## Procedure steps

### 1. Imaging with measurement of kidney size and function

Ultrasound is performed to measure the size of each kidney, the thickness of its tissue and any dilatation of the urinary tract or stones, while kidney function tests, urinalysis and albumin measurement are requested and blood pressure is measured carefully. Comparing the scan with any older imaging helps greatly in judging whether the atrophy is recent or long-standing and stable.

### 2. Searching for the cause

The history is reviewed for childhood urinary infections, reflux, recurrent stones, previous operations, prostate enlargement or long-term painkiller use, along with assessment of diabetes and blood pressure. Additional urinary tract imaging or vascular imaging may be needed when renal artery narrowing is suspected, particularly with blood pressure that is hard to control.

### 3. Assessing each kidney separately

When it matters how much each kidney contributes, before a surgical decision or when considering drainage of an obstruction, a nuclear scan is requested to measure each kidney's share of function and its drainage. This information prevents futile procedures on a kidney that has already lost function, and guards against giving up a kidney that is still working.

### 4. Treating the reversible cause and protecting the other kidney

Obstruction is drained when present, recurrent infection or reflux is managed, and dilating a narrowed artery is considered in selected cases. In parallel, the remaining tissue and the opposite kidney are protected by controlling blood pressure and glucose, reducing salt, stopping medicines and substances harmful to the kidneys, and avoiding dehydration.

### 5. Regular monitoring of blood pressure, function and albumin

Function tests, albumin and blood pressure are repeated at intervals set by your physician, and imaging is repeated if symptoms or results change. Patients are reminded that stability is measured in the laboratory, and that one healthy kidney is usually enough for a normal life provided it is protected from harm.

## About this service

Kidney atrophy means that one or both kidneys are reduced in size with loss of part of their functioning tissue, and it is usually discovered on ultrasound. It is not a disease in its own right but the end result of a previous or ongoing problem: urinary obstruction that was not relieved in time, reflux of urine from the bladder to the kidney dating from childhood, narrowing of the renal artery with reduced blood supply, recurrent or chronic kidney infection, long-standing stones, or chronic kidney disease as a complication of diabetes and hypertension.

Many people have no complaints at all, particularly when only one kidney is affected, because the other compensates. The first sign may be blood pressure that is difficult to control, protein in the urine, recurrent urinary infections, chronic flank pain, or a decline in kidney function tests found on a routine check.

The limit must be stated plainly: atrophied tissue does not return, and there is no medicine, herb or device that restores kidney size or activates a shrunken kidney. The aim of treatment is entirely different: protect the tissue that remains, protect the opposite kidney, treat the cause if it is treatable, and control blood pressure, which may itself be driven by the atrophic kidney.

Treatment therefore begins with the search for a cause: imaging to assess kidney size and the urinary tract, tests of function and albumin with urinalysis, blood pressure measurement, and in selected cases vascular imaging or a nuclear scan to estimate how much each kidney contributes to overall function. Treatable causes are then addressed: relieving obstruction, managing reflux or recurrent infection, and in selected cases dilating a narrowed renal artery. Removing the kidney is not routine treatment and is considered only in limited situations such as blood pressure that no medication controls, dangerous recurrent infection, or intractable pain from a kidney with no function.

Protecting the healthy kidney is the priority: avoid long-term non-steroidal anti-inflammatory painkillers, avoid herbal products, stone-dissolving mixtures and supplements of unknown origin, and avoid dehydration and unnecessary contrast imaging. With blood pressure control and monitoring of albumin and function, many people live a normal life with one effective kidney. Suitability for any procedure is determined by clinical assessment and imaging, not by the kidney size quoted in a report.

## Preparation

Bring all previous imaging reports and discs if you have them, because comparing today's kidney size with its size years ago is the single most useful piece of information at the visit. Write out your urinary history in detail: childhood urinary infections, late bedwetting, stones, previous operations or catheterisation, and symptoms of prostate enlargement. Tell your physician about all your medicines, and in particular any frequent use of non-steroidal anti-inflammatory painkillers, herbal products, stone-dissolving mixtures and supplements, stating how long you have used them. Record your home blood pressure for several days before the appointment. Mention any recent contrast imaging or operation, your family history of kidney disease, and whether you are pregnant or planning pregnancy.

## Aftercare

Make blood pressure control your daily priority, measure it at home and record the readings, because it is the strongest protection for the tissue that remains. Drink the amount of fluid your physician advises and avoid dehydration in hot weather, during fasting or with diarrhoea. Reduce salt and processed food, do not use non-steroidal anti-inflammatory painkillers repeatedly, and do not take herbal products, stone-dissolving mixtures or supplements of unknown origin. Treat any urinary infection promptly rather than postponing it, and tell any physician or imaging centre that you have kidney atrophy before any contrast study. Seek urgent care if you develop: \*\*a sharp drop in urine output\*\*, \*\*sudden swelling of the face and feet\*\*, \*\*blood in the urine\*\*, \*\*fever with flank pain\*\*, severe flank pain, burning on passing urine with shivering, or high blood pressure with severe headache or visual change.
