التعامل الصحيح يبدأ بالتشخيص لا بالإرجاع. يفحص الطبيب الإحساس والنبض وقوّة العضلات لأن الأعصاب والأوعية تمرّ قريباً من المفصل، ويطلب صورة أشعة قبل الإرجاع لاستبعاد كسر مرافق، لأن الشدّ على كتف مكسور قد يوسّع الكسر أو يؤذي العصب. \*\*لا تحاول إرجاع الكتف بنفسك ولا تسمح لغير مختص بشدّه أو «تفكيكه» — هذه ممارسة شائعة وخاطئة تسبّب كسوراً وتمزّقات وإصابات أعصاب دائمة.\*\*

بعد التشخيص يُرجَع المفصل برفق تحت تسكين وإرخاء عضلي، ثم تُعاد الصورة للتأكّد من عودته وسلامة العظم. يلي ذلك تثبيت مؤقّت قصير للراحة، ثم برنامج تأهيل تدريجي يستعيد المدى ويقوّي الكفة المدوّرة وعضلات لوح الكتف، وهو الجزء الذي يحدّد النتيجة النهائية أكثر من لحظة الإرجاع نفسها. عند تكرار الخلع، أو عند وجود تمزّق في الحلقة الغضروفية أو نقص في عظم الحقّ، يُطرح تثبيت الكتف بالتنظير أو بالجراحة المفتوحة.

الحدود مهمّة: الإرجاع يعيد العظم إلى مكانه لكنه لا يُصلح الرباط أو الغضروف الذي تمزّق أثناء الخلع، والحزام لا يشفي ولا يمنع تكرار الخلع، وجراحة التثبيت تقلّل التكرار كثيراً لكنها لا تعيد الكتف كما كان قبل الإصابة تماماً ولا تُعفي من التأهيل. واحتمال التكرار أعلى عند الشباب والرياضيين، وأقل عند الأعمار المتقدّمة التي يغلب فيها التمزّق على عدم الاستقرار. الترشيح لأي خيار يُحدَّد بالفحص السريري والتصوير لا بقراءة صفحة.

## خطوات الإجراء

### 1. التقييم العصبي والوعائي أولاً

قبل أي شدّ يفحص الطبيب الإحساس على الكتف والذراع ونبض الرسغ وقدرتك على شدّ العضلات، ويسجّل أي خدر أو برودة، لأن هذه الملاحظات تُعاد مقارنتها بعد الإرجاع.

### 2. التصوير قبل الإرجاع

تُطلب صورة أشعة لتأكيد اتجاه الخلع واستبعاد كسر في رأس العضد أو حافة الحقّ، فوجود كسر يغيّر طريقة الإرجاع وقد يحوّل الحالة إلى قرار جراحي مباشر.

### 3. الإرجاع تحت تسكين وإرخاء

يُعطى تسكين ومرخٍّ للعضلات ثم يُعاد رأس العضد إلى مكانه بحركة هادئة محسوبة بلا عنف، ويُلاحظ عودة شكل الكتف واختفاء الألم الحاد، ثم يُعاد الفحص العصبي والتصوير.

### 4. التثبيت المؤقّت القصير

يُستخدم حزام أو رافعة لأيام قليلة لإراحة الأنسجة، مع تحريك المرفق والرسغ واليد يومياً. التثبيت الطويل بلا حركة يورث تيبّساً وكتفاً متجمّداً، ولا يقلّل احتمال التكرار.

### 5. التأهيل ثم قرار التثبيت الجراحي

يبدأ برنامج متدرّج يستعيد المدى ثم يقوّي الكفة المدوّرة وثبات لوح الكتف والإحساس بموضع المفصل. إن تكرّر الخلع أو بقي إحساس بعدم الثبات يُقيَّم المريض لجراحة تثبيت بالتنظير أو المفتوحة.

## Procedure steps

### 1. Neurovascular assessment first

Before any manipulation, the doctor tests sensation over the shoulder and arm, the wrist pulse and your ability to contract muscles, and documents any numbness or coldness, because these findings are rechecked after reduction.

### 2. Imaging before reduction

An X-ray confirms the direction of dislocation and excludes a fracture of the humeral head or the socket rim; a fracture changes the reduction technique and may turn the case into a direct surgical decision.

### 3. Reduction under sedation and relaxation

Sedation and a muscle relaxant are given, then the humeral head is guided back with a slow, controlled manoeuvre and no force. The shoulder contour returns and severe pain settles, after which the neurological check and imaging are repeated.

### 4. Short protective immobilisation

A sling is used for a few days to rest the tissues, while the elbow, wrist and hand are moved daily. Prolonged immobilisation causes stiffness and a frozen shoulder, and does not reduce the chance of recurrence.

### 5. Rehabilitation, then the stabilisation decision

A staged programme restores range, then strengthens the rotator cuff, scapular control and joint position sense. If dislocations recur or a sense of instability persists, the patient is assessed for arthroscopic or open stabilisation.

