## Procedure steps

### 1. Clinical examination and identifying the source

The therapist assesses active and passive shoulder range, rotator cuff strength and scapular movement, performs impingement and stability tests, and examines the neck, since much shoulder pain originates there. Conditions needing a doctor and imaging are screened out: a suspected full-thickness tear, a dislocation, or pain following significant trauma.

### 2. Settling irritation and modifying activity

Overhead movements, heavy lifting and sleeping on the affected shoulder are temporarily reduced, while heat or cooling and manual therapy address tight muscles, and pain-relieving devices may be used briefly. The arm is kept moving within its comfortable range, because prolonged immobilisation produces stiffness worse than the original strain.

### 3. Restoring range of motion

Pendulum exercises, table or wall slides, assisted movement with a stick or the sound hand, and stretching for the posterior shoulder capsule and chest where they are tight. Range is widened gradually and never forced; the guide is that pain should not persist after the exercise, since forcing into a painful range increases irritation and delays strengthening.

### 4. Strengthening the cuff and scapular stabilisers

The most important phase: external and internal rotation exercises with light resistance increased weekly, strengthening for the upper back and the muscles that stabilise the scapula, and control work to raise the arm with a sound pattern. Strength is measured and compared between sides, and load is progressed according to next-day symptoms rather than enthusiasm.

### 5. Return to work and sport, and prevention

Overhead tasks, lifting, pushing and pulling are graded up to the level your work or sport requires, alongside workstation changes, lifting technique and shelf arrangement. A weekly maintenance plan is given after discharge, because cuff strength declines once exercise stops. Duration is estimated initially and reassessed, with no promised session count or outcome.

## About this service

Treatment of shoulder strain is the physiotherapy management of overload in the shoulder muscles, tendons and girdle, the complaint that follows heavy lifting, a sudden movement, repetitive overhead work or sleeping on the shoulder. What patients call a strain may in fact be a muscle overload, irritation of the rotator cuff tendons, subacromial impingement, or spasm in the neck and upper back muscles referring pain to the shoulder. The plan therefore begins with a clinical examination that distinguishes between these causes, because each needs a different exercise approach.

Treatment includes a first phase to settle irritation: activity modification without complete rest, heat or cooling, manual therapy for tight muscles, and lower-load sleeping and working positions. A second phase restores range of motion with gliding, pendulum and assisted exercises. The third phase, strengthening, is the foundation: building the rotator cuff tendons, the scapular stabilisers and the upper back, correcting the movement pattern used to raise the arm, and then grading up toward work and sporting tasks.

Results are cumulative and consistency is the deciding factor: short daily home exercises determine the outcome more than the number of clinic sessions, and stopping when the pain disappears but before strength returns is the commonest reason the strain comes back. Two myths need correcting. The first is that physiotherapy is a shoulder massage — massage relieves spasm temporarily but does not build a tendon and does not prevent recurrence. The second is that \*\*exercise must hurt to work\*\* — in reality mild soreness that settles by the next day is acceptable, while sharp pain during lifting or increasing night pain after exercise means the dose was too high and needs adjusting. Devices such as therapeutic ultrasound, laser, electrical stimulation and shockwave help settle a phase and do not replace exercise; no specific machine is recommended here.

The limits are explicit. Physiotherapy does not knit a full-thickness rotator cuff tear, does not necessarily dissolve a calcific deposit, and does not reduce a dislocated joint; injections and medication are adjuncts decided by a doctor. Equally, \*\*complete rest and keeping the arm in a sling for long periods can cause stiffness and a frozen shoulder\*\*, a complication worse than the original strain. Treatment length is estimated initially in weeks and reassessed, and suitability is decided by clinical examination rather than by reading.

## Preparation

Tell the therapist exactly how the pain began: heavy lifting, a sudden movement, a direct blow, or a gradual onset with no clear cause, whether it wakes you at night, and whether it spreads to the neck, arm or hand with numbness. Mention any previous dislocation or shoulder operation, any injections you have had and when, and any diabetes or thyroid disease, since both increase the risk of stiffness, along with your medications including blood thinners and steroids. Bring imaging reports if you have them, and wear a loose or sleeveless top that allows the shoulder and upper back to be fully exposed. Avoid strong painkillers before the session unless needed, so the safe limits of exercise are not masked. If you cannot lift the arm at all after a significant blow, or the shoulder looks deformed, see a doctor for imaging before physiotherapy.

## Aftercare

Keep to your home exercises daily at the prescribed dose, and continue the strengthening for weeks after the pain has gone, because a tendon needs longer than the pain to regain load capacity. Sleep on the unaffected side with a small pillow supporting the injured arm, bring objects closer to you instead of reaching overhead in the early phase, avoid carrying a bag on the affected shoulder, and take movement breaks if your job involves repetitive lifting. Expect mild soreness that settles within a day; pain that worsens at night or prevents you lifting the arm the next day should be reported so the dose can be reduced. Seek medical care immediately if you have: \*\*complete inability to lift the arm after a significant blow, or a deformed-looking joint — emergency assessment and imaging\*\*, \*\*fever with a swollen, red, hot joint\*\*, \*\*persistent night pain that does not settle with rest or worsens week by week\*\*, \*\*increasing numbness or weakness in the arm and hand\*\*, or left shoulder pain with breathlessness, sweating or chest tightness — this needs emergency care as the cause may be cardiac.
