التشخيص سريري أساساً: توزيع الطفح على الجسم وشكله ومدّته، وعلاقته بالعمل والهوايات والمستحضرات ومنتجات التنظيف، والمواسم، والأدوية، والأطعمة، والتاريخ الشخصي والعائلي للربو وحساسية الأنف. قد تُضاف عند الحاجة اختبارات لصق للتحسّس التماسي، أو اختبارات جلدية أو تحاليل نوعية في حالات مختارة. من المهم معرفة أنّ \*\*تحليل الحساسية الشامل لا يشخّص سبب الطفح وحده\*\*، وقد يُظهر إيجابيات لا معنى سريرياً لها، فيُقرأ دائماً مع القصة والفحص، ولا يُبنى عليه حرمان غذائي واسع بلا دليل.

العلاج ثلاثي: إبعاد المحرّض بعد تحديده، وترميم حاجز الجلد بترطيب مكثّف يومي بمرطّبات بسيطة بلا عطور — وهذا هو حجر الأساس لا خطوة ثانوية — ثم أدوية موضعية مضادة للالتهاب أو مضادات هيستامين أو علاجات أخرى بحسب الشدّة والموضع والعمر. تُضاف إجراءات عملية: ماء فاتر لا ساخن في الاستحمام، صابون لطيف، تقليم الأظافر لتقليل أذى الحكّ، أقمشة قطنية، وتجنّب التعرّق المفرط والملامسات المتكرّرة للمنظّفات.

حدود العلاج يجب أن تكون واضحة: هذه الأمراض في كثير من الحالات \*\*تُضبط ولا تُستأصل\*\*، فالهدف فترات صفاء طويلة وسيطرة سريعة على الانتكاسات. والمرطّب لا يعالج التهاباً نشطاً وحده، والدواء الموضعي لا يعمل بلا ترطيب. ولا تستعمل كريمات كورتيزون قوية على الوجه أو لفترات طويلة بلا إشراف، فسوء استخدامها يُرقّق الجلد ويُوسّع الشعيرات ويُفاقم مشاكل الوجه؛ وكذلك \*\*الكريمات المخلوطة أو المجهولة المصدر التي تُوصف عبر الإنترنت أو تُخلط في غير مكانها خطر حقيقي\*\*. راجع فوراً عند تورّم الشفتين أو اللسان أو ضيق التنفّس، فهذه علامات تفاعل شديد لا يُدار بالكريمات. الترشيح لأي علاج يُحدَّد بالفحص السريري لا بالقراءة.

## خطوات الإجراء

### 1. القصة السريرية وتحديد نمط الطفح

يسأل الطبيب عن بداية الطفح ومدّته وشكله وتوزيعه، وهل ينتقل ويزول كل بقعة في ساعات كما في الشرى أو يبقى ويتقشّر كما في الأكزيما، وعن عملك ومستحضراتك ومنظّفاتك وحلياتك وصبغة الشعر، وعن الأدوية والأطعمة، والربو وحساسية الأنف عندك وعند عائلتك، وعن سفر أو حشرات أو حيوان منزلي.

### 2. الفحص الجلدي الكامل

يُفحص الجلد كاملاً لا المنطقة المشتكاة فقط، بما يشمل الثنايا وفروة الرأس والأظافر واليدين، لأن التوزيع يميّز الأكزيما من التماسي من الشرى ومن أمراض أخرى تُشبه الحساسية مثل الفطريات والصدفية والجرب. تُلاحظ آثار الحكّ والعدوى الثانوية، وتُصوَّر الحالة عند الحاجة للمقارنة لاحقاً.

### 3. الاختبارات عند وجود مؤشّر

عند الشكّ بتحسّس تماسي تُجرى اختبارات اللصق بمواد قياسية تُقرأ بعد يومين إلى ثلاثة أيام. في حالات مختارة تُطلب اختبارات جلدية أو تحاليل نوعية أو خزعة جلدية صغيرة أو فحص فطري. لا تُطلب قائمة تحاليل واسعة بلا مؤشّر، لأن الإيجابيات غير المرتبطة سريرياً تؤدّي إلى حرمان وقلق بلا فائدة.

### 4. إبعاد المحرّض وترميم حاجز الجلد

تُوضع قائمة عملية بما يجب تجنّبه في المنزل والعمل، مع بدائل واقعية بلا عطور. ويُبنى روتين ترطيب يومي مكثّف بمرطّب بسيط يُوضع بعد الاستحمام على جلد رطب، مع ماء فاتر وصابون لطيف ومدّة استحمام قصيرة. تقليم الأظافر وأقمشة قطنية وتقليل التعرّق خطوات صغيرة أثرها كبير على الحكّة.

