## Procedure steps

### 1. Assessment and exclusion of other causes

Your doctor reviews the pattern of pain and its relation to standing, menstruation and intercourse, examines the pelvis and arranges Doppler ultrasound, sometimes with MR or CT venography, while ruling out endometriosis, adhesions and urinary or bowel causes before embolization is considered.

### 2. Preparation, local anaesthetic and venous access

The puncture site in the groin or neck is cleaned, local anaesthetic is given and a thin sheath is placed in the vein. You usually stay awake, with light sedation if needed, while blood pressure, heart rate and oxygen levels are monitored throughout.

### 3. Venography and confirmation of reflux

The catheter is advanced into the ovarian and pelvic veins and a small amount of contrast is injected under X-ray to document the direction of flow and the calibre of the veins. This step determines which veins genuinely need closure and which should be left alone.

### 4. Closing the refluxing veins

The affected veins are occluded with fine coils, a sclerosing agent or a combination of both, working from the lower segments upwards, with check imaging after each step. Nothing remains outside the body and no surgical incision or stitches are required.

### 5. Haemostasis, observation and follow-up

The catheter is removed, pressure is applied to the puncture site until bleeding stops and a dressing is applied. You are observed for a few hours and usually discharged the same day, with the result judged at follow-up visits over weeks to months since pain relief appears gradually.

## About this service

Treatment of uterine and pelvic varices addresses reflux of blood within the ovarian and pelvic veins, a condition known as pelvic congestion syndrome. When the valves in these veins stop working properly, blood pools in them and stretches the vein walls. The typical picture is chronic pelvic pain that builds up through the day, worsens with prolonged standing, before menstruation and after intercourse, and eases when lying down. Some women also notice visible varicose veins over the upper thigh, buttock or vulval area.

The most widely used treatment today is percutaneous embolization, performed by an interventional radiologist without open surgery. Under local anaesthetic, a fine catheter is introduced through a vein in the upper thigh or the neck and guided under imaging to the ovarian or pelvic veins. Once reflux is confirmed on venography, the abnormal veins are closed with small metal coils or a sclerosing agent, so blood is redirected through healthy veins and congestion gradually settles.

The limits of the procedure matter as much as its benefits. Embolization treats venous reflux only. It does not treat endometriosis, pelvic adhesions, interstitial cystitis or irritable bowel syndrome, all of which are common causes of chronic pelvic pain and must be excluded first. It does not remove fibroids, does not treat polycystic ovaries, and is not a fertility procedure. Improvement is gradual over weeks to months rather than immediate, and some women are left with residual pain that needs continued follow-up.

Whether you are a candidate is decided by clinical examination and your doctor's assessment alongside imaging, never by reading an article. Assessment usually starts with pelvic ultrasound and Doppler, may be completed with MR or CT venography, and reflux is confirmed by venography during the session itself. Dilated pelvic veins seen on a scan without troublesome symptoms do not by themselves justify treatment.

## Preparation

Tell your doctor if there is any chance you are pregnant; a pregnancy test may be requested because the procedure uses X-rays and contrast. Report any previous contrast reaction, any kidney problem, and every medicine or supplement you take, especially blood thinners or diabetes medication, and never stop a medicine on your own but ask for instructions. Blood tests for kidney function and clotting are usually arranged. You will normally be asked to fast for six to eight hours, with sips of water and approved tablets allowed. Say if you are breastfeeding. Wear comfortable clothing, remove jewellery, and arrange for someone to take you home, as driving on the same day is not advised.

## Aftercare

Press on the puncture site if needed, keep the dressing clean and dry for the first day, and avoid baths, strenuous effort and heavy lifting for about three to seven days. Cramping, a dull dragging pelvic ache, mild tiredness and sometimes a low-grade temperature for a few days are expected; this is called post-embolization syndrome and is managed with the painkillers your doctor prescribed. Drink enough fluids. Pain relief develops gradually over weeks to months. \*\*Go to the emergency department immediately if you develop a high fever with shivering, severe escalating pelvic pain not relieved by painkillers, leg swelling and pain, shortness of breath or chest pain, heavy vaginal bleeding, a rapidly expanding swelling or continued bleeding at the puncture site, or fainting and severe dizziness.\*\* Contact your doctor if the puncture site becomes red, hot or discharges pus.
