البحث عن السبب ليس ترفاً لأنه يغيّر المسار كلّه: مرض في الشرايين التاجية أو جلطة قلبية سابقة، ضغط دم مرتفع غير مضبوط لسنوات، خلل في صمّام، التهاب فيروسي في عضلة القلب، اضطراب نظم سريع مستمرّ، أمراض وراثية في العضلة، الكحول، بعض أدوية العلاج الكيماوي، وضعف مرتبط بالحمل. لذلك يشمل التقييم تخطيط قلب وإيكو لقياس كفاءة الضخّ، وتحاليل دم للكلى والأملاح والغدة الدرقية ومؤشّرات القلب، وقد يُضاف تصوير للشرايين أو رنين مغناطيسي للقلب.

العلاج ثلاثي المسار. الأول علاج السبب حين يكون قابلاً للعلاج: توسيع شريان متضيّق، إصلاح صمّام، ضبط ضغط الدم، السيطرة على اضطراب النظم، إيقاف الكحول. والثاني خطة دوائية تُبنى من مجموعات أساسية يُضاف بعضها فوق بعض وتُرفع جرعاتها تدريجياً مع مراقبة الضغط والنبض والكلى والأملاح — وهذه ليست مجرّد أدوية لتخفيف الأعراض بل أدوية تُبطئ تدهور العضلة، وتُضاف مدرّات البول للتخلّص من احتباس السوائل. والثالث أجهزة في حالات مختارة: منظّم ضربات خاص ينظّم تقلّص حجرات القلب، أو مُقوّم رجفان مزروع للحماية من اضطراب نظم خطير، وفي حالات متقدّمة نادرة تُبحث خيارات أعلى في مراكز متخصصة.

وما تفعله أنت كل يوم جزء من العلاج بالقدر نفسه: وزن يومي في الصباح بعد الحمّام وقبل الطعام وتسجيله، تقليل الملح والانتباه للأطعمة المصنّعة والمخلّلات والمرقات الجاهزة، الالتزام بحدّ السوائل الذي يحدّده طبيبك، حركة منتظمة أو برنامج تأهيل قلبي، الإقلاع عن التدخين، اللقاحات الموسمية، وعلاج السكري والشخير وتوقّف النفس أثناء النوم.

وحدود العلاج يجب أن تكون واضحة: الأدوية لا تُنبت عضلة جديدة، وقد تتحسّن كفاءة الضخّ تحسّناً كبيراً لكن التشخيص والمتابعة والعلاج يستمرّان عادةً مدى الحياة. \*\*وأشهر سبب لعودة المريض إلى الطوارئ هو إيقاف الدواء عند الشعور بالتحسّن، أو انفلات الملح والسوائل.\*\* ولا يوجد عشب ولا مكمّل ولا وصفة شعبية تُغني عن هذه الأدوية، والجهاز المزروع لا يقوّي العضلة بنفسه بل يحمي من مشكلة كهربائية أو ينظّم التقلّص. ولا تُوقف مضادات التصفيح أو مضادات التخثّر بنفسك ولا قبل إجراء أسنان إلا بقرار طبيب القلب.

## خطوات الإجراء

### 1. تشخيص السبب وقياس كفاءة الضخّ

تُوثّق الأعراض ويُفحص القلب والرئتان والساقان، ويُجرى تخطيط قلب وإيكو لقياس كفاءة الضخّ وحجم الحجرات والصمّامات، وتُطلب تحاليل للكلى والأملاح والدرقية ومؤشّرات القلب، وقد يُضاف تصوير للشرايين أو رنين للقلب لتحديد السبب بدقّة.

### 2. بناء الخطة الدوائية وضبط جرعاتها

تُبدأ المجموعات الدوائية الأساسية تدريجياً وتُرفع جرعاتها على زيارات متقاربة مع مراقبة الضغط والنبض ووظائف الكلى والأملاح. هذه المرحلة تحتاج صبراً ومواعيد منتظمة، وأي دوخة أو هبوط ضغط يُبلَّغ للعيادة ليُعدَّل الدواء لا ليُوقف من تلقاء نفسك.

### 3. علاج السبب القابل للعلاج

إن كان السبب تضيّقاً في شريان تاجي أو خللاً في صمّام أو اضطراب نظم سريعاً مستمرّاً أو ضغطاً غير مضبوط أو كحولاً، فعلاج السبب نفسه قد يُحسّن كفاءة الضخّ تحسّناً ملموساً. ولذلك لا يُكتفى بوصف أدوية الأعراض دون البحث عن السبب.

### 4. تقييم الحاجة إلى جهاز

بعد فترة كافية على الخطة الدوائية المثالية يُعاد قياس كفاءة الضخّ. في حالات مختارة يُطرح منظّم ضربات ينظّم تقلّص الحجرات أو مُقوّم رجفان مزروع للحماية من اضطراب نظم قاتل، ويُشرح لك الهدف من الجهاز وحدوده قبل القرار.

### 5. المتابعة والوزن اليومي والتأهيل

تُتابع الأعراض والوزن اليومي وضغط الدم وتحاليل الكلى والأملاح، ويُوضع برنامج نشاط أو تأهيل قلبي متدرّج، وتُراجع اللقاحات وضبط السكري والنوم. تُشرح لك علامات التدهور المبكرة لتتصرّف قبل أن تصل إلى الطوارئ.

