbal products without advice. Wear the compression stockings as prescribed, walk daily and elevate the leg when sitting, and avoid prolonged standing and contact sports. On long journeys move every one to two hours and drink water. \*\*Go to emergency immediately for breathlessness, chest pain, palpitations, coughing blood or fainting; for bleeding that will not stop, a prolonged nosebleed, vomiting blood, black stools or red urine; for severe headache, confusion or weakness after a knock to the head; or for rapidly increasing leg swelling with pallor, coldness and severe pain.\*\* See your doctor before planning pregnancy, at the start of a new pregnancy, or before any surgery, because the treatment plan will need adjusting.
