## Procedure steps

### 1. Complete diagnosis before any treatment

A detailed neurological examination and contrast-enhanced MRI are performed, and tests of the rest of the body are requested if the tumour may have spread from another organ, since that changes treatment radically. A tissue sample is then obtained by image-guided biopsy or during surgery, and completed with molecular tests that establish type and grade.

### 2. Team meeting and plan design

The case is presented to a multidisciplinary team that weighs the tissue result against tumour location, the patient's age, general condition and other illnesses. A written plan with a clear sequence and stated goals is produced and explained honestly to the patient and family, including what each option can achieve and what it cannot.

### 3. Surgery when indicated

Surgery is performed to relieve pressure inside the skull and remove as much tumour as is safe while preserving function, using image guidance and electrophysiological monitoring of critical pathways. The surgeon stops where the next step would carry more risk than benefit, and removal may be deliberately partial with treatment then continued by other means.

### 4. Radiotherapy and drug treatment

Radiotherapy is planned precisely on imaging to treat the tumour while sparing healthy tissue, and may be delivered in fractionated sessions or concentrated on a limited focus. Chemotherapy or targeted drug treatment is added according to tumour type and molecular features, with side effects monitored through blood tests and regular appointments that must not be postponed.

### 5. Supportive care, rehabilitation and follow-up

Seizures, swelling, pain and nausea are treated with prescribed medicines taken on schedule, and physiotherapy and speech therapy begin early to recover as much independence as possible. Follow-up scans are repeated at defined intervals and compared with earlier studies, and psychological and social support is offered to patient and family as part of the plan rather than an optional extra.

## About this service

Treating brain cancer means building an individual, combined plan rather than performing a single procedure. "Brain cancer" is an umbrella term covering malignant tumours that behave very differently: primary tumours arising from the brain's own cells, such as gliomas, and secondary tumours that have spread to the brain from another organ. The distinction is not a matter of wording; it changes the treatment plan completely, and it cannot be settled by imaging alone but only by examining tissue under the microscope.

Tissue is the starting point, not the conclusion. A sample is obtained by image-guided biopsy or during resection surgery, examined by a pathologist, and completed with molecular tests that define the tumour's type, grade and biological features. Every subsequent decision rests on that result: whether to begin with surgery or radiotherapy, whether drug treatment is added, and in what sequence. A plan made before the tissue result is known is guesswork, which is why patients are asked to wait days for the report. That wait is part of correct treatment, not a delay in it.

The available tools are usually combined. Surgery relieves pressure, provides tissue and removes what can be removed safely. Radiotherapy is used in its various forms, including precisely targeted radiation to a limited focus. Drug treatment, whether chemotherapy or targeted therapy, is chosen according to tumour type and molecular features. Supportive care matters just as much: controlling seizures, reducing swelling around the tumour, treating nausea and pain, physiotherapy and speech therapy, and psychological support for the patient and family.

The limits are stated openly. Surgery may be deliberately partial when the tumour is interwoven with a critical centre, because leaving part of it behind is better than permanent paralysis. Radiation does not dissolve a tumour immediately; its aim is to control growth, and post-radiation scans can show changes that resemble tumour growth but are not. That is why images are compared over time and no decision is based on one scan. No treatment erases neurological damage that was already established before treatment began. Herbal and alternative remedies do not replace treatment and do not stop a tumour, and relying on them instead of the medical plan wastes time that cannot be recovered.

This care is by necessity multidisciplinary: neurosurgeon, oncologist, radiation oncologist, radiologist, pathologist and rehabilitation team. Suitability for each step is determined by clinical examination, general condition, the tissue result and team discussion, not by comparing your case with another patient's or by what you read online.

## Preparation

Bring all previous MRI and CT studies as digital files with their reports, and the results of any earlier biopsy, because comparison over time underpins the decision. Write out a complete list of your medicines, including blood thinners, cortisone, anti-seizure, diabetes and blood-pressure medicines, and mention every herbal supplement you use, since some interact with drug treatment or increase bleeding risk. Report any drug or contrast allergy, and any kidney, liver or heart disease. State any previous radiotherapy or chemotherapy and where it was given. Tell the team about any seizure or loss of awareness, any change in vision, speech or walking, and any problems with sleep or mood. Ask a family member to attend with you so they hear the explanation and record the plan and appointments. If you are of reproductive age, ask about the effect of treatment on fertility and pregnancy before it starts rather than afterwards. Write your questions in advance, and ask what each step aims to achieve and what the alternative is if it does not work.

## Aftercare

Take every medicine on time, and never stop or reduce anti-seizure medicine or cortisone without your doctor, because abrupt withdrawal is dangerous. During drug treatment or radiotherapy keep to your blood tests and appointments, look after your mouth hygiene, drink enough fluids, eat a balanced diet, and avoid crowded places if your resistance to infection is low. Start physiotherapy early rather than waiting for strength to return by itself. Do not replace treatment with herbal or alternative remedies, and add no supplement without telling your doctor. Do not drive until your doctor agrees, especially after a seizure or with changed vision. \*\*Go to the emergency department immediately if you have: a severe headache that worsens rapidly or wakes you from sleep with repeated vomiting; a seizure; new weakness or numbness on one side; a sudden change in vision or speech; confusion or severe drowsiness; fever during chemotherapy; unexplained bleeding or bruising; or breathlessness, chest pain or a painful swollen leg.\*\* Keep your imaging appointments even when you feel better: comparison over time is what detects change early.