## Procedure steps

### 1. Diagnosing the cause and measuring pump function

Symptoms are documented and the heart, lungs and legs examined, an ECG and echocardiogram measure pumping function, chamber sizes and valves, and blood tests assess kidneys, electrolytes, thyroid and cardiac markers. Coronary imaging or cardiac MRI may be added to pin down the cause.

### 2. Building the medication plan and adjusting doses

The core drug groups are started gradually and increased over closely spaced visits, with monitoring of blood pressure, pulse, kidney function and electrolytes. This stage needs patience and regular appointments, and any dizziness or drop in blood pressure should be reported to the clinic so the plan can be adjusted rather than stopped on your own.

### 3. Treating the reversible cause

If the cause is a narrowed coronary artery, a valve disorder, a persistent fast rhythm, uncontrolled blood pressure or alcohol, treating that cause itself can improve pumping function appreciably. That is why symptom medication is never prescribed without a search for the underlying cause.

### 4. Assessing the need for a device

After enough time on optimal medical therapy, pumping function is measured again. In selected cases a pacemaker that coordinates chamber contraction or an implantable defibrillator to protect against a lethal rhythm is offered, and the purpose and limits of the device are explained before any decision.

### 5. Follow-up, daily weights and rehabilitation

Symptoms, daily weights, blood pressure and kidney and electrolyte tests are followed, a graded activity or cardiac rehabilitation programme is set, and vaccination, diabetes control and sleep are reviewed. Early signs of deterioration are explained so you can act before you end up in the emergency department.

## About this service

A weakened heart muscle, or cardiomyopathy, is a condition in which the heart muscle loses part of its ability to pump blood to the body, or becomes stiff so that it does not fill properly. The result is the syndrome of heart failure: breathlessness on exertion and later at rest, breathlessness on lying flat or waking at night because of it, swelling of the feet and legs, fatigue and reduced exercise capacity, palpitations, and rapid weight gain from fluid retention.

Looking for the cause is not a luxury, because it changes the whole course: coronary artery disease or a previous heart attack, years of uncontrolled high blood pressure, a valve disorder, viral inflammation of the heart muscle, a persistent fast rhythm disturbance, inherited muscle diseases, alcohol, certain chemotherapy drugs, and weakness related to pregnancy. Assessment therefore includes an ECG and an echocardiogram to measure pumping function, blood tests for kidneys, electrolytes, thyroid and cardiac markers, and sometimes coronary imaging or cardiac MRI.

Treatment runs along three tracks. The first is treating the cause when it is treatable: opening a narrowed artery, repairing a valve, controlling blood pressure, suppressing a rhythm disturbance, stopping alcohol. The second is a medication plan built from core drug groups layered on top of one another and increased gradually while blood pressure, pulse, kidney function and electrolytes are monitored — these are not merely symptom relievers but treatments that slow deterioration of the muscle, with diuretics added to clear fluid retention. The third is device therapy in selected cases: a specialised pacemaker that coordinates contraction of the chambers, or an implantable defibrillator to protect against a dangerous rhythm; in rare advanced cases, higher-level options are explored in specialist centres.

What you do every day is just as much part of the treatment: weighing yourself each morning after the bathroom and before eating and recording it; reducing salt and watching processed foods, pickles and ready-made stocks; keeping to the fluid limit your doctor sets; regular activity or a cardiac rehabilitation programme; stopping smoking; seasonal vaccination; and treating diabetes, snoring and sleep apnoea.

The limits must be clear. Medicines do not grow new muscle. Pumping function may improve substantially, but the diagnosis, the follow-up and the treatment usually continue for life. \*\*The commonest reason patients return to the emergency department is stopping medication once they feel better, or letting salt and fluid intake slip.\*\* No herb, supplement or folk remedy replaces these medicines, and an implanted device does not strengthen the muscle by itself — it protects against an electrical problem or coordinates contraction. And never stop antiplatelet or anticoagulant medication yourself, or before a dental procedure, except on your cardiologist's decision.

## Preparation

Come to the clinic prepared: bring a complete list of your medicines with their names and doses exactly as written on the packs, previous echocardiogram reports, your latest blood results and any old ECG. Record your daily weight in a notebook or on your phone for two weeks beforehand, noting any swelling or night-time breathlessness. Write down your questions, and bring someone with you if there is a lot to take in. Tell the doctor about every supplement or herbal product you use, because some interact with cardiac medicines, and ask before using any painkiller, since some anti-inflammatory drugs worsen fluid retention. \*\*Never stop any of your medicines on your own, and never stop antiplatelets or anticoagulants without consulting your cardiologist — not even before a tooth extraction or a dental cleaning; tell your dentist that you have heart disease and let them contact your doctor.\*\* Ask about seasonal vaccination and about the salt and fluid limits suited to your condition.

## Aftercare

Weigh yourself every morning at the same time in the same clothes and record the number, take your medicines on schedule without interruption even on days when you feel well, keep to your salt and fluid limits, and build up activity at the agreed pace. \*\*Contact your doctor or go to the emergency department immediately for rapid weight gain over a few days (around two kilograms or more), breathlessness at rest or on lying flat or waking at night breathless, increasing swelling of the legs or abdomen, coughing pink frothy sputum, fainting or a racing heartbeat with dizziness, or chest pain.\*\* If you develop dizziness, low blood pressure, a slow pulse or reduced urine output, call the clinic before stopping any medicine yourself — adjusting treatment is a medical decision. And tell any doctor or dentist that you have a weakened heart muscle before a new medicine is prescribed or a procedure performed.
