أكثر أسباب الطنين شيوعاً ضعف السمع المرتبط بالعمر أو بالتعرّض المتكرّر للضجيج العالي، ثم أسباب بسيطة قابلة للعلاج مثل سدادة شمع أو التهاب أذن أو سائل خلف الطبلة أو خلل في وظيفة القناة السمعية بعد الرشح والسفر. وقد يرتبط الطنين باضطراب المفصل الصدغي الفكّي وشدّ عضلات الرقبة، أو بفقر الدم واضطراب الغدة الدرقية، أو بأدوية معروفة بتأثيرها على الأذن، أو بأمراض الأذن الداخلية والدوار الدهليزي، أو بأسباب وعائية في حالة الطنين النابض. ولذلك يبدأ التقييم بقصّة سريرية دقيقة وفحص للأذن وتخطيط سمع، وقد يُضاف تحليل أو تصوير عند وجود دواعٍ محدّدة.

أما العلاج فمركّب لا دواء واحد. يُعالَج السبب إن وُجد: يُرفع الشمع، ويُعالَج الالتهاب أو السائل، وتُراجَع الأدوية بالتنسيق مع الطبيب الواصف. ثم يُصحَّح السمع بالمعينات السمعية عند وجود ضعف، وهي من أنفع ما يخفّف الطنين لأنها تُعيد إلى الدماغ الأصوات المحيطة التي فقدها. ويُستخدم العلاج الصوتي وأجهزة إخفاء الطنين والأصوات الخلفية الهادئة، والعلاج المعرفي السلوكي وبرامج إعادة تدريب الطنين لكسر حلقة الانتباه والقلق، إلى جانب تحسين النوم وتقليل الكافيين والنيكوتين وتجنّب الضجيج.

وحدود العلاج يجب أن تكون واضحة: \*\*لا يوجد دواء يُوقف الطنين، ولا حقنة ولا مكمّل غذائي مُثبت يُزيله\*\*، والهدف الواقعي غالباً هو تقليل الانزعاج واستعادة النوم والتركيز وتحسين السمع، لا إطفاء الصوت كزرّ. والعلاج لا يُعيد خلايا السمع التالفة، ولهذا فإن حماية الأذن من الضجيج أهمّ من أي علاج بعده. وأي إعلان يبيع «شفاءً نهائياً من الطنين» يستحقّ الحذر.

\*\*راجع الطبيب فوراً إذا فقدت السمع فجأة في أذن واحدة خلال ساعات إلى أيام؛ فهذا طارئ سمعي تتحسّن نتائجه كثيراً مع التقييم المبكر.\*\* وراجع سريعاً إن كان الطنين في أذن واحدة فقط، أو نابضاً يوافق النبض، أو مصحوباً بدوار شديد أو ترنّح أو ضعف في عضلات الوجه أو صداع جديد، أو ظهر بعد إصابة رأس أو انفجار. وعند الأطفال تحديداً: إن شكا طفلك من طنين، أو لاحظت أنه لا يستجيب للصوت، أو يرفع صوت التلفاز، أو تأخّر كلامه، فاطلب تقييم سمع فوراً — \*\*تأخير كشف ضعف السمع يؤخّر اللغة، ولا تنتظر أن يتكلّم لوحده\*\*.

## خطوات الإجراء

### 1. القصة السريرية وفحص الأذن

يسأل الطبيب عن صفة الطنين ومتى بدأ، وهل هو في أذن واحدة أم كلتيهما، مستمرّ أم نابض، وما يزيده ويقلّله، وعن الضجيج والأدوية وإصابات الرأس والدوار. ثم يُنظَر في قناة الأذن والطبلة، ويُرفع الشمع إن وُجد سدّ.

### 2. فحوصات السمع والتوازن

يُجرى تخطيط سمع نقي لتحديد وجود ضعف سمع ودرجته ونوعه، مع قياس انضغاط الطبلة ومنعكس العضلة، وقد يُقاس تردّد الطنين وشدّته. تُضاف فحوصات التوازن إن كان هناك دوار أو ترنّح مصاحب.