## التحضير قبل الإجراء

إن كان الخلع حادثاً الآن فلا تحاول تحريك الكتف ولا شدّه، وثبّت الذراع ملتصقة بالجسم بوشاح أو منشفة، وانزع الخواتم والساعة قبل أن يزيد التورّم، ولا تشرب ولا تأكل شيئاً لأن الإرجاع قد يحتاج تسكيناً، ثم اتجه إلى الطوارئ. أخبر الفريق بعدد المرّات التي خُلع فيها الكتف سابقاً وكيف حدث الخلع هذه المرّة، وبكل دواء تتناوله وخاصة مسيّلات الدم، وبأي حساسية دوائية، وبأمراض القلب والرئة والصرع، ومتى كانت آخر وجبة. أحضر تقارير وصوراً سابقة إن وُجدت. وإن كان الموعد لجراحة تثبيت مُقرّرة فالتزم بتعليمات الصيام وتوقيت إيقاف الأدوية كما يحدّدها طبيبك بدقّة، ورتّب من يساعدك في البيت بعدها.

## العناية بعد الإجراء

استخدم الحزام للمدّة التي حدّدها طبيبك فقط ولا تُطِلها من نفسك، وحرّك المرفق والرسغ والأصابع يومياً، والتزم بجلسات العلاج الطبيعي فهي التي تحدّد النتيجة. تجنّب الحركة التي خلعت كتفك أول مرّة، وخصوصاً رفع الذراع بعيداً عن الجسم مع دورانها للخارج، ولا ترفع أوزاناً فوق الرأس ولا ترجع للرياضة التلاحمية قبل إذن الطبيب. النوم على الظهر مع وسادة تحت المرفق يريح الكتف في الأيام الأولى. \*\*راجع فوراً عند تنميل أو خدر متزايد، أو شحوب وبرودة اليد أو ضعف نبضها، أو عجز عن الحركة بعد أن كانت ممكنة، أو ألم متصاعد لا يهدأ بالمسكّن، أو حرارة وتورّم واحمرار ساخن بعد الجراحة، أو إحساس بأن الكتف خرج من مكانه مرّة أخرى — كل ذلك حالة طوارئ لا تنتظر موعداً.\*\*

## About this service

A shoulder dislocation is the displacement of the head of the humerus out of the glenoid socket of the shoulder blade. The shoulder has the widest range of motion in the body, and the price of that mobility is that it dislocates more often than any other joint. Most dislocations are anterior, following a fall on an outstretched hand or a forced outward and backward twist of the arm. The patient has severe pain, a visibly altered shoulder contour and an inability to move, and typically holds the arm pressed against the body.

Correct management begins with diagnosis, not with reduction. The doctor checks sensation, pulse and muscle power, because nerves and vessels run close to the joint, and usually requests an X-ray before reduction to exclude an associated fracture, since pulling on a fractured shoulder can extend the fracture or injure a nerve. \*\*Do not try to put the shoulder back yourself, and do not let an untrained person pull or manipulate it. This widespread practice causes fractures, tears and permanent nerve injuries.\*\*

Once assessed, the joint is reduced gently under sedation and muscle relaxation, and imaging is repeated to confirm that it is back in place and the bone is intact. A short period of protective immobilisation follows, then a graded rehabilitation programme restores range and strengthens the rotator cuff and shoulder-blade muscles. That rehabilitation determines the final outcome far more than the moment of reduction. When dislocations recur, or when there is a labral tear or bone loss at the socket rim, shoulder stabilisation by arthroscopy or open surgery is discussed.

The limits matter. Reduction returns the bone to its place but does not repair the ligament or cartilage torn during the dislocation; a sling neither heals the joint nor prevents recurrence; and stabilisation surgery greatly reduces recurrence but does not restore the shoulder exactly to its pre-injury state and does not remove the need for rehabilitation. Recurrence risk is highest in young people and athletes, and lower in older patients, in whom tendon tearing rather than instability tends to dominate. Suitability for any option is decided by examination and imaging, not by reading a page.

## Preparation

If the dislocation has just happened, do not move or pull the shoulder. Support the arm against the body with a scarf or towel, remove rings and watches before swelling increases, take nothing to eat or drink because reduction may require sedation, and go to the emergency department. Tell the team how many times the shoulder has dislocated before and how it happened this time, list every medicine you take, especially blood thinners, and mention drug allergies, heart, lung or seizure disorders and the time of your last meal. Bring previous reports and images if you have them. If the appointment is for planned stabilisation surgery, follow the fasting instructions and the timing for stopping medicines exactly as your surgeon specifies, and arrange help at home afterwards.

## Aftercare

Wear the sling only for the period your doctor specified and do not extend it on your own, move the elbow, wrist and fingers daily, and attend physiotherapy, because rehabilitation determines the outcome. Avoid the movement that dislocated the shoulder in the first place, especially raising the arm away from the body while rotating it outwards, and do not lift overhead or return to contact sport before your doctor allows it. Sleeping on your back with a pillow under the elbow eases the shoulder in the first days. \*\*Seek care immediately if numbness increases, if the hand becomes pale, cold or has a weak pulse, if movement that was possible is lost, if pain escalates despite analgesia, if you develop fever with hot red swelling after surgery, or if the shoulder feels as though it has come out again. Each of these is an emergency and must not wait for an appointment.\*\*