### 5. الأدوية وخطة الانتكاسات والمتابعة

تُوصف أدوية موضعية مضادة للالتهاب بقوة تناسب الموضع والعمر ولمدّة محدّدة، ومضادات هيستامين للحكّة والشرى، وعلاجات أوسع في الحالات الشديدة أو المزمنة بقرار طبيب الجلد. تُشرَح خطة مكتوبة للانتكاسة: ماذا تبدأ ومتى ومتى تتوقّف ومتى تراجع، وتُحدَّد مواعيد متابعة لتقييم السيطرة.

## Procedure steps

### 1. History and identifying the rash pattern

The doctor asks when the rash began, how long it lasts, its shape and distribution, whether individual lesions move and fade within hours as in urticaria or persist and scale as in eczema, and about your occupation, cosmetics, detergents, jewellery and hair dye, plus medicines, foods, personal and family asthma or hay fever, recent travel, insects and pets.

### 2. Full skin examination

The whole skin is examined rather than only the area you complain about, including folds, scalp, nails and hands, because distribution distinguishes eczema from contact dermatitis and urticaria, and from conditions that mimic allergy such as fungal infection, psoriasis and scabies. Signs of scratching and secondary infection are noted, and photographs may be taken for later comparison.

### 3. Testing when there is an indication

Where contact allergy is suspected, patch tests with standard substances are applied and read after two to three days. In selected cases skin prick tests, specific blood tests, a small skin biopsy or fungal microscopy are requested. A broad panel is not ordered without an indication, since clinically irrelevant positives lead to needless restriction and anxiety.

### 4. Trigger avoidance and barrier repair

A practical avoidance list is drawn up for home and work, with realistic fragrance-free alternatives. A daily intensive moisturising routine is built around a simple emollient applied to damp skin after bathing, with lukewarm water, a gentle cleanser and short showers. Short nails, cotton fabrics and less sweating are small steps with a large effect on itch.

### 5. Medication, flare plan and follow-up

Topical anti-inflammatory treatments are prescribed at a potency suited to the site and age and for a defined period, antihistamines are used for itch and urticaria, and broader therapies are considered by a dermatologist in severe or chronic disease. A written flare plan explains what to start, when, when to stop and when to return, with follow-up appointments to assess control.

## التحضير قبل الإجراء

صوّر الطفح بهاتفك في أوقات مختلفة قبل الموعد، لأن الحالة قد تهدأ يوم الزيارة والصور تحمل معلومة أساسية عن الشكل والتوزيع. أحضر عُلَب كل ما يلامس جلدك: المرطّبات والكريمات والصابون والشامبو والعطور وصبغة الشعر ومواد التنظيف والقفازات، أو صوّر قوائم مكوّناتها. اكتب متى يزيد الطفح ومتى يخفّ، وعلاقته بالعمل والرياضة والسفر والحلي والأطعمة والأدوية. أخبر الطبيب بكل ما استخدمته على الجلد سابقاً وخاصة كريمات الكورتيزون والكريمات المخلوطة أو المجهولة المصدر ومدّة استخدامها، وبأي حساسية دوائية، وهل لديك ربو أو حساسية أنف، وإن كنتِ حاملاً أو مرضعاً. إن كانت اختبارات اللصق محتملة، اسأل قبلها عن إيقاف مضادات الهيستامين أو الكورتيزون وتجنّب تعريض الظهر للشمس.

## العناية بعد الإجراء

رطّب الجلد يومياً وباستمرار حتى في فترات الصفاء، فالترطيب هو ما يقلّل عدد الانتكاسات لا مجرّد مسكّن للحكّة، وضعه على جلد رطب بعد استحمام قصير بماء فاتر وصابون لطيف. استخدم الدواء الموضعي بالقوّة والمدّة الموصوفة ولا تمدّده أسابيع بلا مراجعة، ولا تضع كريمات قوية على الوجه من نفسك، ولا تستعمل كريمات مخلوطة أو مجهولة المصدر مهما مُدحت. قلّم أظافرك وارتدِ قطناً وابتعد عن المحرّض الذي حُدّد لك، ولا تُحرم نفسك من أطعمة كثيرة بلا دليل واختبار موجّه.

