Sensorineural Hearing Loss

## Procedure steps

### 1. Urgent assessment and defining the pattern of loss

The doctor asks how quickly hearing dropped, which ear is affected, and whether there is tinnitus, dizziness, discharge or pain, along with noise exposure, medicines and recent infections. Sudden loss in one ear is treated as urgent and assessed the same day, because the first days determine what can still be recovered.

### 2. Full hearing assessment

Pure tone audiometry, speech testing and middle ear tests are performed to separate sensorineural from conductive loss and to define the degree and the frequency pattern affected. This assessment underpins every later decision, because treatment differs completely between the two types and cannot be guessed from symptoms.

### 3. Investigations to identify the cause

Auditory brainstem response testing, imaging of the inner ear and nerve pathway, and blood tests may be requested, especially with one-sided or asymmetric loss or when dizziness is present. The purpose is to exclude a cause needing specific intervention rather than accepting the loss as inevitable.

### 4. Medical treatment in acute cases

In sudden loss the doctor starts a specific drug treatment as early as possible, with repeat testing to follow the response. Medicines or injections are never self-initiated or bought without prescription, and oils and herbal remedies have no role here. What really changes the outcome is how quickly the patient reaches the doctor.

### 5. Hearing compensation and follow-up

For stable loss, hearing aids are prescribed and programmed precisely to the pattern of loss, with an adaptation period and repeated fine-tuning, and cochlear implantation is discussed if aids are not enough. Communication strategies and noise protection are added, and periodic review is arranged because loss may progress.

## About this service

Sensorineural hearing loss is a reduction in hearing caused by damage to the sensory cells inside the cochlea or to the nerve pathway that carries sound to the brain, rather than by a blocked canal or a middle ear problem. The commonest complaint is not simply that sound is quieter, but that speech is unclear: the person hears that someone is talking yet cannot make out the words, and difficulty increases in background noise and with higher-pitched voices such as those of children and women.

\*\*Sudden hearing loss in one ear is a medical emergency, not something to wait out.\*\* If you wake up or notice a clear drop in hearing in one ear over hours or a day or two, whether alone or with tinnitus, a feeling of fullness or dizziness, see an ear, nose and throat doctor the same day. \*\*The treatment window is only a few days, and delay can leave the loss permanent.\*\* Do not blame wax, a cold or tiredness, do not wait for the weekend to pass, and do not rely on drops or home oils. This is the single most important message on this subject.

Causes are varied: ageing, repeated exposure to loud noise, genetic factors, viral infections, certain medicines that affect hearing, immune disease, and conditions pressing on the nerve. Assessment therefore begins with a full hearing test that separates sensorineural from conductive loss, and may be extended with auditory brainstem response testing, imaging or blood tests depending on the picture, particularly when only one ear is affected.

The limits of treatment must not be hidden. There is currently no medicine that rebuilds damaged sensory cells, and no supplement or session that brings back established lost hearing. Drug treatment has a defined role in sudden, acute cases and in specific causes identified by the doctor. Stable loss is managed by compensation: carefully fitted hearing aids, assistive listening systems, and cochlear implantation for severe loss that hearing aids can no longer serve. All of these improve clarity and communication; none restores hearing as it was.

Prevention is an essential part of the plan, because what is lost does not return. Protect your hearing from occupational and recreational noise, do not take medicines known to affect hearing without supervision, manage blood pressure and diabetes, and have periodic hearing tests if you work in a noisy environment. Candidacy for any compensatory option is determined by clinical examination and audiological assessment, not by reading.

## Preparation

If your hearing loss is sudden, do not prepare anything and do not wait for an appointment: see a doctor the same day, because time is the treatment. For a planned assessment, bring the results of any previous hearing test, including old ones, since comparison matters, a complete list of your medicines including painkillers, antibiotics and long-term treatments, and reports of any ear surgery or infection. Write down when the loss began and whether it came on gradually or within hours, which ear is worse, and whether it is accompanied by tinnitus, dizziness, fullness or discharge. Mention your work and any exposure to noise from machinery, firearms or loud headphones, and any family history of hearing loss. Do not clean inside your ear with cotton buds before testing and do not use oils or drops, and avoid loud noise the day before, as it can temporarily alter results.

## Aftercare

Stick to the treatment plan and to your repeat testing appointments, since the hearing test done weeks later is what shows the true response. If a hearing aid has been prescribed, give it a proper adaptation period and wear it during all waking hours, noting difficult situations so programming can be adjusted rather than abandoning the device. Protect the hearing you still have: use ear protection in noisy work and at weddings and events, lower the volume of personal headphones and shorten listening time, and keep away from very loud sources. Avoid medicines known to affect hearing unless medically supervised, and keep blood pressure and diabetes controlled. Seek care immediately if you develop: \*\*any new sudden drop in hearing over hours or days\*\*, \*\*severe dizziness with vomiting or inability to stand\*\*, \*\*facial weakness or numbness\*\*, \*\*an unusually severe headache or disturbed speech or vision\*\*, or ear pain, discharge or fever.
