## Procedure steps

### 1. Diagnosis and severity assessment

The newborn is examined to define the components of the deformity, its flexibility and severity. The other limb, the hips and the spine are checked because some cases are associated with other conditions, and the full pathway, its duration and the role of the brace are explained before the first cast.

### 2. Gentle manipulation and serial casting

At each visit the surgeon gently manipulates the foot towards a more corrected position and applies a cast extending above the knee to hold it. Casts are changed roughly weekly, correcting the elements of the deformity in a defined sequence rather than all at once.

### 3. Achilles tendon release when required

If the ankle remains tight after rotational correction is complete, a simple Achilles release is performed through a very small incision under local anaesthesia or light sedation, followed by a final cast kept on somewhat longer while the tendon heals.

### 4. Bracing and boots phase

After casting the child wears two boots connected by a bar that holds the feet at the corrected angle: full time in the early months, then during sleep and rest for several years. This phase is the cornerstone of relapse prevention, not an optional extra.

### 5. Regular follow-up and relapse detection

Regular reviews are scheduled until growth is complete to monitor foot shape, range of movement, gait and shoe fit, because a relapse detected early can often be managed with a short repeat casting series, whereas a late one may require more extensive surgery.

## About this service

Clubfoot is a congenital deformity in which a baby is born with the foot turned inward and downward, with a tight Achilles tendon and altered shape and ligament positions of the foot bones. The standard treatment today is, in most cases, a non-surgical pathway: gentle manipulation of the foot followed by serial casting week after week, so that the components of the deformity are corrected in sequence, then a long maintenance phase in a brace to hold the correction.

Timing matters greatly. A newborn's tissues are pliable and respond to gradual correction, so casting usually begins in the first weeks of life. At the end of the casting series many children need a small procedure on the Achilles tendon to complete ankle movement; this is commonly done under local anaesthesia or light sedation, followed by a final cast. The brace phase then begins with shoes attached to a bar, worn day and night at first, with the hours reduced progressively over the following years.

The limits must be clear to parents. A treated foot becomes functional and painless: the child walks, runs, plays and wears ordinary shoes. Yet the foot may remain slightly smaller than the other and the calf thinner, and this cosmetic difference is not a failure of treatment. Casting alone is also not enough. Worldwide, the commonest cause of relapse is stopping the brace or using it incorrectly, and no amount of massage or exercise compensates for that. Equally, home massage and stretching alone do not correct a true structural deformity, and relying on them can waste the crucial first months.

It is also important to remove misplaced guilt: clubfoot is not caused by something the mother did during pregnancy, nor by any food, movement or superstition; it is a congenital difference in how the foot formed. The type of cast, the need for any procedure and the timing of each stage are determined by direct clinical examination of the child and by how the foot responds, not by comparison with another child.

## Preparation

Feed the baby a suitable time before the casting appointment so that he is settled, and bring extra nappies, loose clothing that fits over a cast, and a light blanket. Tell the doctor about any illness, fever, skin infection or nappy rash, about any other deformity or hip and spine problem, about every medicine the child receives, and about similar cases in the family. Bring pregnancy and birth records and any previous X-ray or ultrasound reports. Ask for the complete visit schedule before starting and arrange transport, because weekly consistency is what produces the result. If the small Achilles procedure is planned, follow exactly the fasting instructions given to you according to the type of anaesthesia used.

## Aftercare

Check the toes several times a day: they should be pink, warm and able to move, the cast should not have slipped, and the heel should not be riding out of it. Keep the cast clean and dry, never push anything inside to scratch, and elevate the leg slightly during sleep. In the brace phase, follow the prescribed number of hours exactly and inspect the skin over the heel and outer border daily. Do not stop bracing because the foot now looks normal — that is the single commonest cause of relapse. \*\*Seek immediate review if the toes swell, turn blue or feel cold, or colour does not return after light pressure\*\*; \*\*if there is a foul smell, discharge or blood from the cast\*\*; \*\*if the child develops a fever\*\*; \*\*if the cast slips, cracks or becomes wet\*\*; or \*\*if there is unusual inconsolable crying or refusal to move the limb\*\*. Never remove the cast yourself at home.
