## Procedure steps

### 1. History and identifying the pattern

The clinician asks when symptoms occur and in which season, how they relate to home, work, a pet or cleaning, about snoring, sleep and sense of smell, about asthma, eczema and eye allergy, about family history, and about every treatment you have tried and its result.

### 2. Nasal examination and endoscopy when needed

The inside of the nose is examined to assess the lining, turbinates, septum and secretions, and to look for polyps or one-sided obstruction; a thin nasal endoscope may be used in the clinic, a quick examination performed with a local anaesthetic spray.

### 3. Allergy testing and its interpretation

Where indicated, skin prick testing is performed for selected inhalant allergens chosen from your history, or specific IgE is measured; antihistamines are stopped for a few days before pricking. The result is then matched to your symptoms, and a positive test without symptoms is not treated.

### 4. A stepped treatment plan

A written plan starts with exposure reduction and saline irrigation, then prescribed medication according to severity and pattern, together with teaching correct spray technique — angled slightly outwards and away from the septum — and the expected duration of use.

### 5. Reviewing response and considering immunotherapy

Response is reviewed after several weeks; if symptoms remain disabling despite adherence, span more than one season, or come with asthma, immunotherapy against a confirmed allergen is considered, with a clear explanation that it runs for years and that early doses are given under observation.

## About this service

Treatment of allergic rhinitis addresses immune-mediated inflammation of the nasal lining triggered by inhaled allergens, producing a recurring pattern of symptoms: bouts of sneezing, watery discharge, itching of the nose and throat, blockage that alternates between the two sides, post-nasal drip with throat clearing, and itchy watery eyes. The commonest triggers in Jordan are house dust and dust mites, tree and grass pollens in spring and autumn, mould spores in damp rooms, and cat and dog dander, while cigarette and waterpipe smoke and occupational dust act as irritants that make everything worse.

Diagnosis combines the history and its seasonal or persistent pattern with examination of the nose, sometimes completed by simple nasal endoscopy. Tests — skin prick testing or measuring specific IgE in blood — are interpreted in the context of your symptoms and never alone. The rule most often missed is this: \*\*a positive test without symptoms on exposure is not an allergy, it is silent sensitisation that needs no treatment and justifies no elimination\*\*. Ordering a broad test panel and then removing everything that shows up is harmful, and it is most harmful in children, because it strips essential foods from the diet and threatens growth for no reason.

Treatment works at several levels. Exposure reduction uses realistic measures: protective bedding covers, hot washing, fewer carpets and fabric toys in the bedroom, closing windows at peak pollen, controlling damp, and no smoking indoors. Saline nasal irrigation is simple, safe and useful including for children and in pregnancy. Prescribed medicines follow, such as intranasal corticosteroid sprays and antihistamines chosen according to the pattern of your symptoms. For suitable patients, allergen immunotherapy delivers gradually increasing doses of the trigger under the tongue or by injection; it is the only option that may change the course of the disease, but it runs for years and requires commitment and supervision.

The limits must be equally clear. An intranasal steroid spray does not work from the first dose; it needs regular use over several days to two weeks, and stopping it at the first improvement is the commonest reason symptoms return. Treatment controls allergy, it does not cure it. Fast decongestant sprays used for more than a few days cause rebound congestion and dependence on the spray. \*\*Nasal surgery does not cure allergy: it corrects a deviated septum, enlarged turbinates or polyps, and it may improve breathing, but the underlying allergy remains and still needs treatment.\*\* Whether any step suits you, particularly immunotherapy, is decided by clinical assessment rather than by reading.

## Preparation

Keep a two-week symptom diary before your visit: when symptoms occur, how severe they are, where they worsen (home, bedroom, workplace, outdoors, after cleaning or near a pet), and whether they disappear when you travel. Tell the clinician about every spray, tablet or drop you have used and for how long, especially fast decongestant sprays if you have used them for a long period. Mention asthma, eczema, any food or drug allergy, any previous severe reaction, and your other medicines — particularly blood pressure and cardiac drugs including beta blockers, because they influence decisions about allergy testing and immunotherapy. If skin prick testing is planned, ask in advance about stopping antihistamines for a few days, and do not stop any other medicine on your own. Bring previous sinus imaging or reports if you have them, tell the clinician if you are pregnant or breastfeeding, and avoid applying cream or moisturiser to your forearms on the day of testing.

## Aftercare

Use the prescribed spray daily through your season and do not stop at the first good day, because the full effect needs days to two weeks. Use correct technique: clear the nose first, tilt the head slightly forward, direct the spray outwards away from the septum, and do not sniff hard. Continue saline irrigation prepared with sterile or boiled and cooled water. Do not continue a decongestant spray beyond the few days your doctor specified. Wash bedding weekly in hot water, and keep the home completely free of cigarette and waterpipe smoke. \*\*Go to emergency immediately for a severe allergic reaction: swelling of the lip, tongue or throat, breathlessness or wheeze, or a widespread rash with dizziness, fainting or vomiting — especially after an immunotherapy dose or a new medicine.\*\* \*\*Arrange an early review for bleeding from one nostril, persistent one-sided blockage, loss of smell that does not return, facial pain with fever and purulent discharge, severe headache or visual change, or if wheeze and night cough appear, suggesting the lower airways are involved.\*\*