### 3. البحث عن سبب قابل للعلاج

تُطلب تحاليل مثل صورة الدم ووظائف الغدة عند الاشتباه، ويُقيَّم المفصل الصدغي الفكّي وعضلات الرقبة. أما الطنين في أذن واحدة أو النابض أو المصحوب بأعراض عصبية فقد يستدعي تصويراً مناسباً لاستبعاد أسباب محدّدة.

### 4. خطة علاجية مركّبة

يُعالَج السبب إن وُجد، ويُصحَّح السمع بمعين سمعي عند الحاجة، ويُوصف علاج صوتي أو جهاز إخفاء، ويُضاف علاج معرفي سلوكي أو إعادة تدريب للطنين لتخفيف الانزعاج. تُراجع الأدوية المؤثّرة على الأذن مع الطبيب الواصف ويُعالَج القلق واضطراب النوم.

### 5. المتابعة وحماية السمع

تُقاس الاستجابة بمقياس انزعاج ونوم لا بشدّة الصوت وحدها، وتُعدَّل الخطة كل عدة أسابيع. يُدرَّب المريض على حماية سمعه بسدادات في الأماكن الصاخبة، وضبط مستوى الصوت في السمّاعات، وتجنّب الصمت المطلق باستخدام صوت خلفي خفيف.

### 1. Clinical history and ear examination

The doctor asks about the character of the noise and when it began, whether it is in one ear or both, constant or pulsatile, what makes it better or worse, and about noise exposure, medicines, head injury and dizziness. The ear canal and eardrum are then examined, and any obstructing wax is removed.

### 2. Hearing and balance testing

Pure tone audiometry establishes whether hearing loss exists and defines its degree and type, together with tympanometry and reflex testing, and the pitch and loudness of the tinnitus may be matched. Balance testing is added when vertigo or unsteadiness accompanies the noise.

### 3. Looking for a treatable cause

Blood tests such as a full blood count and thyroid function are requested when suspected, and the temporomandibular joint and neck muscles are assessed. Unilateral or pulsatile tinnitus, or tinnitus with neurological symptoms, may warrant appropriate imaging to exclude specific causes.

### 4. A combined treatment plan

Any cause found is treated, hearing is corrected with an aid where needed, sound therapy or a masking device is prescribed, and cognitive behavioural therapy or tinnitus retraining is added to reduce distress. Medicines that affect the ear are reviewed with the prescribing doctor, and anxiety and insomnia are addressed.

### 5. Follow-up and hearing protection

Response is measured by distress and sleep scores rather than by loudness alone, and the plan is adjusted every few weeks. The patient is taught to protect hearing with earplugs in noisy places, to keep headphone volume moderate, and to avoid total silence by using gentle background sound.

## العناية بعد الإجراء

التزم بالخطة كاملة لا بجزء منها؛ فالمعين السمعي أو العلاج الصوتي يحتاج أسابيع من الاستخدام المنتظم قبل الحكم عليه. لا تجلس في صمت مطلق لأنه يزيد الانتباه للطنين، واستخدم صوتاً خلفياً هادئاً في البيت وعند النوم. احمِ سمعك بسدادات في الأماكن الصاخبة والأعراس والورش، واخفض صوت السمّاعات ولا تنم بها. حسّن نومك وقلّل الكافيين والنيكوتين، وتعامل مع القلق فالتوتّر يضخّم إدراك الصوت. لا تُدخل شيئاً في أذنك ولا تستخدم قطرات أو زيوتاً بلا وصفة، ولا تبدأ مكمّلات إعلانية. سجّل مستوى الإزعاج أسبوعياً لتلاحظ الاتجاه، وراجع طبيبك في مواعيده لتعديل الخطة. \*\*راجع فوراً عند: فقدان سمع مفاجئ في أذن واحدة، طنين نابض جديد، دوار شديد أو ترنّح أو قيء، ضعف في عضلات الوجه، صداع جديد شديد، إفراز أو ألم أو حرارة في الأذن، أو طنين ظهر بعد إصابة رأس.\*\*

## About this service

Tinnitus is the perception of a sound with no external source: ringing, buzzing, whistling, hissing, or a pulsing noise that follows the heartbeat. It is a symptom rather than a disease in itself, so the first step is not to silence the sound but to search systematically for its cause, because real treatment starts from the cause and not from the ear alone.

