Duplex ultrasound and reflux mapping Ultrasound is performed standing and lying to identify the affected vein, the sites of valve failure and the state of the deep veins before any treatment decision. ### 2. Local anaesthesia and fibre insertion The skin is anaesthetised locally and the vein punctured with a fine needle, then the laser fibre is advanced inside the vein to the planned level under ultrasound guidance. ### 3. Perivenous tumescent anaesthesia Dilute anaesthetic is infiltrated around the vein to insulate adjacent tissue from heat and reduce pain during energy delivery. ### 4. Energy delivery and vein closure The fibre is withdrawn gradually while thermal energy is delivered along the diseased vein, and closure is confirmed by ultrasound at the end. ### 5. Compression and follow-up A compression stocking is applied with immediate walking encouraged, and a follow-up visit with duplex confirms the vein remains closed, with residual spider veins treated if needed. ## التحضير قبل الإجراء أخبر الطبيب بأدويتك خصوصاً مسيّلات الدم وحبوب منع الحمل والعلاج الهرموني، وبأي تاريخ لتجلّط وريدي عميق أو جلطة رئوية أو اضطراب تخثّر عائلي. أبلغه إن كنت حاملاً أو تخطّطين للحمل. أحضر تقارير الدوبلر السابقة إن وُجدت، واحلق ساق العلاج إن طُلب منك، والبس ملابس واسعة. اشترِ الجورب الضاغط بالمقاس ودرجة الضغط التي يحدّدها الطبيب قبل الموعد، ورتّب المشي بعد الإجراء لأنه مطلوب لا ممنوع. اسأل عن عدد الأوردة التي ستُعالج وهل تحتاج جلسات حقن تصليب لاحقة. ## العناية بعد الإجراء اِلبس الجورب الضاغط بالمدة اليومية التي يحدّدها الطبيب، فهو جزء من العلاج لا إضافة تجميلية. امشِ مباشرة وبانتظام من اليوم نفسه لتقليل خطر التجلّط، وتجنّب الوقوف الطويل الساكن والجلوس المتواصل ورفع الأثقال الشديدة والساونا والحمّامات الساخنة في الأسابيع الأولى. توقّع شدّاً أو وجعاً على مسار الوريد وكدمات وتحسّس خفيف بالجلد يتراجع تدريجياً. ارفع الساق عند الراحة، واستخدم مسكّنات بسيطة كما يوصي الطبيب. راجع فوراً إن ظهر تورّم مفاجئ في الساق أو ألم شديد في السمانة أو حرارة أو احمرار ممتد أو ضيق نفس أو ألم صدري، فهذه علامات تستدعي استقصاء تجلّط عاجلاً. ## About this service Laser treatment of leg varicose veins (endovenous thermal ablation) is a minimally invasive procedure that treats the diseased superficial vein from inside rather than removing it surgically. A fine laser fibre is introduced into the vein through a small puncture under ultrasound guidance, then thermal energy is delivered to seal the diseased vein, which fibroses and closes gradually while blood is redirected to healthy deeper veins. The underlying problem is failure of the superficial vein valves, which lets blood fall back downwards under gravity instead of ascending toward the heart. Pressure inside the vein rises and it dilates and becomes tortuous. It is therefore not enough to look at the appearance of the varicosities: correct diagnosis requires a duplex ultrasound scan to identify exactly which vein is affected and where the reflux is, and treatment is built on that map. Its advantage over conventional surgery is that it is usually performed under local anaesthesia, with return to activity typically the same or next day and less pain and bruising. It does not, however, remove the need for compression stockings afterwards, nor the fact that varicose disease can recur in other veins over time, especially with prolonged standing, weight gain or pregnancy. Fine surface spider veins may need complementary sclerotherapy sessions. Candidacy is determined by the duplex findings and the state of the deep veins, not by outward appearance alone. ## Preparation Tell your surgeon about your medications, especially blood thinners, contraceptive pills and hormone therapy, and about any history of deep vein thrombosis, pulmonary embolism or familial clotting disorder. Report pregnancy or plans to conceive. Bring previous duplex reports if available, shave the treatment leg if asked, and wear loose clothing. Buy the compression stocking in the size and pressure grade your surgeon specifies before the appointment, and plan to walk afterwards — it is required, not forbidden. Ask how many veins will be treated and whether later sclerotherapy sessions will be needed. ## Aftercare Wear the compression stocking for the daily duration your surgeon specifies; it is part of the treatment, not a cosmetic extra. Walk immediately and regularly from the same day to reduce clotting risk, and avoid prolonged static standing, continuous sitting, heavy lifting, saunas and hot baths in the early weeks. Expect tightness or tenderness along the vein track, bruising and mild skin sensitivity that settle gradually. Elevate the leg at rest and use simple analgesia as advised.