راجع الطوارئ فوراً عند \*\*تورّم الشفتين أو اللسان أو الحلق أو تغيّر الصوت أو ضيق النفس أو دوخة شديدة\*\*، وهذه علامات تفاعل شديد. وراجع الطبيب سريعاً عند \*\*حرارة مع طفح مؤلم سريع الانتشار أو تقرّحات أو تقيّح أو قشور صفراء\*\*، أو \*\*إصابة العين أو الفم أو المناطق التناسلية\*\*، أو طفح بعد بدء دواء جديد، أو حكّة تمنع النوم رغم العلاج، أو طفح لا يتحسّن بعد الفترة التي حدّدها لك الطبيب.

## About this service

Treatment for skin allergy is a diagnostic and therapeutic pathway for allergic and inflammatory skin reactions, most commonly three patterns: atopic eczema, which affects dry sensitive skin with itching, redness and scaling in the body folds; contact dermatitis, which appears where a specific substance touches the skin, such as nickel, fragrances, cleaning products or hair dye; and urticaria (hives), which appears as itchy raised weals that move around, each individual weal fading within a day. Identifying the pattern is half the treatment, because each has a different cause and course.

Diagnosis is primarily clinical: the distribution, shape and duration of the rash, its relationship to work, hobbies, cosmetics and cleaning products, seasons, medicines and foods, and the personal and family history of asthma and allergic rhinitis. Patch testing for contact allergy may be added when indicated, or skin prick tests and specific blood tests in selected cases. It is important to understand that \*\*a broad allergy blood panel does not by itself diagnose the cause of a rash\*\*: it can show positives with no clinical meaning, so it is always read alongside the history and examination and never used to justify wide dietary restriction without evidence.

Treatment has three strands: removing the trigger once identified; repairing the skin barrier with intensive daily moisturising using simple fragrance-free emollients, which is the foundation rather than an optional extra; and anti-inflammatory topical medicines, antihistamines or other treatments according to severity, site and age. Practical measures are added: lukewarm rather than hot showers, gentle cleansers, short fingernails to limit the damage of scratching, cotton fabrics, and avoiding excessive sweating and repeated contact with detergents.

The limits should be stated plainly: in many cases these conditions are \*\*controlled rather than cured\*\*, and the goal is long clear periods with rapid control of flares. A moisturiser alone will not treat active inflammation, and a topical medicine will not work without moisturising alongside it. Do not use potent steroid creams on the face or for long periods without supervision, because misuse thins the skin, dilates fine vessels and worsens facial problems; likewise \*\*mixed or unlabelled creams recommended online or blended outside a proper pharmacy are a genuine hazard\*\*. Seek immediate care for swelling of the lips or tongue or any breathing difficulty, because these signal a severe reaction that creams cannot manage. Suitability for any treatment is decided by clinical examination, not by reading.

## Preparation

Photograph the rash at different times before your appointment, because it may settle on the day of the visit while the images carry essential information about shape and distribution. Bring the actual containers of everything that touches your skin: moisturisers, creams, soap, shampoo, perfume, hair dye, cleaning products and gloves, or photograph their ingredient lists. Note when the rash worsens and eases and how it relates to work, sport, travel, jewellery, foods and medicines. Tell the doctor about everything you have already applied, especially steroid creams and any mixed or unlabelled cream, and for how long, along with drug allergies, whether you have asthma or hay fever, and whether you are pregnant or breastfeeding. If patch testing is likely, ask in advance about pausing antihistamines or steroids and keeping your back out of the sun.

## Aftercare

Moisturise every day, continuously, even during clear periods: emollients reduce the number of flares rather than merely soothing itch, and they work best applied to damp skin after a short lukewarm shower with a gentle cleanser. Use topical medication at the prescribed potency and duration, do not extend it for weeks without review, never apply potent creams to your face on your own initiative, and do not use mixed or unlabelled creams however highly they are recommended. Keep nails short, wear cotton, avoid the trigger identified for you, and do not eliminate large numbers of foods without evidence and directed testing.

Go to the emergency department immediately with \*\*swelling of the lips, tongue or throat, a change in your voice, breathing difficulty or severe dizziness\*\*, which indicate a severe reaction. See a doctor promptly with \*\*fever alongside a painful rapidly spreading rash, blistering, pus or yellow crusting\*\*, \*\*involvement of the eyes, mouth or genital area\*\*, a rash that started after a new medicine, itching that prevents sleep despite treatment, or a rash that has not improved within the period your doctor specified.