The most common driver is hearing loss related to age or to repeated exposure to loud noise. Beyond that lie simple and treatable causes such as a wax plug, ear infection, fluid behind the eardrum, or eustachian tube dysfunction after a cold or a flight. Tinnitus may also be linked to temporomandibular joint disorder and neck muscle tension, to anaemia or thyroid disturbance, to medicines known to affect the ear, to inner ear disease and vestibular disorders, or to vascular causes in the case of pulsatile tinnitus. Assessment therefore begins with a careful history, examination of the ear and a hearing test, with blood tests or imaging added only when there is a specific indication.

Treatment is a combination rather than a single medicine. Any identified cause is treated: wax is removed, infection or fluid is managed, and medicines are reviewed together with the doctor who prescribed them. Hearing is then corrected with hearing aids where loss exists, and this is among the most effective ways of easing tinnitus, because it restores the surrounding sounds the brain has lost. Sound therapy, maskers and gentle background noise are used, along with cognitive behavioural therapy and tinnitus retraining programmes that break the cycle of attention and anxiety, together with better sleep and less caffeine, nicotine and noise exposure.

The limits must be equally clear: \*\*no medicine stops tinnitus, and no injection or proven supplement removes it\*\*. The realistic goal is usually to reduce distress and restore sleep and concentration while improving hearing, not to switch the sound off like a button. Treatment does not regenerate damaged hearing cells, which is why protecting the ears from noise matters more than any later therapy, and any advertisement selling a permanent cure for tinnitus deserves scepticism.

\*\*See a doctor immediately if hearing drops suddenly in one ear over hours to days, because this is an ear emergency whose outcome improves substantially with early assessment.\*\* Seek prompt review if tinnitus affects one ear only, pulses in time with your heartbeat, or comes with severe vertigo, unsteadiness, facial weakness or a new headache, or if it began after a head injury or a blast. In children in particular: if your child reports noise in the ear, does not respond to sound, turns the television up, or is late to speak, ask for a hearing assessment at once. \*\*Delaying the detection of hearing loss delays language, so never wait for a child to start speaking on their own.\*\*

## Preparation

Before your visit, write a precise description of the noise: when it started, whether it is in one ear or both, its quality, whether it pulses with your heartbeat, and what makes it worse such as noise, poor sleep or caffeine. Rate how much it bothers you out of ten and how it affects your sleep and concentration, because this is the measure against which improvement will be judged. Bring a written list of all your medicines and supplements, since some affect the ear, and stop nothing on your own. Bring any previous hearing test, report or imaging. Avoid loud noise and do not use headphones at high volume before the appointment, as this distorts the results. Do not put cotton buds, oils or water into the ear beforehand, and tell the doctor about any head injury, diving, recent air travel or blast exposure.

## Aftercare

Follow the whole plan rather than part of it: a hearing aid or sound therapy needs weeks of regular use before it can be judged. Do not sit in total silence, which sharpens attention to the noise; use gentle background sound at home and at bedtime. Protect your hearing with earplugs in loud places, weddings and workshops, keep headphone volume low and never sleep wearing them. Improve your sleep, cut caffeine and nicotine, and treat anxiety, because tension amplifies how loudly the sound is perceived. Put nothing into your ear and use no drops or oils without a prescription, and do not start advertised supplements. Score your distress weekly so you can see the trend, and keep your review appointments so the plan can be adjusted. \*\*Seek immediate care for sudden hearing loss in one ear, new pulsatile tinnitus, severe vertigo or unsteadiness or vomiting, facial muscle weakness, a new severe headache, ear discharge or pain or fever, or tinnitus that began after a head injury.\*\*
