## Procedure steps

### 1. A precise clinical history

The physician takes a detailed food history: what was eaten, how much, how long before symptoms began, exactly what those symptoms were and how long they lasted, whether they recurred with wheat alone or with other foods, and whether exertion or a painkiller was involved. This history is the foundation, and tests are interpreted in its light rather than in isolation.

### 2. Targeted testing

Skin prick testing or specific IgE to wheat proteins is performed, requested selectively according to what the history suggests rather than as a broad random panel. A positive result means sensitisation, while a negative result with a strong history does not end the investigation. Component-resolved testing may be added when the exercise-related form is suspected.

### 3. Excluding coeliac disease and intolerance

Before any permanent exclusion, coeliac disease is ruled out with immune blood tests and sometimes an intestinal biopsy, and you must remain on a gluten-containing diet until those samples are taken, since early removal masks the diagnosis. Intolerance states and functional bowel disorders that mimic the picture are assessed at the same time.

### 4. A targeted avoidance plan with dietetic support

If allergy is confirmed, the core treatment is avoiding the proven trigger only, not removing everything as a precaution. A dietitian trains you to read product labels and 'may contain' statements, to prevent cross-contamination in the kitchen, and to use substitutes that preserve iron, fibre and B vitamins so no nutritional deficiency develops, particularly in children.

### 5. A written emergency plan and reassessment

A clear written emergency plan is prepared: the signs of a severe reaction, what is used first, and when to call for an ambulance. It is explained to your family and to the school or workplace, with hands-on training on the auto-injector if one is prescribed. Reassessment is scheduled periodically, since many childhood food allergies ease with growth and the food may be reintroduced under a medically supervised challenge.

## About this service

Wheat allergy is an immune reaction to wheat proteins, and treating it begins with a precise diagnosis that separates it from two conditions with which it is frequently confused: coeliac disease, an autoimmune reaction to gluten that damages the small intestine, and non-coeliac gluten sensitivity or intolerance. All three appear to improve when wheat is removed, yet the diagnosis, the follow-up and the level of risk differ completely.

Immediate immune wheat allergy usually appears within minutes to two hours of eating: rash, itching and hives, swelling of a lip or eyelid, vomiting or abdominal pain, coughing or wheezing, and in some cases a severe whole-body reaction. There is also a distinctive form in which the reaction occurs only when wheat is combined with physical exertion, a painkiller or alcohol, so the event follows exercise and the effort rather than the food is wrongly blamed.

\*\*A severe allergic reaction is a medical emergency. If there is swelling of the lip, tongue or throat, breathing difficulty, wheeze or a hoarse voice, dizziness, pallor or fainting, or a rapidly spreading rash with sudden vomiting, call emergency services immediately and do not wait to see whether it settles on its own, because an apparent improvement can be followed by a second wave.\*\* For anyone prescribed an adrenaline auto-injector, early use is the life-saving step and must never be delayed while waiting for oral antihistamines to work.

The limits of testing are fundamental. Skin prick tests and specific IgE blood tests demonstrate sensitisation, not clinical allergy, and \*\*a positive test without symptoms on eating the food is not an allergy\*\* and does not justify lifelong avoidance. The decision combines the clinical history with test results and may require a supervised oral food challenge in hospital. Commercial food IgG panels and food intolerance profiles are not reliable. Do not remove gluten before coeliac testing, as doing so invalidates the result. All of this is decided by clinical assessment, not by reading a page.

## Preparation

Before the visit, write a precise log of every episode: the date, the complete food with its ingredients, the amount eaten, the interval before symptoms began, the symptoms themselves in detail, what you did and what helped, and whether you were exercising or had taken a painkiller or alcohol beforehand. Bring photographs of suspect product packaging with their ingredient lists, and photographs of the rash if you took any. Do not remove gluten before the visit if you have not yet been tested for coeliac disease. Ask your physician how long before skin prick testing to stop antihistamines, and do not stop asthma or cardiac medication on your own. Report all your medications, especially beta blockers, any previous drug allergy and exactly how it presented, and any family history of asthma, eczema or hay fever. Bring reports of any earlier allergy testing.

## Aftercare

Always carry your emergency plan and adrenaline auto-injector if one has been prescribed, check its expiry date, and train your family, school or workplace to use it. Read every product label every time, since formulations change, and ask in restaurants about cooking methods and shared oil. \*\*Document the culprit food or drug by its generic name on a card you carry and in your medical file, and inform every physician, dentist, pharmacist and paramedic before any prescription, anaesthetic or contrast scan — this is the single most important step in preventing a repeat reaction.\*\* Never attempt a food challenge at home, even with a tiny amount; its place is in hospital under supervision. \*\*Call emergency services immediately for swelling of the lip, tongue or throat, breathing difficulty, wheeze or hoarseness, dizziness, fainting, or pallor with cold sweat, or a rapidly spreading rash with vomiting — do not wait for spontaneous improvement, do not rely on an oral antihistamine, and lie flat with the legs raised rather than standing or walking to the car.\*\*
